Long-term follow-up of bradycardia in elite athletes.

The aim of the study was to examine the hypothesis that when former high-level athletes retired from competition, bradycardia would vanish. ECG changes and factors possibly related to the persistence of bradycardia were investigated. We performed a longitudinal follow-up study in 157 former elite athletes who had records of bradycardia < 50 bpm when they were active in high-level competition. All had retired from competitive sport for a minimum of five years prior to participation in the follow-up examination. Data on sport modality, hours of weekly training during competition periods, years p

A novel C-terminal truncation SCN5A mutation from a patient with sick sinus syndrome, conduction disorder and ventricular tachycardia.

OBJECTIVES: Individual mutations in the SCN5A-encoding cardiac sodium channel alpha-subunit cause single cardiac arrhythmia disorders, but a few cause multiple distinct disorders. Here we report a family harboring an SCN5A mutation (L1821fs/10) causing a truncation of the C-terminus with a marked and complex biophysical phenotype and a corresponding variable and complex clinical phenotype with variable penetrance. METHODS AND RESULTS: A 12-year-old male with congenital sick sinus syndrome (SSS), cardiac conduction disorder (CCD), and recurrent monomorphic ventricular tachycardia (VT) had muta

A new and simple method for distinguishing complete from incomplete block through the cavotricuspid isthmus.

BACKGROUND: A complete line of block (CLOB) in the cavotricuspid isthmus (CTI) is the endpoint of typical atrial flutter ablation. Before CTI block is obtained, a progressive CTI conduction delay due to an incomplete line of block (InLOB) can be difficult to distinguish from CLOB. The purpose of this study was to assess a new simple approach based on the changes in atrio-ventricular (AV) conduction delays during septal and lateral right atrial pacing, to distinguish a CLOB from an InLOB during typical atrial flutter (AFL) ablation. METHODS AND RESULTS: Forty patients who presented an InLOB be

Interaction between inflammation and thrombosis in acute coronary syndrome.

BACKGROUND: Inflammation and thrombosis are important in the pathogenesis of acute coronary syndrome (ACS). Cytokines [interleukin-1beta (IL-1beta) and interleukin-6 (IL-6)] are inflammation markers which play a major role in the development of coronary heart disease. Experimental data documented that an increase of cytokine and von Willebrand factor (vWF) levels in unstable angina (UA) and non-Q wave myocardial infarction (MI) predicts an adverse outcome. AIM: To examine the correlation between the IL-1beta, IL-6 and vWF levels in patients with ACS. METHODS: We examined 92 patients (74 men,

Transmural dispersion of repolarization and ventricular tachyarrhythmias.

BACKGROUND: Myocardial transmural dispersion of repolarization (TDR) has been associated with reentrant arrhythmias in animal studies but a clinical association has not yet to been demonstrated. The present study examines the relationship between TDR and ventricular tachyarrhythmias in human subjects. METHODS: This study consisted of 65 patients with non-sustained ventricular tachycardia, sustained ventricular tachycardia, ventricular fibrillation or unexplained syncope with organic heart disease. The control group included 65 patients with paroxysmal supraventricular tachycardia. The 12 ECG

Mechanisms of recurrent atrial fibrillation after pulmonary vein isolation by segmental ostial ablation.

OBJECTIVES: The aim of this study was to determine the mechanisms responsible for recurrent atrial fibrillation (AF) after pulmonary vein isolation (PV) by segmental ostial ablation. BACKGROUND: Recovery of conduction into a previously isolated PV is a common observation when there is recurrent AF soon after segmental ostial ablation. However, the mechanisms of recurrent AF have been unclear. METHODS: A repeat ablation procedure was performed in 50 patients who had recurrent paroxysmal AF at a mean of 7 +/- 6 months after segmental ostial ablation to isolate the PVs. During the repeat proced

Brugada syndrome: a decade of progress.

The Brugada syndrome has gained wide recognition throughout the world and today is believed to be responsible for 4% to 12% of all sudden deaths and approximately 20% of deaths in patients with structurally normal hearts. The incidence of the disease is on the order of 5 per 10 000 inhabitants and, apart from accidents, is the leading cause of death of men under the age of 50 in regions of the world where the inherited syndrome is endemic. This minireview briefly summarizes the progress made over the past decade in our understanding of the clinical, genetic, cellular, ionic, and molecular aspe

The role of the epicardium and neural crest as extracardiac contributors to coronary vascular development.

At species-specific times in embryonic development, the pro-epicardial organ appears as an outcropping of the mesothelial body wall, near the sinus venosus-liver region. The pro-epicardial vesicles attach to the myocardium, flatten, and join to form the epicardium. The epicardium shows epithelial-mesenchymal transformation: cells detach from the epithelium, fill the subepicardial space, and invade the heart tube. Epicardium-derived cells migrate as far as the core of the endocardial cushions, which differentiate into the atrioventricular valve leaflets. In the cardiac wall, other epicardium-de

Significance of anterior Q waves in left anterior fascicular block--a clinical and noninvasive assessment.

BACKGROUND: Electrocardiographic (ECG) Q waves in V leads (V2 or V3) pose a diagnostic challenge in the presence of left anterior fascicular block (LAFB). Benign Q waves in the absence of coronary artery disease (CAD) primarily due to LAFB have been described. This study evaluates Q waves in the presence of LAFB. HYPOTHESIS: Anterior Q waves in the presence of LAFB may not be indicative of myocardial infarction (MI). METHODS: From 1990 to 1997, ECGs of 236 male patients with LAFB were analyzes for presence of Q waves in the V leads. Records were reviewed for evidence of CAD. Of 236 patients

Arterial baroreceptors control plasma vasopressin responses to graded hypotension in conscious dogs.

