Perforated Meckel's Diverticulitis in a Patient with Unknown Intestinal Malrotation: Clinical Pitfall.

Symptomatic Meckel's diverticulum is rare in adults. The most frequent complications are intestinal obstruction and diverticulitis. Diagnosis of Meckel's diverticulitis can be challenging due to nonspecific clinical manifestation of pain in the right lower abdominal quadrant, mimicking acute appendicitis. If associated with congenital malformation, such as intestinal malrotation, the anomalous anatomy makes the diagnosis even more challenging. In such cases, radiological imaging is essential to guide further management. We present a case of Meckel's diverticulitis in which physicians were init…

Perforated Jejunal Diverticulitis.

Small intestinal diverticula are very rare; their incidence ranges from 0.06 to 1.3%, with a higher prevalence after the 6th decade of life. Among these small intestinal diverticula, duodenal diverticula are more frequent, followed by diverticula of the jejunum and ileum. A jejunal diverticulum is usually asymptomatic; sometimes patients complain of vague chronic symptoms like malabsorption, pain, or nausea that easily lead to misdiagnosis. Complications are rarely reported, only in 10% of patients. We report a unique case of a 70-year-old female who presented with confusion due to sepsis from…

Lower Gastrointestinal Bleeds: False-Negative and False-Positive Tests.

Lower gastrointestinal bleeding in infants and children is a symptom with a broad differential diagnosis. The diagnosis depends on the quantity and quality of the bleeding and on the patient's age. We describe a case of a 10-month-old infant who presented to our pediatric emergency department 4 times with lower gastrointestinal bleeding before a diagnosis of Meckel diverticulum was made. Although this pathology was strongly suspected at an earlier visit, the initial Meckel scan result was interpreted as normal, and the result of a stool test for Clostridium difficile was positive. The case ill…

Bile Salt Enterolith: An Unusual Etiology Mimicking Gallstone Ileus.

Primary enterolithiasis is a relatively uncommon but important cause of small bowel obstruction. We present a case of a 69-year-old male with a history of laparoscopic Roux-en-Y gastric bypass and asymptomatic duodenal diverticulum diagnosed with small bowel obstruction. CT imaging showed an obstruction distal to the jejunojejunostomy, and surgical intervention was warranted. A 4.5 cm enterolith removed from the distal jejunum was found to contain 100% bile salts, consistent with a primary enterolith. Clinicians should retain a high index of suspicion for enteroliths as a cause of small bowel …

[Early Diagnosis of Squamous Cell Carcinoma of the Urinary Bladder Reached by Routine Abdominal Ultrasonography in a Patient on Self-Catheterization : A Case Report].

A 69-year-old woman was referred to our department for refractory cystitis. Incomplete emptying of the bladder was observed, and symptoms did not improve after medication. Clean intermittent catheterization was initiated. Two years after introduction of catheterization, a mass in the bladder dome was detected by routine abdominal ultrasonography. She was diagnosed with malignant tumor of the bladder diverticulum based on cystoscopy, computed tomography, and magnetic resonance imaging. Partial cystectomy and lymph node dissection was performed following transurethral resection of the bladder tu…

Spontaneous regression of bronchogenic cyst accompanied by pneumonia.

Bronchogenic cysts arise from abnormal budding of the ventral diverticulum of the foregut or tracheobronchial tree during embryogenesis, are the most common cystic masses in the mediastinum, and are generally asymptomatic. A spontaneous regression in a mediastinal bronchogenic cyst (MBC) with pneumonia is rare. A 30-year-old male had a tumor shadow in the middle mediastinum. When he visited our hospital, he had a mild fever with coughing and sputum. A chest computed tomography (CT) scan showed a decrease in the tumor size and the existence of right pneumonia. MBC may be involved in the etiolog…

Patent vitellointestinal duct.

During the 3rd week of intrauterine life there is a communication between the intraembryonic gut and the yolk sac. As the development proceeds this communication narrows into a tube known as the vitellointestinal duct (VID). With the establishment of placental nutrition this duct usually becomes obliterated by the end of the 7th week of intrauterine life. In about 2% of humans this duct persists and gives rise to a group of anomalies of which Meckel's diverticulum is the commonest and complete patency of the duct is the rarest.1 We report a case of a 4-month-old infant who presented with umbil…

[Guidelines for the conventional surgical indications for urinary stress incontinence in women (colposuspension, aponeurotic support of the bladder neck].

The equivalence of the functional results of the suburethral sling and open colposuspension and the greater morbidity of colposuspension is such that the indications for traditional surgery correspond to the contraindications for suburethral sling: there no longer seem to be but few indications for supporting the bladder neck using an aponeurotic swing; caution recommends preferring colposuspension when the functional result of placing a suburethral sling risks being compromised by either vaginal trophicity abnormalities or previous repair to the urethra such as suburethral diverticulum or ure…

Primary urethral malignancy: review of 22 cases.

Twenty-two cases of primary urethral malignancy have been diagnosed at the Medical University Hospital from 1955 to 1976. These lesions are uncommon and prognosis is poor except when disease is diagnosed in the early stages and treated aggressively. Anterior lesions are more often diagnosed at an early stage but may be mistaken for benign processes, delaying appropriate therapy. Our experience with urethral malignancies is reviewed and includes an association of carcinoma with urethral caruncles and diverticula. The first reported association of a urethral diverticulum with carcinoma in the ma…

Small-bowel diverticulosis: perceptions and reality.

BACKGROUND: Small-bowel diverticulosis is a rare entity that can be discovered incidentally during celiotomy, endoscopy, or radiographic imaging studies. The reported complication rate is low, giving rise to the current recommendation not to treat uncomplicated small-bowel diverticula. STUDY DESIGN: A retrospective review was performed of patients with small-bowel diverticulosis seen during 23 years at three major institutions. RESULTS: Two hundred eight patients were identified. Diverticula were located in the duodenum in 79 percent; in the jejunum or ileum in 18 percent; and in duodenum, j

Diverticular disease of the cecum and ascending colon. A review of 881 cases.

A total of 881 cases of diverticula of cecum and the ascending colon were reviewed, including 30 new cases at the authors' institution. The … colectomy. The overall mortality rate was 2.5 per cent. The authors advocate local resection of the diverticulum, when possible, and a right hemicolectomy or segmental resection when the diagnosis is in doubt or when local resection or invagination will jeopardize the ileocecal valve or the blood supply to the intestine.

Skene's Gland Abscess: A Case Report and Narrative Review.

Skene's gland cysts and abscesses are uncommon periurethral lesions that can cause chronic vulvar pain and lower urinary tract symptoms but are often underrecognized because their manifestations overlap with more common urological and gynecological conditions. Owing to their close anatomical relationship with the urethra, accurate preoperative diagnosis is essential to distinguish these lesions from urethral diverticulum and avoid inappropriate treatment or urethral injury. We report the case of a 25-year-old woman with a 12-month history of chronic vulvar pain and urinary symptoms. Physical e…
