Successful treatment of eosinophilic esophagitis with upadacitinib prescribed for atopic dermatitis.

We describe a pediatric patient treated with upadacitinib for atopic dermatitis (AD) who subsequently achieved sustained clinical and histologic remission of eosinophilic esophagitis (EoE). Upadacitinib is an oral small molecule selective Janus kinase 1 inhibitor that inhibits janus kinase-signal transduction and activation of transcription (JAK-STAT) pathway with wide inhibitory effects on the immune system, including Th2 signaling pathways. Therefore, it leads to broad suppression of inflammatory cytokines, including interleukin-4 (IL-4), interleukin-13 (IL-13), and thymic stromal lymphopoie…

Topical Therapy for Atopic Dermatitis: What is New and the New Paradigm.

Atopic dermatitis (AD) is a chronic inflammatory skin disorder that requires a complex management strategy, which often involves multiple and diverse topicals and systemic treatment regimens. While topical steroids and more recently calcineurin inhibitors have been the mainstay therapy for mild-to-moderate disease, recent advances in the understanding of AD pathogenesis have led to the development of different new targets, rapidly widening our therapeutic armamentarium. This review summarizes their efficacy and safety data. We also review topical optimization strategies, including the recently…

Psoriasis and Atopic Dermatitis Overlap: Pathogenesis and Therapeutic Considerations for the Clinician.

Psoriasis (PsO) and atopic dermatitis (AD) are chronic, inflammatory skin diseases with distinct, yet occasionally overlapping, immunological profiles. PsO is characterized by T helper (Th) 1/Th17 immune pathways, while AD predominantly involves Th2 responses. However, a subset of patients exhibit features of both conditions, presenting a diagnostic and therapeutic challenge. This review aims to comprehensively assess the pathogenesis, clinical presentation, and therapeutic considerations of the PsO-AD overlap phenotype. This unique clinical entity is characterized by mixed Th17/Th2 immune res…

The role and management of dietary and environmental factors in pediatric atopic dermatitis.

BACKGROUND: The pathogenesis of pediatric atopic dermatitis (AD) is complex, and the role of dietary and environmental factors in its onset and progression warrants systematic investigation. OBJECTIVE: To explore the associations between dietary allergen sensitization, environmental factors, and the development of pediatric AD through a retrospective case-control analysis, and to evaluate the clinical efficacy of a multidimensional intervention strategy. METHODS: This study employed a two-stage design. The first stage was a retrospective case-control study, enrolling 535 pediatric AD patient…

Evaluating Medicaid Coverage for Over-the-Counter Treatments for Diaper Dermatitis.

Diaper dermatitis is a common dermatologic condition in infancy, frequently managed with over-the-counter (OTC) products. We conducted a cross-sectional review of Medicaid Preferred Drug Lists across 49 states and the District of Columbia to evaluate coverage of 14 commonly used OTC diaper dermatitis treatments. Coverage varied widely by state and product class, with therapeutic agents, such as topical antifungals, low potency corticosteroids, and antibiotics, more frequently covered than preventive barrier products, such as petrolatum and zinc oxide. These findings reveal significant state-le…

Gut microbiota contributes to the therapeutic effect of acupuncture in atopic dermatitis.

UNLABELLED: The gut microbiome is increasingly recognized as a central regulator of immune homeostasis, metabolic balance, and therapeutic outcomes. Atopic dermatitis (AD), a chronic inflammatory skin disease, is closely linked to gut microbial dysbiosis. Traditionally regarded as a neurostimulatory therapy, acupuncture (Acu) has demonstrated increasing efficacy in alleviating AD symptoms and improving gastrointestinal function. These observations suggest that the therapeutic effects of Acu in AD may be mediated, in part, by modulation of the gut microbiome. In this study, AD patients were str…

Atopic Dermatitis and Markers of Early Cardiovascular Risk in Children and Adolescents.

IMPORTANCE: Early identification of individuals at elevated risk of cardiovascular disease and preventive treatment during childhood may reduce cardiovascular disease in adults with chronic inflammatory diseases. Children with atopic dermatitis (AD) may have elevated cardiovascular risk, but studies to date have not accounted for heterogeneity in disease activity and severity across childhood. OBJECTIVES: To evaluate whether active and more severe AD across childhood and adolescence are associated with cardiovascular risk. DESIGN, SETTING, AND PARTICIPANTS: This longitudinal cohort study use…

Tofacitinib-Associated Acne in a Young Patient With Severe Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease that may require systemic therapy in patients with moderate-to-severe or refractory disease. Tofacitinib, a Janus kinase (JAK) inhibitor, has demonstrated efficacy in the treatment of severe inflammatory dermatoses, including refractory AD. However, its use may be associated with various adverse events, and acneiform eruptions have increasingly been reported with JAK inhibitor therapy. We report the case of an 18-year-old female patient with severe AD who was treated with tofacitinib, resulting in marked improvement of her eczematou…

Dermatitis Herpetiformis in Celiac Disease: A Systematic Review and Meta-Analysis.

