First-Trimester Cardiac Myxoma Diagnosed at 12 Weeks with Postmortem Validation.

Primary cardiac tumors are extremely rare in fetal life, with rhabdomyomas being the most frequently encountered. In contrast, fetal cardiac myxomas are exceptionally uncommon, and reports of diagnosis during the first trimester are exceedingly rare. We report a fetal right atrial myxoma diagnosed at 12 weeks' gestation. Fetal echocardiography revealed a hyperechoic, pedunculated mass originating from the right atrial wall. Serial follow-up demonstrated rapid tumor growth, with increasing pericardial effusion and reversed A-wave flow in the ductus venosus by 14 weeks' gestation, indicating ear

Lemierre's Syndrome With External Jugular Vein Thrombosis Following Submandibular Sialadenitis: A Life-Threatening Complication.

BACKGROUND: Lemierre's syndrome is a rare but potentially life-threatening condition traditionally characterized by bacteremia, septic thrombophlebitis of the internal jugular vein, and metastatic septic emboli following pharyngotonsillitis. We report an atypical presentation originating from submandibular sialadenitis and complicated by external jugular vein thrombophlebitis. CASE PRESENTATION: A 26-year-old man presented with high-grade fever, sore throat, and right submandibular swelling persisting for 7 days. Contrast-enhanced CT revealed right submandibular sialadenitis, thrombosis of th

Life-threatening tracheal leiomyoma presenting as acute respiratory failure masquerading as refractory asthma: A case report.

Primary tracheal tumors represent less than 1% of all pulmonary neoplasms, with endobronchial leiomyomas accounting for a minute fraction of benign airway lesions. Due to their slow growth and nonspecific clinical presentation, these tumors are frequently misdiagnosed as obstructive airway diseases, such as asthma. We report the case of a 27-year-old female with a 3-year history of progressive, treatment-resistant respiratory symptoms initially managed as severe asthma. Despite escalating medical therapy, her condition deteriorated, culminating in an emergency department presentation with fulm

A Rare Case of Iliopsoas Abscess Extending to Scarpa's Triangle With Gluteal Fistulization.

Iliopsoas abscess (IPA) is a rare and potentially life-threatening condition, often presenting with nonspecific symptoms that delay diagnosis. Secondary IPA, typically resulting from contiguous spread of infection, can occasionally demonstrate atypical anatomical extension. We report the case of a 64-year-old female patient with a history of right nephrectomy, type 2 diabetes mellitus, and hypertension, presenting with a progressively enlarging mass in the right thigh. Imaging revealed a large multiloculated retroperitoneal abscess originating from the iliacus muscle and extending through the

Respiratory manifestations of sickle cell disease in children: a comprehensive review for the pediatrician.

INTRODUCTION: Sickle cell disease (SCD) is an inherited hemoglobinopathy characterized by the production of sickle hemoglobin, leading to red blood cells sickling and hemolysis in hypoxic conditions. The resulting acute and chronic endothelial inflammation leads to chronic organ damage. Respiratory manifestations in SCD usually start from childhood and represent the leading causes of morbidity and mortality. Nevertheless, they are generally poorly addressed or recognized later in life, often contributing to a more severe course and complications. AREAS COVERED: This narrative review aims to o

Contrastive Learning With Transformer to Predict the Chronicity of Children With Immune Thrombocytopenia.

Immune thrombocytopenia (ITP) is a typically self-limiting and immune-mediated bleeding disorder in children. Approximately 20% of children with ITP experience chronicity, leading to reduced quality of life and increased treatment burden. The accurate prediction of chronicity would enable clinicians to make personalized treatment plans at an early stage. However, due to the self-limiting nature of ITP and the scarcity of available children patients, the data presents two prominent issues: small data and imbalanced class, which are unfavorable for effectively training a deep learning model. To

Current advances in the clinical management of Perry syndrome: is there hope for the future?

