Target-controlled dosing of remifentanil during cardiac surgery reduces postoperative hyperalgesia.

OBJECTIVE: One of the strategies to attenuate opioid-induced hyperalgesia (OIH) may be to decrease intraoperative doses of opioids by using target-controlled infusion (TCI). DESIGN: Double-blind and randomized study. SETTING: A single university hospital. PARTICIPANTS: Forty American Society of Anesthesiologists II to III patients scheduled for elective cardiac surgery. INTERVENTIONS: patients were randomized to 1 of the 2 groups: 1 group received an infusion of intraoperative remifentanil using TCI (target: 7 ng/mL), and the 2nd one was given an intraoperative continuous infusion (CI) (0.

THE ACTION OF SODIUM 4-HYDROXYBUTYRATE ON SPINAL REFLEXES.

The actions of sodium 4-hydroxybutyrate, gamma-aminobutyric acid and meprobamate have been studied in unanaesthetized animals, in local anaesthetic tests, on isolated organ preparations, on convulsions induced by picrotoxin and strychnine, and on monosynaptic (patellar) and polysynaptic (plantar) reflexes of the spinal cord. Sodium 4-hydroxybutyrate induced a sleep-like state with three unusual features: the righting reflex was remarkably persistent, respiration was good throughout and recovery was abrupt. gamma-Aminobutyric acid was inactive and meprobamate caused flaccid paralysis with loss

Shoulder anterior capsular (SHAC) block for analgesia in anterior shoulder dislocation: An emergency department case series.

BACKGROUND: Anterior shoulder dislocation is among the most common major joint dislocations presenting to the emergency department (ED) and is frequently managed with procedural sedation. Several analgesic and regional anesthesia strategies are available for shoulder reduction, each with distinct advantages and limitations. The Shoulder Anterior Capsular (SHAC) block is a superficial, ultrasound-guided interfascial plane technique that targets pericapsular nerve contributions to the glenohumeral joint via an anterior approach, offering a potentially motor-sparing and sedation-sparing alternati

Predicting postoperative delayed awakening after flexible ureteroscopic lithotripsy based on radiomic features of body composition.

We aimed to explore the predictive role of radiomic features of body compositions in the occurrence of delayed awakening after flexible ureteroscopic lithotripsy, and further develop a predictive model to identify patients at higher risk. We analyzed the data from 314 admitted patients undergoing total intravenous general anesthesia at two institutions. Binary logistic regression was used to assess univariable and multivariable associations of clinical factors with delayed awakening. Radiomic features of body compositions were deeply extracted from preoperative computed tomography (CT) scans u

Functional and reperfusion outcomes after mechanical thrombectomy at high altitude: a 32-patient series from Quito, Ecuador.

INTRODUCTION: Mechanical thrombectomy (MT) is the standard of care for acute ischemic stroke due to large vessel occlusion. In Ecuador, no previous national data on MT outcomes have been reported, and, to our knowledge, there are no published series performed at high altitude. METHODS: This was a retrospective, observational, descriptive study based on consecutive patients treated within a multicenter mechanical thrombectomy program in Quito, Ecuador, located at an altitude of ≈2,800 meters above sea level. Thirty-two patients treated between October 2023 and November 2024 were included. Base

Antiarrhythmic Potential of the β-Adrenoceptor Blocker Metoprolol against Dofetilide-Induced Torsades de Pointes in Anesthetized Bradycardic Rabbits.

We have established a proarrhythmic rabbit model of acute atrioventricular block (AVB) with severe bradycardia to detect delayed rectifier K+ channel current (IKr) blocker-induced torsade de pointes (TdP). To better understand the role of β-adrenoceptors in this model, we investigated the effect of the β-adrenoceptor blocker metoprolol against an IKr blocker dofetilide-induced TdP. AVB was induced by catheter ablation under isoflurane anesthesia, and the ventricles were electrically paced at a constant rate of 60 beats/min throughout the experiments. Monophasic action potentials (MAPs) were re

Short-term corticosteroid therapy in aspiration pneumonitis complicated by acute respiratory distress syndrome: A case report.

RATIONALE: Aspiration of gastric contents (Mendelson syndrome) can cause chemical pneumonitis and quickly progress to acute respiratory distress syndrome (ARDS). Evidence for corticosteroids in this setting is limited, and guidelines are unclear. We describe a postpartum case of aspiration pneumonitis evolving to ARDS that improved after a short course of methylprednisolone. PATIENT CONCERNS: A 27-year-old woman developed severe dyspnea, chest tightness, and hypoxemia after a cesarean section performed under general anesthesia. DIAGNOSES: On arrival at Cho Ray Hospital, she was tachypneic wi

Pediatric Cryptococcosis Presenting as a Mediastinal Mass: A Case Report And Endobronchial Ultrasound-Guided Transbronchial Needle Aspiration Protocol.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is a minimally invasive diagnostic technique. While widely used in adults, its application for diagnosing mediastinal masses in pediatric patients remains underreported. This gap poses challenges when addressing atypical presentations, such as pulmonary cryptococcosis in immunocompetent children, which is frequently misdiagnosed due to nonspecific symptoms. We present the case of a healthy nine-year-old male with an eight-day history of fever and cough. Computed tomography revealed a mediastinal mass in the left hilum

Comparing Single Dose Versus Multiple Doses Antibiotic Prophylaxis in Preventing Surgical Site Infections Following Open Appendicectomy for Uncomplicated Appendicitis.

BACKGROUND: Acute appendicitis is the leading cause of acute surgical abdomen. Studies have shown that a single dose of antibiotics is as effective as multiple doses in preventing surgical site infections (SSI) after appendectomy, with advantages such as cost savings, fewer adverse reactions, and reduced risk of antibiotic resistance. Despite these benefits, multiple doses are still commonly used. This study compared the efficacy of single-dose versus multiple-dose antibiotic prophylaxis in preventing SSI after open appendectomy for uncomplicated appendicitis. METHODOLOGY: A one-year prospect

Does persistent crossover (ipsilateral) motor evoked potential (MEP) responses represent a technical failure for intracranial motor tract monitoring? A case example and practical solution.

PURPOSE: Attention to motor evoked potential (MEP) stimulation intensity is necessary to avoid false negative MEP results during intracranial procedures. Observing ipsilateral (crossover) MEP responses has been hypothesized to indicate inappropriately strong stimulation intensity. We describe a case where persistent crossover MEP responses falsely suggested that stimulus intensity was too high and describe an alternative method to guide the selection of MEP stimulation intensity. METHODS: A patient undergoing a suboccipital craniotomy for tumor resection had bilateral transcranial electrical

A Novel Technique for Intraoperative Mapping of the Somatosensory Cortex.

OBJECTIVE: Intraoperative mapping of the nervous system is used to identify "eloquent" cortical areas. In this technical report, we describe a novel way of mapping the somatosensory cortex so that injury to those critical pathways can be avoided. METHODS: An 8-year-old female with drug resistant epilepsy presented for resection of a right posterior parietal focal cortical dysplasia. Left median nerve stimulation was used to record somatosensory evoked potentials (SEPs) directly from the somatosensory cortex with a strip electrode. A handheld monopolar electrode was also used to record both th
