Developmental-genetic effects on level and change in childhood fears of twins during adolescence.

BACKGROUND: If the adaptive significance of specific fears changes with age, the genetic contribution to individual differences may be lowest at the age of greatest salience. The roles of genes and environment in the developmental-genetic trajectory of five common childhood fears are explored in 1094 like-sex pairs of male and female monozygotic and dizygotic twins assessed on up to three occasions during adolescence (ages 8-18 years). METHODS: Dichotomous self-ratings of a cluster of five correlated fears from Ollendick's schedule of fears (FSSC-R) were extracted for subjects at each occasio

The role of drinking motives in social anxiety and alcohol use.

Although social anxiety and problem drinking commonly co-occur, the relationship between social anxiety and drinking among college students is not well understood. The current study examined the relationship between drinking motives, or reasons for drinking, and social anxiety in 239 volunteers. Contrary to hypotheses, high (n=83), moderate (n=90), and low (n=66) social anxiety groups did not differ in endorsement of coping and conformity drinking motives. Further, social anxiety was negatively related to weekly alcohol use and unrelated to alcohol-related problems. Post hoc hierarchical multi

Transcriptome analysis of cold syndrome using microarray.

Microarrays are widely used to study changes in gene expression in diseases. In this paper, we use this technology to discover gene expression patterns in the cold syndrome in Chinese medicine. We identify differentially expressed genes and extracted gene modules that are enriched with differentially expressed genes in the cold syndrome by analyzing cDNA samples, which are purified from blood taken from a pedigree. Our results suggest that the cold syndrome might be caused by the physiological imbalance and/or the disorder of metabolite processes. The study confirms the hypotheses about molecu

Epidemiology of social phobia in later life.

OBJECTIVE: Although anxiety disorders, including social phobia (SP), are common among older adults, very little is known about the epidemiology of SP in later life. METHOD: Using data drawn from a large, nationally representative sample of older adults from Canada (N=12,792), the authors estimate lifetime and 12-month prevalence of social phobia and examine demographic predictors and patterns of comorbidity of current SP in this population. RESULTS: The results reveal that SP is a prevalent disorder in later life with lifetime and 12-month prevalence estimates of 4.94% and 1.32%, respectivel

The appraisal of social concerns scale: psychometric validation with a clinical sample of patients with social anxiety disorder.

The Appraisal of Social Concerns (ASC) Scale was created by Telch et al. (2004) to improve upon existing self-report measures of social anxiety-related cognition. In a largely nonclinical sample, the ASC was found to possess three factors and was psychometrically sound. In a smaller clinical sample, the ASC demonstrated sensitivity to the effects of cognitive behavioral therapy. In the present study, the psychometric properties of the ASC were examined in a larger sample of patients with social anxiety disorder. In this sample, the ASC exhibited a 2-factor structure; the nature of the factors

Social anxiety disorder in 11-12-year-old children: The efficacy of screening and issues in parent-child agreement.

OBJECTIVE: To investigate the level of diagnostic and discriminative accuracy of The Social Anxiety Scale for Children - Revised (SASC-R) for identifying social anxiety disorder (SAD) in a community-based sample of 11-12 year-old children. Parent-child diagnostic agreement was also examined. METHOD: A questionnaire including SASC-R and items on impulsive behavior was sent to a population based sample of children, born in 1992. A total of 2568 parents returned their questionnaires (rr: 70%), and 1297 (51%) consented to further participation. An index group [50 high-scoring children on social a

Is the chronic fatigue syndrome an exercise phobia? A case control study.

OBJECTIVE: The aim of this study was to test whether patients with chronic fatigue syndrome (CFS) have an exercise phobia, by measuring anxiety-related physiological and psychological reactions to ordinary activity and exercise. METHODS: Patients and healthy but sedentary controls were assessed over 8 h of an ordinary day, and before, during and after an incremental exercise test on a motorised treadmill. To avoid confounding effects, those with a comorbid psychiatric disorder were excluded. Heart rate, galvanic skin resistance (GSR) and the amount of activity undertaken were measured, along

Integrating neurobiology and psychopathology into evidence-based treatment of social anxiety disorder.

Social anxiety disorder (SAD) is a common, chronic psychiatric disorder characterized by a persistent fear of social or performance situations in which embarrassment can occur. This disorder typically appears during the mid-adolescent years and is unremitting throughout life if not properly treated. SAD presents as two subtypes: the more common and debilitating generalized form, and the nongeneralized form, which consists predominantly of performance anxiety. The majority of patients with SAD have comorbid mental disorders, including mood, anxiety, and substance abuse. No single development th

Predictors and consequences of food neophobia and pickiness in young girls.

