Mechanisms of pain-induced local cerebral blood flow changes in the rat sensory cortex and thalamus.

It is a well-known phenomenon that cerebral blood flow is coupled to neural activation induced by non-noxious somatosensory stimulation. However, basic questions related to pain-induced cerebral blood flow changes remain unanswered. In the present study, the sciatic nerve of anesthetized rats was subjected to electric stimulation with noxious and non-noxious parameters. Changes in local cerebral blood flow and neuronal activity were determined simultaneously in the sensory cortex and in the thalamus by laser-Doppler flowmetry and c-fos immunohistochemistry, respectively. The role of different …

Spherical harmonics representation of the steady-state membrane potential shift induced by tDCS in realistic neuron models.

Objective. We provide a systematic framework for quantifying the effect of externally applied weak electric fields on realistic neuron compartment models as captured by physiologically relevant quantities such as the membrane potential or transmembrane current as a function of the orientation of the field.Approach.We define a response function as the steady-state change of the membrane potential induced by a canonical external field of 1 V m-1as a function of its orientation. We estimate the function values through simulations employing reconstructions of the rat somatosensory cortex from the B

Nerve growth factor injection into semispinal neck muscle evokes sustained facilitation of the jaw-opening reflex in anesthetized mice -- possible implications for tension-type headache.

Nociceptive input from neck muscles probably plays a role in the pathophysiology of tension-type headache. In order to elaborate an animal model, the impact of noxious input from neck muscles on orofacial sensorimotor processing was investigated by electrophysiological means in anesthetized mice. Group IV muscle afferents of the semispinal neck muscle were excited by local injection of nerve growth factor (NGF, 0.8 microM, 20 microl). Orofacial sensorimotor processing was monitored by the jaw-opening reflex (JOR) elicited by electric tongue stimulation. After unilateral NGF injection into the

[Diprivan and electroconvulsive therapy].

Anaesthesia for Electroconvulsive Therapy (ECT) is characterized by short repeat anaesthetic procedures, performed outside an operating theatre. The efficacy of ECT relies upon the occurrence of tonoclonic convulsions. Propofol seems to be the intravenous induction agent of choice for ECT. Its pharmacokinetic properties ensure a rapid and deep anaesthesia, of short duration, with a minimum of side effects, and a rapid recovery of good quality, suitable for short repetitive procedures. As low doses of propofol are used, a rapid injection is required to obtain the hypnotic effect. These low dose

Transfer of synaptic vesicle antigens to the presynaptic plasma membrane during exocytosis.

We have utilized immunofluorescence techniques to look for synaptic vesicle antigens on the plasma membrane of resting and active nerve terminals. Rabbit antiserum was raised against purified cholinergic synaptic vesicles from the electric organ of Narcine brasiliensis, a marine electric ray. Antibodies to synaptic vesicles were shown to bind selectively to nerve terminals in cryostat sections of frog nerve-muscle preparations. Binding was demonstrated indirectly by using fluorescein-labeled goat anti-rabbit antibodies. Structures in cross sections that bound antiserum were identified as nerve

Adaptive dual-modality neuromodulation for chronic pain and muscle dysfunction: real-world insights from the NXTSTIM™ EcoAI digital platform.

BACKGROUND: Chronic musculoskeletal pain is often inadequately managed with pharmacologic therapy. Although transcutaneous electrical nerve stimulation (TENS) and electrical muscle stimulation (EMS) are established noninvasive modalities, conventional systems lack personalization and adherence monitoring. OBJECTIVE: To evaluate real-world outcomes associated with the EcoAI adaptive dual-modality (TENS+EMS) neuromodulation platform for chronic pain management. DESIGN, SETTING AND PARTICIPANTS: Retrospective analysis of de-identified symptom diaries from 2,135 adults with chronic musculoskelet

Use of Temporary Peripheral Nerve Stimulation for Management of Recalcitrant Pain in Patients With Complex Regional Pain Syndrome: Case Series and Literature Review.

BACKGROUND: Complex regional pain syndrome (CRPS) is a chronic pain syndrome involving autonomic, sensory, motor, and sudomotor dysfunction. Its pathophysiology involves both central and peripheral nervous systems, with a constant interplay between multiple mechanisms. Treatment for CRPS varies from pharmacological choices, physical therapy, and sympathetic blocks to neuromodulation techniques, namely use of spinal cord stimulation, dorsal root ganglion stimulation, and peripheral nerve stimulation (PNS). CASE REPORT: We present a case series detailing the use and efficacy of temporary PNS fo

Pain Relief From Non-pharmacological Rehabilitative Therapies for Musculoskeletal Injuries Immediately Following Treatment Sessions: Secondary Data Analysis.

INTRODUCTION: Musculoskeletal injuries, with loss of readiness in military warfighters, underscore the importance of therapies to reduce pain and accelerate recovery for return to duty. Non-pharmacologic interventions, including exercise and electrotherapy, may be beneficial in garrison and austere environments. A concern associated with electrotherapy is that the electrical current may intensify discomfort, resulting in reduced adherence and longer recovery. This secondary analysis examined whether electrotherapy (transcutaneous electrical nerve stimulation [TENS] and neuromuscular electro-st

Wearable non-invasive neuroprosthesis for targeted sensory restoration in neuropathy.

