Lymphangioma of the tongue: report of case.

Lymphangiomas are benign tumors of the lymphatic vessels, usually found in children. Lymphangiomas, however, may be misdiagnosed and in the absence of treatment may increase in size, producing macroglossia, swallowing and speech interferences, and respiratory difficulty. The size and isolated location of the lesion in the midline of the tongue reported here are unusual; few similar cases have been reported in the literature. Complete surgical excision provided not only removal of the lesion, but allowed normal recontouring of the tongue.

Functional Laryngectomy and Quality of Life in Survivors of Head and Neck Cancer With Intractable Aspiration.

IMPORTANCE: Late effects of head and neck cancer (HNC) treatment include profound dysphagia, chronic aspiration, and death. Functional laryngectomy (FL) can improve patient survival and quality of life (QoL); however, removing a failing larynx for a noncancer reason is a difficult decision. Data regarding the ability of FL to improve self-perceptions of voice, swallowing, and QOL in survivors of HNC with intractable aspiration are inconclusive. OBJECTIVE: To investigate the association of FL with changes in self-reported perceptions of voice, swallowing, oral intake, QoL, and mood in survivor

Complete dentures for a patient after a stroke by means of orofacial myofunctional therapy: A clinical report.

After a stroke, patients frequently show compromised swallowing, mastication, and speech, as well as unfavorable motion and deviation of the tongue and mandible. The dentist can improve the oral rehabilitation of a patient with deteriorated facial and oral muscles after a stroke by incorporating orofacial myofunctional therapy. This report describes a method for tongue exercises and correction of mandible deviation in an edentulous patient after a stroke by using a pearl on a wire in the anteriomedian palatal part of the maxillary denture.

Symptomatic Anterior Cervical Osteophyte Causing Dysphagia: Case Report, Imaging, and Review of the Literature.

Anterior cervical osteophytes are found in 20-30% of elderly patients. Rarely, severe osteophytes can cause dysphagia, dysphonia, and dyspnea. Here, we illustrate a case of severe dysphagia caused by a large post-traumatic osteophyte with oropharyngeal swallow study showing a significant mass effect on the pharynx and resolution following osteophytectomy. We also review the literature regarding the etiology, diagnosis, and treatment of symptomatic anterior cervical osteophytes.

Evaluation and management of amyotrophic lateral sclerosis.

Motor neuron diseases can cause progressive impairment of voluntary muscles of movement, respiration, speech, and swallowing. This review discusses the most common motor neuron disease, amyotrophic lateral sclerosis (ALS). It reviews the evaluation, diagnosis, and management of ALS, and its epidemiology, pathophysiology, and management. A coordinated approach by the primary care physician and neurologist is necessary with a focus on treatment options, durable medical equipment needs, and end-of-life discussions.

Prosthodontic management of radiation induced xerostomic patient using flexible dentures.

Xerostomia causes discomfort for complete denture wearers as the tissues become dry and friable due to lack of lubricating properties of saliva. Common problems faced by such patients are glossitis, mucositis, angular chelitis, dysgeusia and difficulty in chewing and swallowing. This case report describes a new method in addressing such issues by using flexible complete denture construction in radiation induced xerostomic patient with minimal tissue damage during and after denture construction procedures.

Incidental computed tomography diagnosis of a giant oesophageal leiomyosarcoma.

Leiomyosarcoma of the oesophagus is a very unusual tumour; only 53 cases have been reported in the English-language literature. A case is reported here of a patient with a giant leiomyosarcoma, without any symptoms of dysphagia. The diagnosis was made incidentally during CT examination of the chest for detection of possible pulmonary metastases from a coexisting carcinoma of the bladder. This was confirmed by a barium swallow, oesophagoscopy and tissue diagnosis. The literature pertaining to this most uncommon tumour is reviewed.

[Late diagnosis of retropharyngeal abscess in an infant].

In patients with respiratory distress and difficulty in swallowing life-threatening conditions should be born in mind. We report a case in which the diagnosis of retropharyngeal abscess was established with considerable delay in an eight-month girl. The etiology and clinical picture is discussed. We conclude that patients with stridor and drooling should be admitted to hospital at once and should be examined with equipment for tracheotomy/intubation ready. If the epiglottis is found to be normal the possibility of a retropharyngeal abscess should be considered.

New method for dealing with late-presenting spontaneous esophageal ruptures.

A new technique is described for dealing with late-presenting spontaneous esophageal ruptures. This method requires only a short period of general anesthesia to drain the periesophageal abscess by a drainage tube inserted into the abscess cavity from the esophagus with the aid of a gastroscope and fluoroscopy. Gastric fluids are diverted from the esophageal rupture with a gastrostomy, and a jejunostomy is used for enteral feeding. The esophagus is retained, and closure of the fistula with resumption of normal swallowing is documented with serial sinograms.

Ascariasis.

