Rapid Detection of Carbapenemase and Extended-Spectrum β-Lactamase Producing Gram-Negative Bacteria Directly from Positive Blood Cultures Using a Novel Protocol.

BACKGROUND: Early and adequate antibiotic treatment is the cornerstone of improving clinical outcomes in patients with bloodstream infections (BSI). Delays in appropriate antimicrobial therapy have catastrophic consequences for patients with BSI. Microbiological characterization of multi-drug resistant pathogens (MDRP) allows clinicians to provide appropriate treatments. Current microbiologic techniques may take up to 96 h to identify causative pathogens and their resistant patterns. Therefore, there is an important need to develop rapid diagnostic strategies for MDRP. We tested a modified pro

A unified multi modal transformer framework for breast cancer recurrence prediction and survival analysis.

Breast cancer recurrence prediction is an important feature of post-treatment therapy, requiring accurate identification of both recurrence risk and time-to-event outcomes. In this paper, we offer a unified deep learning system that jointly performs survival analysis and multi-class recurrence classification, enabling full risk stratification for breast cancer patients. The proposed model includes a Transformer-based survival module to estimate time-until-recurrence, and an attention-guided classification module to differentiate between second primary cancer, low-risk, and high-risk recurrence

T174M-M235T AGT Gene Haplotypes in Women with Pre-Eclampsia from Northwest Mexico: A Pilot Case-Control Study.

Pre-eclampsia is a Hypertensive Disorder of Pregnancy (HDP) characterized by hypertension and proteinuria, affecting 2-8% of pregnancies worldwide and constituting a major public health concern. Genes of the renin-angiotensin system have been investigated as potential causative factors, but inconclusive results have been obtained. The objective of this pilot study is to evaluate the possible contribution of alleles, genotypes or haplotypes of two single-nucleotide polymorphisms (SNPs) T174M (rs4762) and M235T (rs699) in AGT gene to pre-eclampsia in the Mexican population. We analyzed the assoc

The Bucharest international European Society for Organ Transplantation consensus on paediatric controlled donation after circulatory determination of death.

Paediatric controlled donation after circulatory determination of death (pcDCDD) is a well described pathway for deceased organ donation, but there has been wide variability in global uptake. There are substantial and differing barriers to pcDCDD across countries, including clinical, legal, and ethical issues. This process utilized a Delphi consensus methodology involving 30 international experts to develop recommendations to guide the development and operation of pcDCDD programs. Two survey rounds evaluated agreement on system requirements, donor identification, medical suitability, communica

[A case of paraganglioma-related catecholamine-induced cardiomyopathy manifesting as dilated cardiomyopathy and literature review].

Paraganglioma (PGL) are rare, catecholamine-secreting neuroendocrine neoplasms arising from the adrenal medulla or the extra-adrenal paraganglionic system. Excessive secretion of catecholamines by PGL can induce myocardial damage, leading to catecholamine-induced cardiomyopathy (CICM). Some CICM presents morphologically as a dilated cardiomyopathy (DCM) phenotype. Its clinical identification and differential diagnosis are highly challenging, necessitating a comprehensive evaluation that integrates biochemical testing and multimodality imaging. Treatment strategies require a precise balance amo

Cannabidiol alleviates the inflammatory response in rats with traumatic brain injury through the PGE 2-EP2-cAMP-PKA signaling pathway.

Traumatic brain injury (TBI) is a recognized global public health problem. However, there are still limitations in the available therapeutic approaches and a lack of clinically effective drugs. Therefore, an in-depth exploration of the secondary pathological mechanism of TBI and the identification of new effective drugs are urgently needed. Cannabidiol (CBD), a component derived from the cannabis plant, has potential therapeutic effects on neurological diseases and has received increasing attention. However, few reports on CBD intervention in TBI patients exist. Here, we use the Feeney free-fa

Isolated Foot Drop Due to a Cerebral Infarction Mimicking Lumbar Radiculopathy: A Case Report and Literature Review.

Isolated foot drop is a neurological sign frequently linked to lower motor neuron (LMN) lesions, including peroneal nerve damage or L4-L5 radiculopathy. Nonetheless, upper motor neuron (UMN) lesions, such as strokes or tumors located in the parasagittal motor cortex, may sometimes manifest as isolated foot drops. The main causes of isolated foot drop secondary to central etiologies are uncommon, with few instances documented in the literature. An 83-year-old male presented with a four-day history of left isolated foot drop that started in the big toe and then spread to involve the whole foot.

[Moyamoya Disease and the RNF213 Gene].

Genetic research on moyamoya disease(MMD) has advanced significantly following the identification of RNF213 as a susceptibility gene. Approximately 80% of Japanese patients with MMD harbor the RNF213 p.Arg4810Lys variant, which has been increasingly linked to variations in clinical phenotypes, including disease progression, mode of onset, and postoperative outcomes. Of note, this variant is also associated with intracranial arterial stenosis that does not meet the diagnostic criteria for MMD, as well as with systemic vascular conditions such as non-cardioembolic ischemic stroke, coronary arter

Chronic Obstructive Pulmonary Disease (COPD) is a pathological condition of the respiratory system characterized by chronic airflow obstruction, associated with changes in the lung parenchyma (pulmonary emphysema), bronchi (chronic bronchitis) and bronchioles (small airways disease). In the last years, the importance of phenotyping and endotyping COPD patients has strongly emerged. Metabolomics refers to the study of metabolites (both intermediate or final products) and their biological processes in biomatrices. The application of metabolomics to respiratory diseases and, particularly, to COPD

Molecular Profiling of Circulating Tumour Cells and Circulating Tumour DNA: Complementary Insights from a Single Blood Sample Utilising the Parsortix® System.