Dermatitis herpetiformis (DH) shares a pathophysiological basis with celiac disease (CD), but its epidemiology remains poorly defined. This systematic review and meta-analysis estimated the prevalence of DH among CD patients. Following the PRISMA 2020 guidelines, PubMed, Embase, Web of Science, and Scopus were searched. Observational studies with extractable DH data in CD patients were pooled using a random-effects model, with subgroup and meta-regression analyses. The risk of bias was assessed using the JBI Checklist. Of 7271 records, 24 studies published between 1996 and 2023, comprising 73,…

Enteral Green Beans in the Management of Neonatal Irritant Diaper Dermatitis.

Irritant diaper dermatitis (IDD) is frequently observed in neonatal intensive care units and is often exacerbated by chronic loose stools. Despite standard treatments, erosive or persistent IDD remains challenging to manage. We introduced pureed green beans into enteral feeds as a source of fiber to improve stool consistency and reduce skin breakdown. This intervention has been highly effective as an easy-to-use and safe non-topical intervention for treating IDD.

Skin Barrier Dysfunction in Contact and Atopic Dermatitis: A Comparative Review.

The epidermis, the outermost layer of the skin, consists of 5 distinct layers, with the stratum corneum (SC) serving as the primary barrier. Disruption of skin barrier leads to inflammatory skin conditions, including irritant contact dermatitis (ICD), allergic contact dermatitis (ACD), and atopic dermatitis (AD). ICD arises from direct damage to the SC by irritants, while ACD is mediated by a type IV hypersensitivity. AD is characterized by chronic inflammation and genetic defects that compromise barrier integrity. Accordingly, strategies aimed at barrier restoration, such as the use of cerami…

Clinical Severity and Outcome Measures in Seborrheic Dermatitis: A Systematic Review.

INTRODUCTION: Seborrheic dermatitis (SD) is a common chronic inflammatory dermatosis associated with substantial symptom burden and impaired quality of life. Despite increasing interest in development of advanced therapies for SD, standardization of outcome measures appears limited. We sought to understand how disease severity, symptoms, and quality of life are measured in seborrheic dermatitis clinical studies. METHODS: We conducted a systematic review of clinical studies (both interventional and observational) assessing SD severity and treatment response. Eligible studies were identified th…

Overlapping mycoses in a case of Seborrheic dermatitis with coexistent Malassezia folliculitis.

Seborrheic dermatitis (SD) is a chronic inflammatory skin condition likely caused by several factors, including overgrowth of the lipophilic yeast Malassezia, increased sebaceous gland activity, and a potential genetic predisposition, whereas Malassezia folliculitis (MF) is a fungal infection of hair follicles. In this case, a 22-year-old male initially presented with erythematous papules on the malar region and was clinically diagnosed with SD. Following partial improvement with topical and oral antifungals, the patient developed new pustular lesions involving the malar and chin regions, rais…

Cross-sectional Study of Dermatitis Herpetiformis with Focus on Neurological Comorbidities.

Dermatitis herpetiformis (DH) is a chronic autoimmune blistering skin disease strongly associated with coeliac disease. This cross-sectional study investigated the clinical, serological, and neurological characteristics of 114 DH patients (70 males, 44 females; mean age: 50.2 years; 13 newly, 101 previously diagnosed) at Semmelweis University, Budapest, between 2020 and 2023. Diagnosis was confirmed by direct immunofluorescence microscopy. Serum autoantibodies against transglutaminase 2 (TG2), 3 (TG3), and 6 (TG6) were quantified using immunoassays. While at the time of evaluation, 65.8% of pa…

Phenotypic and genomic study of digital dermatitis in UK Holstein heifers.

BACKGROUND: Digital dermatitis (DD) is a global infectious hoof disease causing lameness in cattle. The condition has been studied extensively in milking cows. The present study deployed data from repeated clinical examinations of young heifers to develop DD phenotypes reflecting the disease prevalence and progression. The newly defined traits were then subjected to single-step genome-wide association analyses to assess their genetic profile. Data were collected during four monthly clinical examinations of 921 Holstein heifers aged less than 15 months. The M-score system was used to assess hoo…