INTRODUCTION: Perry syndrome (PS) is a rare, inherited neurodegenerative disorder caused by mutations in the DCTN1 gene. It is characterized by parkinsonism, neuropsychiatric symptoms, central hypoventilation, and progressive weight loss, typically leading to a rapid disease course and early death. As genetic testing becomes more widespread, PS is increasingly diagnosed, and its clinical spectrum is expanding. AREAS COVERED: The authors conducted a comprehensive search of public databases through September 2025 to identify original research, conference proceedings, and book chapters related t

Neurodevelopmental outcomes in congenital heart disease: modifiable and nonmodifiable substrates.

PURPOSE OF REVIEW: Neurodevelopmental impairments are the most common comorbidity among children, adolescents, and adults with congenital heart disease (CHD). Despite significant advances in operative and perioperative care resulting in increased survival, neurodevelopmental impairments remain prevalent in this population. Neurodevelopmental impairments, though subtle, can have a major impact on quality of life, attainment of independence and societal contribution. In this review, we summarize current knowledge on neurodevelopmental outcomes in the CHD population, including neuroimaging findin

Translation and cross-cultural adaptation of a questionnaire for assessing hyperacusis in Williams syndrome.

Williams syndrome (WS) is a genetic, multisystemic, and neurodevelopmental disorder. The prevalence of auditory hypersensitivity in WS is high, especially in childhood, with reports in the literature from 94 to 100% of individuals evaluated, which can generate significant impacts on their quality of life. Therefore, the existence of instruments for screening hyperacusis that are easy and quick to apply for use in clinical routine is essential. To translate and perform the cross-cultural adaptation of the Hyperacusis Screening Questionnaire into Brazilian Portuguese.A questionnaire concerning au

A Case of Neonatal Pyriform Sinus Cyst Presenting With Respiratory Distress.

Pyriform sinus cysts are rare congenital anomalies originating from the third or fourth branchial pouch that can cause neonatal airway obstruction. We report the case of a male neonate born at 38 weeks of gestation who developed respiratory distress at birth. Physical exam revealed subcostal retractions and nasal flaring, but no stridor. Laryngoscopy on day 4 demonstrated a bulging posterior pharyngeal wall, and computed tomography (CT) identified an air-filled cyst extending into the left neck, suggestive of a pyriform sinus cyst. Ultrasound (US)-guided aspiration of 7 mL of fluid temporarily

A Case Report of a Large Bowel Obstruction and Associated Closed-Loop Small Bowel Obstruction Secondary to Diverticular Disease.

Diverticular disease is one of the most common gastrointestinal pathologies, and its prevalence is rising in areas where diets are low in fiber and high in processed foods containing preservatives and emulsifiers. While this commonly affects the colon, in rare cases, chronic diverticulitis can lead to scarring and fibrosis that impact the small bowel. This case report describes the clinical course of a 56-year-old male with multiple comorbidities who presented to the emergency department for acute abdominal pain with worsening abdominal distention. Imaging revealed extensive intraperitoneal fr

Tumor microenvironment of ameloblastoma with a focus on osteoclastogenesis, cell migration, and malignant transformation.

BACKGROUND: Odontogenic tumors arise in the jawbone and originate from cells associated with tooth development. Therefore, understanding odontogenic tumors requires knowledge of all aspects of dental research, including tooth development and eruption. Ameloblastoma is the most common odontogenic tumor. HIGHLIGHT: Although a benign tumor, ameloblastoma progresses with marked jawbone resorption. Because of its locally aggressive features, it can be treated surgically by resecting the surrounding bone. From a molecular pathology perspective, several genetic mutations and dysregulated signaling p

To Show or Not to Show: Factors within the School Environment That Influence the Expression of LGBTQ Identities.

Prejudice against LGBTQ people during their schooling years can be detrimental due to its long-term consequences. This includes the development of beliefs that the world is unsafe, which can perpetuate mental health struggles later in life. Fostering a school environment where LGBTQ people can express their identity can contribute to greater well-being. This qualitative study drew on interviews with 13 school graduates to examine the environmental factors within Australian schools that influenced LGBTQ students' expression of their identity. Drawing on Altman's conceptualization of oppression