The study of molecular drivers of cancer is an area of rapid growth and has led to the development of targeted treatments, significantly improving patient outcomes in many cancer types. The identification of actionable mutations informing targeted treatment strategies are now considered essential to the management of cancer. Traditionally, this information has been obtained through biomarker assessment of a tissue biopsy which is costly and can be associated with clinical complications and adverse events. In the last decade, blood-based liquid biopsy has emerged as a minimally invasive, fast,

Misinterpretation of an inflammatory FDG uptake in a patient treated for Hodgkin lymphoma: a case report.

Hodgkin Lymphoma (HL) is a malignancy involving lymph nodes and lymphatic system. [18F]F-FDG PET/CT (FDG-PET) imaging is routinely used for staging, to assess early chemotherapy response (interim FDG-PET), at the end of treatment (EoT FDG-PET) and for the identification of disease recurrence. We present a case of a 39-year-old man treated for HL. FDG-PET scans performed after first line therapy (both Interim PET and at the end of therapy) demonstrated a persistent and significant mediastinal FDG uptake. The patient was treated with a second line therapy but the FDG-PET uptake did not change. A

An Extensive Study of Phenol Red Thread as a Novel Non-Invasive Tear Sampling Technique for Proteomics Studies: Comparison with Two Commonly Used Methods.

Tear samples are considered in recent publications as easily, noninvasively collectible information sources for precision medicine. Their complex composition may aid the identification of biomarkers and the monitoring of the effectiveness of treatments for the eye and systemic diseases. Sample collection and processing are key steps in any analytical method, especially if subtle personal differences need to be detected. In this work, we evaluate the usability of a novel sample collection technique for human tear samples using phenol red threads (cotton thread treated with the pH indicator phen

Renal Papillary Necrosis (RPN) in an African Population: Disease Patterns, Relevant Pathways, and Management.

Renal papillary necrosis (RPN) is characterized by coagulative necrosis of the renal medullary pyramids and papillae. Multiple conditions and toxins are associated with RPN. Several RPN risk factors, or POSTCARDS, have been identified, with most patients presenting with RPN having at least two contributing risk factors. Currently, there is no specific test to diagnose and confirm RPN; however, several imaging tools can be used to diagnose the condition. RPN is currently underdiagnosed in African populations, often with fatal outcomes. In African clinical settings, there is a lack of consensus

Cnpy32xHA mice reveal neuronal expression of Cnpy3 in the brain.

BACKGROUND: Identification of biallelic CNPY3 mutations in patients with epileptic encephalopathy and abnormal electroencephalography findings of Cnpy3 knock-out mice have indicated that the loss of CNPY3 function causes neurological disorders such as epilepsy. However, the basic property of CNPY3 in the brain remains unclear. NEW METHOD: We generated C-terminal 2xHA-tag knock-in Cnpy3 mice by i-GONAD in vivo genome editing system to investigate the expression and function of Cnpy3 in the mouse brain. RESULTS: 2xHA-tagged Cnpy3 was confirmed by immunoblot analysis using anti-HA and CNPY3 ant

Portal Venous and Mesenteric Wall Calcifications along with Intra-splenic Vascular Calcification: A Rare Case Report and Literature Review.

BACKGROUND: Cross-sectional imaging findings of portal vein calcification were found to be an indicator of portal vein thrombosis. Calcification of the portal vein and its tributaries in portal hypertension is an uncommon finding and the mechanical stress due to long-standing portal hypertension on the vessel wall is considered leading to sclerosis and calcification in the intima and media of the vessel wall. Intra-splenic vascular calcification is a rare occurrence and has been seen in patients with portal hypertension. CASE PRESENTATION: A 60-year-old male was referred for an ultrasound sca

Characterization of a rare clinical isolate of A. spinulosporus following a central nervous system infection.

A rarely reported clinical specimen of Aspergillus spinulosporus was isolated from an immunocompetent 22-month-old boy who was suffering from central nervous system aspergillosis and meningitis. The patient had no comorbidity, organ transplantation, or other surgical operations that lead to invasive aspergillosis. A. spinulosporus is mostly a soil borne strain, and only three invasive aspergillosis cases involving this strain have been reported. We isolated this strain from cerebrospinal fluid, cultured it successfully on PDA medium, and named it BJCH M5. We performed a complete genomic and ph

Acute Hemorrhagic Leukoencephalitis (AHLE): A Comprehensive Review on Causes, Symptoms, Link with COVID-19, Diagnosis, and Treatment.

Acute hemorrhagic leukoencephalitis (AHLE), also called Hurst disease, is a rare demyelinating disease of the central nervous system (CNS) marked by rapid progression and acute inflammation of the white matter. Due to the correlation in their suspected postinfectious autoimmune pathogenesis, it is regarded as the most severe form of acute disseminated encephalomyelitis (ADEM). Because this clinical scenario has a high mortality rate, aggressive and immediate treatment is required. Although the exact cause of AHLE is unknown, it usually occurs after a bacterial or viral infection, or, less freq
