BACKGROUND: Mycosis fungoides, the most common cutaneous T-cell lymphoma, typically exhibits a gradual progression. However, aggressive forms with extracutaneous involvement and low proliferative indices may present diagnostic and prognostic challenges. Nail involvement appears to be infrequent and poorly understood, and is often considered a late-stage manifestation. The Ki-67 index, although frequently utilized as an indicator of tumor aggressiveness, may have limitations in specific mycosis fungoides subtypes that appear to be influenced by distinct molecular or immunological pathways.

A novel technique for hindfoot arthrodesis using distal fibula strut autograft for talar bone loss is described. A case series of six patients who underwent the novel procedure is presented. Six patients were retrospectively evaluated with traumatic talar injury and subsequent avascular necrosis (AVN) who underwent reconstruction with a novel technique. Rosai Dorfman disease is a rare disease with massive lymphadenopathy. It presents significant challenges to the anaesthesiologists involving multiple organs especially the airway. Here we present a patient who is a known case of Rosai Dorfman disease with severe hypothyroidism and Alzheimer's disease with previous history of laryngotracheal reconstruction, now presenting with femur inter trochanteric fracture for proximal femur nailing. A tailored approach with detailed preoperative evaluation and planning, emphasis on airway, with backup plan added to the successful management of this case.

BACKGROUND: The Brunelli flap is an option in the reconstruction of the thumb after trauma or oncological resections. The arc of movement of the flap makes it possible to resolve defects in the proximal, palmar, dorsal, and lateral regions.
METHODS: We present a case series of 11 patients in whom a Brunelli flap was performed for postoncological reconstruction, melanoma in situ, and invasive squamous cell carcinoma of the thumb nail unit associated with Mohs micrographic surgery. This combination has not been described before.

CASE: A 71-year-old man with castration-resistant Stage IVB prostate cancer developed symptomatic oligometastatic disease in the lumbar spine and bilateral proximal femurs. He was treated with a single-position L2-L4 kyphoplasty with concomitant prone left-sided femoral prophylactic cephalomedullary nailing.