Peripheral neuropathy (PN), the most common complication of diabetes, leads to sensory loss and associated health issues as pain and increased fall risk. However, present treatments do not counteract sensory loss, but only partially manage its consequences. Electrical neural stimulation holds promise to restore sensations, but its efficacy and benefits in PN damaged nerves are yet unknown. We designed a wearable sensory neuroprosthesis (NeuroStep) providing targeted neurostimulation of the undamaged nerve portion and assessed its functionality in 14 PN participants. Our system partially restor

Electroconvulsive therapy in patients with vagus nerve stimulation.

Numerous studies have demonstrated the safety and efficacy of electroconvulsive therapy (ECT) for various psychiatric conditions, and it has been approved by the Food and Drug Administration for the treatment of refractory depression. Recently, the Food and Drug Administration also approved vagus nerve stimulation as a treatment for chronic or recurrent depression. Although electrical stimulation is used for both ECT and vagus nerve stimulation, the mechanisms of their action are very different. The American Psychiatric Association task force identifies no absolute contraindications to the use

Depletion of adrenal catecholamines alters the amnestic effect of amygdala stimulation.

Amygdala stimulation produces a robust retrograde amnesia which appears to be related to peripheral hormonal responses to stress. This present study investigated the effects of electrical stimulation of the amygdala delivered immediately after inhibitory and active avoidance training to rats depleted of adrenal catecholamines or sham adrenal operated rats. Comparable findings were obtained with the two training tasks: amygdala stimulation impaired retention of sham adrenal operated rats but it enhanced retention of rats depleted of adrenal catecholamines. These findings suggest that adrenal ca

High-density theta oscillatory-modulated tDCS over the parietal cortex for targeted memory enhancement.

OBJECTIVES: Associative memory (AM) declines due to healthy aging as well as in various neurological conditions. Standard transcranial electrical stimulation (tES) protocols show inconclusive facilitatory effects on AM, often lacking function specificity and stimulation focality. We tested the effectiveness of high-density electrode montage delivering anodal theta oscillatory-modulated transcranial direct current stimulation (HD-Theta-otDCS) over the left posterior parietal cortex (PPC), aiming to target AM in a spatially focused and function-specific manner. METHODS: In a sham-controlled cro

Discriminability of multiple cutaneous and proprioceptive hand percepts evoked by intraneural stimulation with Utah slanted electrode arrays in human amputees.

BACKGROUND: Electrical stimulation of residual afferent nerve fibers can evoke sensations from a missing limb after amputation, and bionic arms endowed with artificial sensory feedback have been shown to confer functional and psychological benefits. Here we explore the extent to which artificial sensations can be discriminated based on location, quality, and intensity. METHODS: We implanted Utah Slanted Electrode Arrays (USEAs) in the arm nerves of three transradial amputees and delivered electrical stimulation via different electrodes and frequencies to produce sensations on the missing hand

Effects of transcranial direct current stimulation of prefrontal cortex on risk-taking behavior.

AIM: Recent cognitive neuroscience research shows that noninvasive brain stimulation can modify a wide range of behaviors in healthy people. Such regulation effects on human behaviors provide new insights into the neurobiology of cognitive processes and establish causal brain-behavior relations. Here, we aimed to examine the effects of transcranial electrical stimulation (TES) of the prefrontal cortex on risk-taking. METHODS: We performed a systematic search on the PubMed, Web of Science, and Cochrane databases with appropriate keywords for original studies reporting the use of TES to modulat

A brief period of moderate noxious stimulation induces hemorrhage and impairs locomotor recovery after spinal cord injury.

Spinal cord injury (SCI) is often accompanied by additional tissue damage (polytrauma) that provides a source of pain input. Our studies suggest that this pain input may be detrimental to long-term recovery. In a rodent model, we have shown that engaging pain (nociceptive) fibers caudal to a lower thoracic contusion SCI impairs recovery of locomotor function and increases tissue loss (secondary injury) and hemorrhage at the site of injury. In these studies, nociceptive fibers were activated using intermittent electrical stimulation. The stimulation parameters were derived from earlier studies

Transcutaneous Facial Nerve Frontal Branch Stimulation to Restore Dynamic Elevation of the Paralyzed Eyebrow in Synkinetic Patients.

BACKGROUND: While patients commonly recover motion following facial nerve injury, aberrant neural regeneration can lead to synkinesis and chronic nonflaccid facial palsy (NFFP); many patients fail to recover volitional eyebrow elevation. OBJECTIVE: To assess if transcutaneous electrical stimulation of injured frontal branches can achieve eyebrow elevation. METHODS: We studied healthy volunteers (N = 10) and patients with unilateral NFFP (N = 36). We performed monopolar transcutaneous stimulation of healthy and paralyzed frontal branches with 4-10 mA pulse trains. We measured eyebrow elevatio
