When hearing loss mimics autism: Enlarged vestibular aqueduct syndrome in an 11-year-old child-A case report.

The relationship between auditory input and cognitive development is fundamental, as early sensory deprivation can impair social communication and mimic neurodevelopmental disorders. Comprehensive audiological assessment is critical for children suspected of autism spectrum disorder to prevent "diagnostic overshadowing," where behavioral symptoms of hearing loss-such as social withdrawal and speech delay-are mislabeled as neurodevelopmental deficits. Although autism spectrum disorder involves social communication deficits under DSM-5 criteria, congenital progressive sensorineural hearing loss

Bilateral Superior Semicircular Canal Dehiscence Presenting as Progressive Gait Instability in a Patient With Parkinsonism and Autoimmune Features.

Superior semicircular canal dehiscence (SSCD) is a vestibular disorder caused by a defect in the bony roof of the superior semicircular canal, resulting in a third mobile window in the inner ear. This structural abnormality alters vestibular and auditory processing, producing symptoms such as vertigo, imbalance, sound sensitivity, autophony, and oscillopsia. We describe a 65-year-old man with a longstanding diagnosis of atypical Parkinsonism who presented with progressive gait apraxia, disequilibrium, and cognitive slowing. Despite undergoing bilateral deep brain stimulation (DBS) for tremor,

Exploring the role of infected keratinocytes during rabies virus infection.

Rabies virus (RABV), a lyssavirus causing fatal encephalitis, is primarily transmitted via dog bites, though superficial exposures such as scratches or bat bites can also result in infection. The mechanisms underlying transmission through these minor exposures remain unclear. This study investigated the involvement of epidermal keratinocytes in RABV entry. RABV antigen was detected in keratinocytes at inoculation sites in experimentally infected mice and at potential viral entry sites in naturally infected dogs. However, keratinocyte infection could not be replicated in ex vivo skin biopsies f

Sudden Sensorineural Hearing Loss Post Coronavirus Disease: A Systematic Review of Case Reports.

The coronavirus disease of 2019 is a global pandemic disease severely affecting the upper respiratory tract that can be fatal in some instances. The virus most commonly affects the respiratory system. However, in certain cases it affects the other systems, including cardiovascular, renal, gastrointestinal, neurological, and auditory. Concerning the hearing and balance system, the microcirculation supply to the inner ear is hampered thus causing audiovestibular symptoms. Several case studies have reported sudden sensorineural hearing loss post-coronavirus disease and its detrimental impact on o

Keratitis-ichthyosis-deafness syndrome with lethal p.Ala88Val variant and severe hypercalcemia.

Keratitis-ichthyosis-deafness (KID) syndrome is a rare genetic disease caused by pathogenic variants in connexin 26 (gene GJB2), which is part of the transmembrane channels of the epithelia. Connexin 26 is expressed mainly in the cornea, the sensory epithelium of the inner ear, and in the skin keratinocytes, which are the three main target organs in KID syndrome. Approximately a dozen pathogenic variants have been described to date, including some lethal forms. Patients with lethal pathogenic variants present with severe symptoms from birth and die from sepsis during the first year of life. We

Combined Endoscopic/Microscopic Cochlear Implantation Through the Oval Window.

Standard round window (RW) cochlear implantation is a well-described technique. Implantation might be difficult in patients with inner and middle ear anomalies, in some cases because of not achieving adequate exposure to the RW, with a related higher risk of complications such as facial nerve injury. It is proposed a combined microscopic/endoscopic oval window approach in a 63 year old man affected by bilateral Menière disease, with bilateral severe sensorineural hearing loss, speech discrimination score for bysillabic words under 40% and a hidden RW by anomalous facial nerve course. Are diuretics effective for Ménière`s disease?

INTRODUCTION: Ménière`s disease is an inner ear disorder characterized by episodes of spontaneous vertigo, fluctuating hearing loss and tinnitus. Diuretics have been widely used for the treatment of attacks, but there is controversy about their effectiveness. METHODS: To answer this question we used Epistemonikos, the largest database of systematic reviews in health, which is maintained by screening multiple information sources, including MEDLINE, EMBASE, Cochrane, among others. We extracted data from the systematic reviews, reanalyzed data of primary studies, conducted a meta-analysis and ge

Loss of liver kinase B1 causes planar polarity defects in cochlear hair cells in mice.

The tumor suppressor gene liver kinase B1 (LKB1), also called STK11, encodes a serine/threonine kinase. LKB1 plays crucial roles in cell differentiation, proliferation, and polarity. In this study, LKB1 conditional knockout mice (LKB1Pax2 CKO mice) were generated using Pax2-Cre mice to investigate the function of LKB1 in inner ear hair cells during early embryonic period. LKB1Pax2 CKO mice died perinatally. Immunofluorescence and scanning electron microscopy revealed that stereociliary bundles in LKB1Pax2 CKO mice were clustered and misoriented, respectively. Moreover, ectopic distribution of

A case of patulous Eustachian tube associated with dizziness induced by nasal respiration.

