Dislocation after reverse total shoulder arthroplasty using contemporary medialized modular systems. Can we still consider it such a frequent complication?

INTRODUCTION: Improvement in reverse total shoulder arthroplasty (RTSA) prosthesis and technique has reduced the initially high complication and revision rates associated with this implant. However, instability continues to be a leading cause of reoperation after RTSA, being related in previous studies with medialized designs. The aim of this study was to determine the dislocation rate with the employment of two medialized modular systems of RTSA in our institution. MATERIALS AND METHODS: A retrospective review of two different modular systems of RTSA (SMR Modular Shoulder System and Delta XT

Preoperative partial-thickness rotator cuff tears do not compromise anatomic total shoulder replacement outcomes: medium-term follow-up.

BACKGROUND: Reverse total shoulder replacement (TSR) in elderly patients with primary osteoarthritis (OA) and rotator cuff pathology is increasingly being performed. The purpose of our study was to determine the medium-term results of anatomic TSR for OA in patients with established preoperative partial-thickness rotator cuff tears on magnetic resonance imaging (MRI) scans. METHODS: We reviewed a cohort of patients who had undergone anatomic TSR for OA with a preoperative MRI diagnosis of partial-thickness rotator cuff tear. Patients were assessed with preoperative and post operative Oxford S

Geometric modification of the humeral position after total reverse shoulder arthroplasty: what is the optimal lowering of the humerus?

BACKGROUND: The main mechanical effect after reverse shoulder arthroplasty (RSA) is the lowering of the glenohumeral rotation center. The optimal value of the humeral lowering after RSA is still debated. The main objective of our study was to determine the lowering and medialization of the humerus and to correlate these parameters with short-term functional results. MATERIALS AND METHODS: The study included 70 patients with complete radiographic and clinical data. A multivariate analysis was used to compare the clinical and radiographic outcomes with the humeral lowering. RESULTS: The mean h

Treatment of humeral shaft fractures using antegrade nailing: functional outcome in the shoulder.

BACKGROUND: The purpose of this study was to evaluate shoulder outcomes and function after humeral shaft fractures treated with antegrade nailing. MATERIALS AND METHODS: Thirty patients with acute humeral shaft fractures who underwent antegrade locked intramedullary nailing were retrospectively studied. Range of motion (ROM) of the affected shoulder was evaluated, comparing it with the nonaffected shoulder, radiologic position of the nails, complications, and need for a second surgery. RESULTS: The study enrolled 20 men and 10 women (average age, 41.9 years). The average follow-up was 35.8 m

Associations among shoulder strength, glenohumeral joint motion, and clinical outcome after rotator cuff repair.

Rotator cuff tears are a common condition causing pain and disability, but the relationships among clinical measures of shoulder function and measures of glenohumeral joint (GHJ) function are not well known. In the study reported here, dynamic in vivo GHJ motion was measured during abduction from biplane radiographs in 22 rotator cuff repair (RCR) patients and 36 control subjects. Isometric shoulder strength was measured and clinical outcomes were assessed using the Western Ontario Rotator Cuff (WORC) Index. Associations among WORC, GHJ motion, and several shoulder strength ratios were assesse

[Image-based guidance in shoulder traumatology. Initial clinical experience].

Computer-assisted systems are used frequently in pelvis and spine surgery but are still rare in general trauma and hardly ever used in shoulder surgery. The major limitation is the use of rigid markers and the problem of obtaining navigable images in the complex shoulder region. The aim of this study was to evaluate the use of a new image-based guidance system in shoulder traumatology. The system was used to implant a tightrope system in Tossy III lesions of the AC joint (n=7) and compared to the conventional method (n=8). Outcome parameters were duration of surgery, number of trials for perfe

Ambulatory open Bankart repair under a single general anesthesia: a prospective study of the immediate outcome.

The interscalenic block technique is widely used for ambulatory shoulder surgery despite a substantial number of failures and adverse effects. We prospectively evaluated satisfaction in 40 consecutive patients who underwent open Bankart repair under a single general anesthesic performed in an ambulatory care unit. The mean age of the patients was 23 years. There were 29 men and 11 women. Patients were evaluated postoperatively with a visual analog scale of pain (in the recovery room, at the time of discharge, and the day after and 1 week after surgery) via a home assessment questionnaire that

Kinematic analysis of shot-putting performed by wheelchair athletes of different medical classes.

