Parents' expectations, awareness, and experiences of accessing evidence-based speech-language pathology services for their children with autism.

The aim of this study was to explore the expectations, awareness, and experiences of parents in their efforts to access evidence-based speech-language pathology (SLP) services for their children with autism spectrum disorders (ASD). Four focus groups were conducted with 20 parents of pre school aged children with ASD and transcripts were analysed using thematic analysis. Two themes emerged to account for the participants' expectations, awareness, and experiences. The two themes, "Speech-language pathology: More than just a business", and "Parents and power", represented the complex interaction…

Phonological and articulation treatment approaches in Portuguese children with speech and language impairments: a randomized controlled intervention study.

BACKGROUND: In Portugal, the routine clinical practice of speech and language therapists (SLTs) in treating children with all types of speech sound disorder (SSD) continues to be articulation therapy (AT). There is limited use of phonological therapy (PT) or phonological awareness training in Portugal. Additionally, at an international level there is a focus on collecting information on and differentiating between the effectiveness of PT and AT for children with different types of phonologically based SSD, as well as on the role of phonological awareness in remediating SSD. It is important to …

The use of subject-verb agreement and verb argument structure in monolingual and bilingual children with specific language impairment.

This study investigates the use of subject-verb agreement and verb argument structure in the spoken Dutch of monolingual Dutch children with specific language impairment (SLI) and bilingual Frisian-Dutch children with SLI. Both SLI groups appeared to be less efficient in their use of subject-verb agreement and verb argument structure than the control group (consisting of monolingual typically developing Dutch children matched on mean length of utterance in morphemes) in that they showed significantly more agreement errors as well as a relation between verb agreement structure complexity and om…

The aromatase gene CYP19A1: several genetic and functional lines of evidence supporting a role in reading, speech and language.

Inspired by the localization, on 15q21.2 of the CYP19A1 gene in the linkage region of speech and language disorders, and a rare translocation in a dyslexic individual that was brought to our attention, we conducted a series of studies on the properties of CYP19A1 as a candidate gene for dyslexia and related conditions. The aromatase enzyme is a member of the cytochrome P450 super family, and it serves several key functions: it catalyzes the conversion of androgens into estrogens; during early mammalian development it controls the differentiation of specific brain areas (e.g. local estrogen syn…

Do children with dyslexia and/or specific language impairment compensate for place assimilation? Insight into phonological grammar and representations.

English speakers have to recognize, for example, that te[m] in te[m] pens is a form of ten, despite place assimilation of the nasal consonant. Children with dyslexia and specific language impairment (SLI) are commonly proposed to have a phonological deficit, and we investigate whether that deficit extends to place assimilation, as a way of probing phonological representations and phonological grammar. Children with SLI plus dyslexia, SLI only, and dyslexia only listened to sentences containing a target word in different assimilatory contexts-viable, unviable, and no change-and pressed a button…

I can't sleep at night with discharging this lady: The personal impact of ending therapy on speech-language pathologists.

The ending of therapy is a crucial time for speech-language pathologists and can impact on their sense of achievement and satisfaction. Drawing on literature from psychotherapy, social work and rehabilitation as well as from the area of aphasia therapy, this paper explores how speech-language pathologists juggle the tensions of coping with real versus ideal endings, of managing the building of close therapeutic relationships which then have to be broken, and of balancing a respect for client autonomy while retaining control over caseloads and fair allocation of resources. I suggest that the wa…

Language development, literacy skills, and predictive connections to reading in Finnish children with and without familial risk for dyslexia.

Discriminative language markers and predictive links between early language and literacy skills were investigated retrospectively in the Jyväskylä Longitudinal Study of Dyslexia in which children at familial risk for dyslexia have been followed from birth. Three groups were formed on the basis of 198 children's reading and spelling status. One group of children with reading disability (RD; n = 46) and two groups of typical readers from nondyslexic control (TRC; n = 84) and dyslexic families (TRD; n = 68) were examined from age 1.5 years to school age. The RD group was outperformed by typical r…

[A study of autism spectrum disorders and language disorders using magnetoencephalography. The scientific contribution of Dr. Muñoz Yunta].