OBJECTIVE: To determine the relationship between Eustachian tube function and inner ear function in patulous Eustachian tube (pET). METHODS: We encountered a patient with pET accompanied by dizziness that was induced by nasal respiration. Eye movements were recorded using video-oculography, and Eustachian tube function was assessed using a Eustachian tube function analyzer. Horizontal and vertical components of pupil position were assessed to test fixation, positional, and positioning nystagmus. Impedance testing with a Eustachian tube function analyzer was performed to confirm tympanometry r

Type II spiral ganglion afferent neurons drive medial olivocochlear reflex suppression of the cochlear amplifier.

The dynamic adjustment of hearing sensitivity and frequency selectivity is mediated by the medial olivocochlear efferent reflex, which suppresses the gain of the 'cochlear amplifier' in each ear. Such efferent feedback is important for promoting discrimination of sounds in background noise, sound localization and protecting the cochleae from acoustic overstimulation. However, the sensory driver for the olivocochlear reflex is unknown. Here, we resolve this longstanding question using a mouse model null for the gene encoding the type III intermediate filament peripherin (Prph). Prph((-/-)) mice

Progressive hearing loss following acquired cytomegalovirus infection in an immunocompromised child.

We report a rare case of progressive hearing loss after acquired CMV infection in a child with Langerhans cell histiocytosis (LCH). A 5-month-old female was diagnosed as having LCH. When she was 14 months old, she received an unrelated donor umbilical cord blood transfusion for the treatment of intractable LCH. CMV infection was confirmed after the blood transfusion. Because her own umbilical cord had no CMV, the CMV infection was not congenital. When she was 7 years old, mixed hearing loss was noted with bilateral otitis media with effusion. After that time, the sensorineural hearing loss pro

Purines and sensory nerves.

P2X and P2Y nucleotide receptors are described on sensory neurons and their peripheral and central terminals in dorsal root, nodose, trigeminal, petrosal, retinal and enteric ganglia. Peripheral terminals are activated by ATP released from local cells by mechanical deformation, hypoxia or various local agents in the carotid body, lung, gut, bladder, inner ear, eye, nasal organ, taste buds, skin, muscle and joints mediating reflex responses and nociception. Purinergic receptors on fibres in the dorsal spinal cord and brain stem are involved in reflex control of visceral and cardiovascular activ

Augmented ototoxic effect of cisplatin in heterozygotes of the German waltzing guinea pig.

It has previously been demonstrated that the carriers of the German waltzing guinea pig are less susceptible to noise trauma. To explore whether this represents a general resistance to inner ear trauma, carriers of the German waltzing guinea pig were exposed to the ototoxic agent cisplatin. Two doses of cisplatin were injected intravenously into anesthetized carriers and weight-matched control animals. Prior to and 96 h after the injections hearing thresholds were established by recording the auditory brainstem responses at 3.5, 7, 14, and 28 kHz. The cochleae were harvested to estimate hair c

The petrosal approach with hearing preservation.

Twenty-six patients with petroclival lesions were operated on via a petrosal approach designed to preserve hearing. The surgical pathology included 14 meningiomas, three chordomas, three epidermoid cysts, four vertebrobasilar aneurysms, and two pontine cavernous malformations. The approach allowed complete resection of 14 of 20 tumors and definitive treatment of all six vascular lesions. Complications included cerebrospinal fluid leakage in three patients, high-frequency sensorineural hearing loss in three, meningitis in one, and cranial nerve palsies (which were usually transient). This appro

[Retrocochlear hearing disorders].

Sensorineural hearing loss is a common disorder that results from damage to the inner ear in over 95% of all cases; therefore, retrocochlear hearing disorders are rare and cannot be differentiated from sensory losses by clinical symptoms alone. Associated vestibular and other neurotological symptoms must lead to appropriate diagnostic procedures. Besides audiological and neurotological tests, e.g. auditory-evoked potentials, otoacoustic emissions or electronystagmography, modern imaging methods are the most valuable tools to detect tumorous, vascular and inflammatory processes. Acoustic neurom

Bone repair in the otic capsule of the rabbit.

The repair of 1 mm drill hole lesions in rabbit long bones and otic capsules was studied in undecalcified sections. Following surgery, bone formation was monitored during 12 weeks by intravital sequential time labeling with bone-seeking fluorochromes. In diaphyseal bone, initial repair by trabecular woven bone was accompanied by a transient remodeling response in primary repair bone and the surrounding compacta. In comparison, capsular defects showed a slightly deficient initial repair by woven bone which, once formed, persisted unchanged with no subsequent osteonal remodeling. In the surround