The aim of this study was to identify those kinematic characteristics that are most closely related to an athlete's medical classification and measured distance of a put. Two S-VHS camcorders (60 fields per second) were used to record the performance of 17 males of different classes. Each participant performed six trials and the best trial for each was selected for analysis. Three-dimensional kinematics of the shot and upper body segments at the instant of release and during the forward thrust (delivery) were determined. The average speeds and angles of the shot at release for different classe

A prospective comparison of computerized arthrotomography and magnetic resonance imaging of the glenohumeral joint.

Twenty-five patients with shoulder instability or shoulder pain of undetermined etiology were prospectively evaluated with magnetic resonance imaging and computerized arthrotomography. Actual lesions were determined by arthroscopy or at the time of open surgical repair. The images obtained were interpreted independently by three radiologists blinded to both surgical results and the results of previous diagnostic tests. Sensitivity, specificity, and accuracy were determined for each imaging technique for a variety of pathologic entities, including anterior and posterior labral abnormalities, ca

Bidirectional endovascular treatment for axillary artery injury secondary to proximal humerus fracture: a case report.

BACKGROUND: Axillary artery injury secondary to proximal humerus fracture is a rare but serious complication. The management of this injury has traditionally involved surgical treatment. CASE SUMMARY: A 66-year-old female with gait disturbance slipped and fell off her wheelchair at home. She presented to a local hospital with right shoulder pain and was subsequently urgently transferred to our hospital by helicopter because of suspicion of axillary artery injury. Computed tomography angiography revealed disruption of the right axillary artery. We decided to perform endovascular treatment inst

[Neuralgic amyotrophy: a common cause of unilateral and bilateral diaphragmatic pareses].

There are several causes for unilateral or bilateral diaphragmatic paresis. The most common cause is an (intraoperative) injury to the phrenic nerve. However, in up to 20% of cases, no explanation can be found despite extensive workup. Neuralgic amyotrophy (NA, also known as Parsonage-Turner syndrome) is a common underdiagnosed multifocal autoimmune-inflammatory disease that predominantly affects proximal nerve segments of the upper extremities. Classic symptoms include acute onset of severe pain in the shoulder girdle with delayed onset of paresis of the shoulder and arm muscles. In at least 7

Editorial Commentary: Arthroscopic Bankart Repair With Remplissage, Irrespective of Glenoid Bone Loss, May Be Advantageous Over Latarjet: Don't Widen the Lane, Just Fill in the Pothole!

Anterior glenohumeral instability has an incidence of 21.9 individuals per 100,000 in the general population. When recurrent instability occurs, bone loss to the anteroinferior glenoid can occur. The concept of "on-track" and "off-track" Hill-Sachs lesions is significant in cases of bipolar bone loss, because if humeral-sided bony injury also requires treatment, a remplissage combined with arthroscopic Bankart repair (ABR) may be an alternative to a Latarjet procedure. The Latarjet often is touted as a "definitive," because it adds bone to increase the glenoid surface area that the humeral hea

Rabbit supraspinatus motor endplates are unaffected by a rotator cuff tear.

Rotator cuff tears are a major cause of morbidity. Following rotator cuff tears, muscle atrophy and fatty infiltration begin in the tissue, limiting repair potential and leading to a higher re-tear rate and a worse functional outcome. We evaluated whether fatty degeneration resulting from a complete supraspinatus tear with retraction is associated with an injury to the suprascapular nerve. Four skeletally mature New Zealand white rabbits were randomized to receive an index procedure on either their right or left shoulder with the opposite shoulder serving as a control. At the index procedure,

[Traumatic rupture of the rotator cuff in an adolescent--case report].

A traumatic rotator cuff tear is a very rare condition, especially in the adolescent. We report about a 14-year-old boy who fell onto the right arm while riding his bicycle. Immediately after the accident he described some kind of click in his right shoulder. he was referred to our institution 3 days after the trauma. The MRI showed a huge defect of the labrumligament complex including the rotator interval. Intraoperatively, we found a complete disruption of the subscapularis and supraspinatus tendons as well as of the anterior stabilizers. We performed a modified Bankart repair and a reconstr