We studied the role of cardiac and arterial baroreceptors in the reflex control of arginine vasopressin (AVP) and renin secretion during graded hypotension in conscious dogs. The dogs were prepared with Silastic cuffs on the thoracic inferior vena cava and catheters in the pericardial space. Each experiment consisted of a control period followed by four periods of inferior vena caval constriction, during which mean arterial pressure (MAP) was reduced in increments of approximately 10 mmHg. The hormonal responses were measured in five dogs under four treatment conditions: 1) intact, 2) acute ca

Electropharmacologic effects of class I and class III antiarrhythmia drugs on typical atrial flutter: insights into the mechanism of termination.

BACKGROUND: Acute effects of class I and class III antiarrhythmia drugs on the reentrant circuit of typical atrial flutter are not fully studied. Furthermore, the critical electrophysiologic determinants of flutter termination by antiarrhythmia drugs are not clear. METHODS AND RESULTS: The study population consisted of 36 patients (mean age, 53+/-17 years) with clinically documented typical atrial flutter. A 20-pole "halo" catheter was positioned around the tricuspid annulus. Incremental pacing was performed to measure the conduction velocity along the isthmus and lateral wall, and extrastimu

Influence of beta-adrenergic and vagal activity on the effect of exogenous adenosine on supraventricular tachycardia termination.

Adenosine, which binds to cell surface receptors and couples with guanosine triphosphate-binding inhibitory proteins (G(i)), is potent in terminating supraventricular tachycardia (SVT). However, whether the differences in autonomic tone will influence this effect remains unknown. This study was designed to investigate the role of beta-adrenergic and vagal activity on the action of adenosine. Forty patients with clinically documented SVT (22 with atrioventricular node reentrant tachycardia and 18 with atrioventricular reciprocating tachycardia) were divided into 4 groups with 10 patients in eac

[Accessory pathways of decremential conduction (Mahaim fibers). Treatment by endocavitary ablation; apropos of 8 cases].

Mahaim fibres are rare, right sided accessory pathways comparable with respect to certain properties (slow, decremential conduction) with "accessory atrioventricular node" located on the lateral tricuspid annulus at a distance from the Aschoff-Tawara node. Atriofascicular and atrioventricular fibres may be distinguished, both responsible for wide complex tachycardia (left bundle branch block pattern with left axis deviation). The authors report a series of 8 patients (6 women, 2 men: age: 27 +/- 11 years) without underlying cardiac disease, incapacitated by episodes of antidromic reciprocating

Electrical remodeling of the human atrium: similar effects in patients with chronic atrial fibrillation and atrial flutter.

OBJECTIVES: This study sought to determine whether chronic atrial fibrillation (AF) and atrial flutter in patients lead to electrical remodeling of the human atrial myocardium, manifested by an abnormal relation between atrial cycle length and action potential duration (APD). BACKGROUND: Experimental studies in goats and isolated human atrial tissue have shown that prolonged AF leads to persistent shortening of atrial refractoriness, a phenomenon referred to as electrical remodeling and which helps to explain why AF begets AF. Direct data on human in situ myocardium are still lacking. METHOD

Fusion with postpaced return cycle identical to tachycardia cycle length during transient entrainment of ventricular tachycardia and its implications.

In reentrant ventricular tachycardia (VT), the postpaced return cycle (RC) during transient entrainment at a pacing site far from the central common pathway is longer than the VT cycle length (VTCL), when VT is represented by a figure-eight model. However, the reentrant circuit has not been fully elucidated. The purpose of this study was to present VT in which the postpaced RC became identical to VTCL during transient entrainment while fusion is evident in the surface electrocardiogram (ECG). Among 38 patients with inducible reentrant VTs who underwent electrophysiologic study (EPS), 10 VTs of

Comparison of the electrophysiologic effects of oral sustained-release and intravenous verapamil in patients with paroxysmal supraventricular tachycardia.

The electrophysiologic effects of intravenous verapamil (0.15 mg/kg) and oral sustained-release verapamil (verapamil-SR) (240 mg once daily for 7 days) were studied in 17 patients with paroxysmal supraventricular tachycardia (SVT). Ten patients had atrioventricular (AV) nodal reentrant tachycardia and 7 had AV reciprocating tachycardia involving an accessory AV pathway. Both preparations significantly prolonged anterograde effective refractory period of the AV node and depressed the retrograde AV nodal conduction system. The sinus cycle length, and atrial and ventricular effective refractory p

Impaired subsidiary pacemaker function in patients with a right bundle branch block created prior to complete catheter ablation of the AV junction.

The impact of a right bundle branch block (RBBB), inadvertently created prior to complete ablation of the atrioventricular (AV) junction, on the intrinsic subsidiary pacemaker function was investigated. In 31 patients suffering from intractable supraventricular tachyarrhythmia, catheter ablation of the AV junction was performed using direct current (n = 13) or radiofrequency (n = 18) energy. In 16/31 patients a RBBB was created prior to complete AV ablation. Subsidiary pacemaker function was evaluated after a mean period of 5 months. Following 5 min of ventricular pacing (70 beats.min-1) escap

EKG abnormalities in pediatric patients with myotonic dystrophy.

Electrocardiographic (EKG) abnormalities are frequent in patients with myotonic dystrophy; cardiac complications may lead to significant morbidity and mortality. The charts of 17 pediatric patients with myotonic dystrophy were reviewed to ascertain the frequency of EKG abnormalities and cardiovascular symptoms. Fifteen of 17 patients had abnormal EKGs with sinus bradycardia being the most common abnormality. Only 1 of 17 patients had cardiovascular symptoms. Four patients had moderate to severe weakness and 3 of them had a conduction disturbance (i.e., first-degree AV block or intraventricular