OBJECTIVE: Vegetable intake among children is well below recommended levels. We assessed whether food neophobia and pickiness contribute to low vegetable intake in school-aged girls and if there are distinct predictors for neophobia and pickiness. Children with food neophobia are reluctant to eat new foods whereas picky children resist eating many familiar foods. Design/subjects Participants were 192 7-year-old girls and their parents, recruited for a study of girls' nutrition and development. We examined relationships between food neophobia and pickiness and assessed whether these variables p

Social phobia and potential childhood risk factors in a community sample.

BACKGROUND: This study examined the relationship between potential childhood risk factors and social phobia in an epidemiological sample. Identifying risk factors such as childhood adversities can often uncover important clues as to the aetiology of a disorder. This information also enables health care providers to predict which individuals are most likely to develop the disorder. METHODS: Data came from the Mental Health Supplement to the Ontario Health Survey of a survey of 8116 Canadian respondents, aged 15-64. Social phobia was diagnosed using the Composite International Diagnostic Interv

Using intranasal midazolam spray to prevent claustrophobia induced by MR imaging.

OBJECTIVE: Up to 37% of patients undergoing MR imaging examinations experience moderate to severe levels of anxiety that necessitate the termination of the procedure in 5-10% of patients. Although the clinical use of MR imaging has increased, effective procedures to handle claustrophobia are lacking. We evaluated the effectiveness of intranasally administered midazolam spray in preventing claustrophobic responses of patients undergoing MR imaging. SUBJECTS AND METHODS: Fifty-four patients scheduled for MR imaging were included in this prospective study. Anxiety and sedation of patients were e

Processing of phobic stimuli and its relationship to outcome.

In the present study, we explored factors related to successful treatment outcome in a sample of participants with fear of spiders. We specifically examined the relationship of general memory, memory for the phobic stimulus, memory for anxious responses, and perceived self-efficacy to treatment outcome. Forty-eight participants who were afraid of spiders participated in two sessions of in vivo exposure therapy. On day 1, participants completed measures of general memory, memory for the phobic stimulus, recall of anxiety level during exposure tasks, and self-efficacy during exposure tasks. At p

Heritability and prevalence of specific fears and phobias in childhood.

Fears and phobias are relatively common in childhood. Both environmental and genetic theories have tried to explain the etiology behind these conditions. However, data supporting the different theories are sparse. To investigate the relative importance of genetic and environmental influences on specific phobias and fears, parental reports of animal, situational, and mutilation fears and phobias were completed for 1106 pairs of 8- to 9-year-old Swedish twins. The prevalence of specific phobias was 7.3% for boys and 10.0% for girls. Genetic. shared environmental, and nonshared environmental effe

Dealing with the fear of mutilation in the donation discussion.

Fear of mutilation is a significant barrier to organ and tissue donation. It constitutes an example of Mystical Thinking and may be seen as an exemplar of animal learning or, more specifically, as a representation of the "blood phobia." As such the fear is not amenable to conventional public education efforts. Cognitive and behavioral techniques used in treating other types of phobias should be studied as a way to remove this barrier to donation.

Assessment and treatment of social phobia.

Social phobia is an anxiety disorder characterized by heightened fear and avoidance of one or more social or performance situations, including public speaking, meeting new people, eating or writing in front of others, and attending social gatherings. People with social phobia are typically anxious about the possibility that others will evaluate them negatively and/or notice symptoms of their anxiety. Social phobia affects up to 13% of individuals at some time in their lives and is usually associated with at least moderate functional impairment. Research on the nature and treatment of social ph

Exposure with and without cognitive therapy for generalized social phobia: effects of individual and group treatment.

Patients with generalized social phobia (n = 73) were randomly allocated to two treatment modalities: (1) group or (2) individual treatment, and to three treatment packages: (1) two blocks of exposure in vivo (2) a block cognitive therapy followed by a block exposure in vivo, or (3) two blocks cognitive-behavioural treatment in which both strategies were integrated from the start. All treatments consisted of 16 sessions, given in two treatment blocks of 4 weeks each, separated by a no-treatment phase of 4 weeks. Self-report assessments were held before and after the treatment blocks and at 3-m

Origins of childhood fears: an evaluation of Rachman's theory of fear acquisition.

This study explored Rachman's (1977) theory of fear acquisition in a large sample of Australian and American children and adolescents. Participants completed a questionnaire that addressed different pathways of fear acquisition for 10 highly prevalent fears. The majority of children attributed the onset of their fears to vicarious and instructional factors, although these indirect sources of fear were often combined with direct conditioning experiences. Also examined were effects for gender, age, and nationality. Boys and preadolescents were found to report more direct and vicarious experience

The use of ambulatory services by persons with and without phobia.

The delivery of ambulatory mental health and general health services to persons with phobias (unweighted n = 1,689) and without phobias during a 6-month period are examined. The phobics were part of a larger study of 18,572 subjects, drawn as a representative sample of the population in five locations, as part of the Epidemiologic Catchment Area Program (ECA). Among phobic conditions, agoraphobia most often leads to use of services related to emotional problems, especially in the specialty mental health sector. There were no significant differences between male and female subjects in their use