INTRODUCTION AND DEVELOPMENT: Autism spectrum disorders (ASD) are characterised by the alteration of three basic areas of behaviour, qualitative alterations in reciprocal social interaction, qualitative alterations in communication and patterns of behaviour, and stereotyped, repetitive and restrictive activities and interests. Specific language impairment (SLI) is a disorder in which language is slow and retarded with respect to the patient's chronological age, and which is not related to a sensory-auditory or motor deficit or to a pervasive developmental disorder. In both disorders, epilepsy …

Non-motor basal ganglia functions: a review and proposal for a model of sensory predictability in auditory language perception.

While the primary function of the basal ganglia (BG) is linked to motor behaviour, several investigations of non-motor behaviour allocate cognitive and language-specific functions to the BG. What may such seemingly discrepant functions have in common? Some neurophysiologic theories of motor behaviour assign temporal sequencing, others the sequencing of general cognitive patterns to the BG. Turning to auditory language perception and syntax in particular, one may consider syntactic processing as a hierarchical sequencing phenomenon. Furthermore, previous data have shown that if events are predi…

Why do speech and language therapists stay in, leave and (sometimes) return to the National Health Service (NHS)?

BACKGROUND: Research into recruitment, retention and return of speech and language therapists in the National Health Service (NHS) is relatively limited, particularly in respect of understanding the factors that drive employment choice decisions. AIMS: To identify what factors influence speech and language therapists working in the NHS to stay, and consider leaving, but not do so. To identify what features of the NHS and alternative employers influence speech and language therapists to leave the NHS. To identify why some speech and language therapists return to the NHS after working elsewhere…

Whose voice is it anyway? Hushing and hearing 'voices' in speech and language therapy interactions with people with chronic schizophrenia.

BACKGROUND: Some people with schizophrenia are considered to have communication difficulties because of concomitant language impairment and/or because of suppressed or 'unusual' communication skills due to the often-chronic nature and manifestation of the illness process. Conversations with a person with schizophrenia pose many pragmatic challenges as they may include delusional talk or hallucinatory references considered irrelevant or inappropriate to ongoing conversation. Yet delusional talk and references to hallucinations are often an intrinsic part of the overall presentation in people wi…

Social knowledge in children with language impairments: examination of strategies, predicted consequences, and goals in peer conflict situations.

This study investigated social knowledge in school-age children, aged 8-12 years, with and without language impairment (LI and TD groups). A hypothetical peer conflict task was administered to examine the relationship among prosocial responses and parent/teacher ratings of children's social behaviours. Stimuli included 12 hypothetical peer conflict vignettes presented in an open-ended and forced choice condition. The LI group generated (open-ended) and selected (forced choice) fewer prosocial strategies. When asked to predict a friend's reaction to a selected conflict resolution strategy, the …

Fingerspelling, signed language, text and picture processing in deaf native signers: the role of the mid-fusiform gyrus.

In fingerspelling, different hand configurations are used to represent the different letters of the alphabet. Signers use this method of representing written language to fill lexical gaps in a signed language. Using fMRI, we compared cortical networks supporting the perception of fingerspelled, signed, written, and pictorial stimuli in deaf native signers of British Sign Language (BSL). In order to examine the effects of linguistic knowledge, hearing participants who knew neither fingerspelling nor a signed language were also tested. All input forms activated a left fronto-temporal network, in…

Effects of cross-sex hormones on cerebral activation during language and mental rotation: An fMRI study in transsexuals.

Androgens and estrogens affect the performance on certain cognitive tests, particularly those measuring verbal fluency and mental rotation. Their effects on cognition have frequently been attributed to changes in cerebral lateralization. This study tested the impact of a reversal of the sex steroid milieu on cerebral activation and lateralization during verbal and spatial tasks in transsexuals. fMRI scans were obtained from 6 female-to-male and 8 male-to-female transsexuals at baseline and after cross-sex steroid treatment. Activation was measured during language and mental rotation tasks. Lan…
