Maximizing outcomes in the treatment of radial head fractures.

The radial head plays a critical role in the stability of the elbow joint and its range of motion. Injuries may occur across a spectrum of severity, ranging from low energy non-displaced fractures to high energy comminuted fractures. Multiple classification systems exist to help characterize radial head fractures and their associated injuries, as well as to guide treatment strategies. Depending on the type of fracture, non-operative management may be possible if early range of motion is initiated. Other options include open reduction and internal fixation or excision followed by arthroplasty. …

Radial head button holing: a cause of irreducible anterior radial head dislocation.

"Buttonholing" of the radial head through the anterior joint capsule is a known cause of irreducible anterior radial head dislocation associated with Monteggia injuries in pediatric patients. To the best of our knowledge, no report has described an injury consisting of buttonholing of the radial head through the annular ligament and a simultaneous radial head fracture in an adolescent. In the present case, the radiographic findings were a radial head fracture with anterior dislocation and lack of the anterior fat pad sign. Magnetic resonance imaging (MRI) clearly demonstrated anterior dislocat

[Osteosynthesis in distal intra-articular humeral fractures. External fixation with motion capacity].

The rising incidence of distal intra-articular humeral fracture in elderly patients associated with osteoporosis frequently leads to difficulties in acute reconstruction and may result in early failure of internal fixation. The biomechanics of the elbow joint and especially the flexor group as well as the brachioradialis muscle as part of the extensor group is analyzed. The complex motion pattern of the transarticular muscle groups increases the load on internal fixation and may affect the interface between implant and osteoporotic bone. An external fixator with motion capacity serves to prote…

Randomized prospective evaluation of injection techniques for the treatment of lateral epicondylitis.

Lateral epicondylitis is a commonly made diagnosis for general practitioners and orthopedic surgeons. Corticosteroid injection is a mainstay of early treatment. However, conflicting evidence exists to support the use of steroid injection, and no evidence in the literature supports an injection technique. Nineteen patients diagnosed with acute lateral epicondylitis were evaluated to compare the peppered- and single-injection techniques using the Disabilities of the Arm, Shoulder and Hand (DASH) score, visual analog score (VAS), and grip strength. For elbows with a single injection, mean grip st…

OBJECTIVE: To study the surgical procedures and results for treating the nonunion of lateral humeral condyle fracture combined with cubitus … peratively, the angle of cubitus valgus deformity was 25-55 degrees (average 44.8 degrees), and the elbow motion range of flexion and extension was 135-140 degrees (average 139 degrees) and 0-20 degrees (average 7 degrees), respectively. One case with the symptoms of ulnar neuritis was diagnosed as incomplete injury of ulnar nerve. The time between admi …

Simultaneous dislocation of the radial head and distal radio-ulnar joint. A case report.

Isolated dislocation of the distal radio-ulnar joint and isolated dislocation of the radial head in adults are not common injuries. A simultaneous dislocation of the radial head and distal radio-ulnar joint with no other injury seems to be extremely rare since only one report was found in the English literature. A similar case, but with some differences in presentation and treatment is reported.

Irreducible dorsal radiocarpal fracture dislocation with dissociation of the distal radioulnar joint: a case report.

We report a case of dorsal radiocarpal fracture dislocation with dissociation of the distal radioulnar joint. Closed reduction was unsuccessful due to interposition of the osteochondral fragments and open reduction and fixation was carried out with a satisfactory end result. The advantages of volar approach and use of external fixator in the management of this injury are discussed.

Pathoanatomic findings in radiohumeral epicondylopathy. A combined anatomic and electromyographic study.

Several authors believe that a compression syndrome of the radial nerve or its muscle branches is responsible for the clinical picture of radiohumeral epicondylopathy. Various structural and functional stenoses have been discussed as possible causes. We performed systematic electromyographies (EMGs) on the extensors subdividing from the radial epicondyle and found significant changes (P < 0.05) in 27/51 patients regarding latency, velocity of nerve conduction and rate of polyphasic potentials. Especially affected were the extensor carpi radialis brevis and the extensor digitorum muscle. In ord

Comminuted intra-articular fracture of the distal radius--results of early open reduction and internal fixation.

Comminuted intra-articular fracture of the distal radius remains a challenging problem. This article reviews the results of 15 consecutive cases which were treated with open reduction and internal fixation with T-plate after failing to achieve articular congruency with closed reduction. The follow-up ranges from 20 months to 28 months. In thirteen cases, articular congruency was restored. However, 4 of these 13 cases later lost some of the articular congruency. This suggests the need for supplementary bone grafting. Reduction of the extra-articular portion of distal radius was generally well m

[Surgical debridement of epicondyles. A 10-year follow up of 100 cases].

A single method was used in all cases: debridement of the radial head and exeresis of the apophysis. The statistics presented are derived from 100 elbows operated in 90 patients between 1970 and 1988. Fifty-four (54%) of the patients were men. Thirty-one (31) of the patients regularly took part in sport. Eighty-seven (87) cases were subjected to histopathological examination. Eighty-two (82) showed major signs of insertion tendinitis. With a mean follow-up of 7 years 4 months, the rate of good or very good results was 88% in subjective terms and 85% in objective terms. It was possible to exami…

Reconstruction of the ulnar collateral ligament in athletes.

Reconstruction of the ulnar collateral ligament using a free tendon graft was performed on sixteen athletes. All participated in sports that involved throwing (mostly professional baseball), and all had valgus instability of the elbow. After reconstruction and rehabilitation, ten of the sixteen patients returned to their previous level of participation in sports, one returned to a lower level of participation, and five retired from professional athletics. Despite precautions, there was a high incidence of complications related to the ulnar nerve. Two patients had postoperative ulnar neuropathy…

Brachioradialis muscle pain: a common source of underdiagnosed or misdiagnosed forearm pain.

The brachioradialis (BR) muscle is a long, large, fusiform muscle on the lateral side of the forearm. It originates from the lateral distal humerus and inserts to the base of the styloid process of the radius. The function of the BR muscle is to flex the elbow, especially with the hand in a neutral position, to pronate and supinate the forearm, and to support wrist extension, especially gripping or picking up something. BR muscle pain arises from repetitive overuse, sudden overloading, and direct blow. Common painful daily activities include putting a cup back down after drinking, opening door…

The anatomical variations of the cubital tunnel in a South African body donor sample.

PURPOSE: The ulnar nerve (UN) courses through the cubital tunnel, which is a potential site of entrapment. Anatomical variations of the cubital tunnel may contribute towards cubital tunnel syndrome (CuTS), however, these are not well described. The aim was to compare the range of variations and dimensions of the cubital tunnel and the UN between sexes and sides of the body. METHODS: Sixty elbows from 30 embalmed bodies (17 males and 13 females) were dissected. The prevalence of the cubital tunnel retinaculum (CuTR) or anconeus epitrochlearis (AE) forming the roof of the tunnel was determined.…

Prevalence of bilateral ulnar nerve subluxation among professional baseball pitchers.

BACKGROUND: Radiographic and physical examination findings of ulnar nerve instability have been recognized in overhead throwing athletes, despite the fact that some of these abnormalities may be asymptomatic and represent adaptive changes. While recommendations for screening and early detection have been made that can adversely impact an athletes' career, the presence of bilateral ulnar nerve subluxation and its relationship with medial elbow symptoms has not been characterized in professional overhead throwing athletes. PURPOSE: To characterize the prevalence of bilateral ulnar nerve subluxa…
