A novel sheath system to improve target access for preoperative mapping biopsy: A randomized trial to inform surgical planning.

BACKGROUND: Accurate assessment of biliary tract cancer spread is important for appropriate surgery. However, conventional fluoroscopy-guided transpapillary biopsy often fails to reach intrahepatic bile ducts. This study evaluated a novel tapered-tip sheath system (SS) for preoperative mapping biopsy (MB). METHODS: In this single-center randomized trial, 90 patients with suspected malignant bile duct stenosis were assigned 1:1 to SS or conventional biopsy; crossover after technical failure was allowed. The primary outcome was MB success (intention-to-treat [ITT]). MB success was defined as ob

A prospective observational study of laparoscopic approaches for suspected gallbladder cancer in Yamaguchi (YPB-002 LAGBY).

We have previously reported laparoscopic total biopsy methods for suspected gallbladder cancer (GBC). The present prospective observational study evaluated the safety and feasibility of a novel two-stage algorithm using laparoscopic total biopsy methods. The two-stage algorithm was applied for 40 patients with suspected GBC between July 2018 and September 2022. Laparoscopic whole-layer cholecystectomy (LWLC) was performed for early-stage or suspected malignant lesions without liver invasion and laparoscopic gallbladder bed resection (LGBR) was performed for lesions with an unclear border betwe

Analysis of the surgical waiting list for conditions not covered by the Explicit Health Guarantees in orthopedics and traumatology in Chile.

INTRODUCTION: Surgical waiting lists for conditions not covered by the Explicit Health Guarantees represent unmet needs and structural gaps within the Chilean public health system. The field of orthopedics and traumatology accounts for a high volume of pending procedures, with total knee arthroplasty being the most frequently delayed. The coexistence of deferrable pathologies not formally recorded, combined with the low efficiency in the use of operating rooms, aggravates this problem. This study aims to characterize the surgical waiting list for conditions not covered by the Explicit Health G

Surgical Outcomes of Gallbladder Retrieval via Different Ports in Laparoscopic Cholecystectomy.

BACKGROUND: An important step during laparoscopic cholecystectomy that has a proven effect on postoperative morbidity and outcomes is the site of gallbladder retrieval, which is either through the umbilical or epigastric port. Currently, no guidelines recommend the superiority of one port over the other, and it is mainly based on the surgeon's preference. This study aimed to address the outcomes and complications of gallbladder retrieval through the epigastric port in comparison with the umbilical port and to identify the associated risk factors. METHODS: A retrospective review of 658 patient