Ascaris lumbricoides is the most common helminth to infect humans. Infection occurs when contaminated soil containing mature eggs is swallowed. Clinically, infection ranges from the asymptomatic carrier state to life-threatening pulmonary disease or intestinal obstruction. Diagnosis is most frequently established by identification of the eggs in stool specimens. Treatment with antiparasitic drugs is effective in preventing serious complications and eradicating the parasite from all members of the household.

Spontaneous anastomosis of the esophagus in esophageal atresia.

An operative technique is described for the management of babies with esophageal atresia, where primary anastomosis is either impossible or unsuccessful. The procedure has been performed on six patients. One infant subsequently required resection of a resultant stricture, and one developed a recurrent tracheoesophageal fistula. All six infants required several esophageal dilatations. No patient required dilatation beyond the age of 5 months and all are now swallowing normally and thriving.

Unsuspected esophageal foreign bodies.

Retrospective study of ten infants and young children in whom an unsuspected foreign body was found in the esophagus revealed that all had had wheezing of difficulty swallowing solid food. In four, stridor or wheezing had been ascribed to respiratory infection. All foreign bodies were in the upper esophagus. Their presence can cause prolonged, severe morbidity. They can be identified by radiography of the neck and chest, occasionally supplemented by barium studies.

Pseudotumours of the oropharynx due to muscular contraction.

Normal filling defects in the barium-filled oropharynx are described. These pseudotumours result from the contraction, elevation and backward pulling of the soft palate assisted by the rounded ridge (of Passavant) in the pharyngeal wall produced by contraction of the palatopharyngeal sphincter, and the action of the palatopharyngei during swallowing. Other anatomical explanations for these filling defects are mentioned. The importance of recognising the changing pattern of these defects as being normal, and their differentiation from space occupying lesions in the oropharynx is emphasised.

Early injection laryngoplasty for acute unilateral vocal fold paralysis after thoracic aortic surgery.

OBJECTIVE: Unilateral vocal fold paralysis (UVFP) following open thoracic aortic surgery increases pulmonary complications and hospital stays. An intervention protocol with early injection laryngoplasty (IL) and swallowing maneuvers was developed for acute UVFP following thoracic aortic surgery. This study aimed to compare the incidence of complications and length of medical care between the non-VFP and the IL-UVFP group managed under this protocol. METHODS: Patients who underwent open thoracic aortic surgery from March 2020 to February 2023 were included, excluding those with preoperative VF

Predictors of the experience of a Cytosponge test: analysis of patient survey data from the BEST3 trial.

BACKGROUND: The Cytosponge is a cell-collection device, which, coupled with a test for trefoil factor 3 (TFF3), can be used to diagnose Barrett's oesophagus, a precursor condition to oesophageal adenocarcinoma. BEST3, a large pragmatic, randomised, controlled trial, investigated whether offering the Cytosponge-TFF3 test would increase detection of Barrett's. Overall, participants reported mostly positive experiences. This study reports the factors associated with the least positive experience. METHODS: Patient experience was assessed using the Inventory to Assess Patient Satisfaction (IAPS),

Transoral Robotic Surgery for Early-T Stage Glottic Cancer Involving the Anterior Commissure-News and Update.

BACKGROUND: About 20% of all glottic carcinomas involve the anterior commissure (AC), and AC involvement was deemed to be a risk factor of local recurrence and poor prognosis. Transoral robotic surgery (TORS) has been developed for a panoramic view of the AC and en-bloc resection of the tumor by multidirectional dissection with endo-wristed instruments. With satisfactory preliminary results, we would like to update the data with a bigger cohort and present the news on using TORS for salvage treatment of recurrence from irradiation failure. METHODS: From July 2010 to December 2019, 22 patients

Lesion location and other predictive factors of dysphagia and its complications in acute stroke.

BACKGROUND AND AIMS: Early detection of dysphagia is crucial in stroke patients as a result of increased morbidity and mortality due to malnutrition and respiratory tract infections. The aim of this study was to identify possible predictors of the onset of dysphagia following stroke in order to be able to act precociously. METHODS: Observational, prospective study in which a Volume-Viscosity Swallow Test (V-VST) was carried out in the first 72 h following admission to assess dysphagia in acute stroke patients with a previous result of <3 in the Eating Assessment Tool-10. Lesions were analysed

Increased Interval From Initial Evaluation to Initial Voice Therapy Session is Associated With Missed Voice Therapy Appointments.

BACKGROUND: Voice therapy (VT) is an essential component to a successful treatment of laryngological disorders and thus patient outcomes depend on patient compliance with VT. As volume of VT patients' ebb and flow, there is often variation in wait times for initial VT sessions. There is a paucity research evaluating VT latency and missed appointments. It is important to find associations between latency and patient compliance to assist with increasing the quality of patient care in all voice and swallowing clinics and decreasing missed appointments for VT following. Should a clinical setting b
