Double semi-tubular plating of clavicle using a piggyback technique--an alternative way of treating clavicle mid-shaft fractures in young patients.

The majority of clavicle fractures unite uneventfully. Indications for operative surgery are limited, with high rates of complications. We describe our results with an alternative cost effective method of treating these injuries in young patients. It is a retrospective series of 20 patients treated between January 2005 and January 2009 with a "piggy back" technique using double plating with two 3.5 mm drill hole semitubular plates. All patients were assessed with DASH (Disability of Arm, Shoulder and Hand) score at last review. Average time to union was 12.5 weeks. The average DASH score was 8

Pathological fracture after migration of cement used to treat distal femur physeal arrest.

Treatment of physeal arrest with bar removal and placement of interposition materials in young patients has been shown to restore physeal growth. Among the various materials that have been used to prevent early reformation of the physeal bar (fat, silastic, cartilage), Peterson recommended the use of Cranioplast, as this material can prevent bar reformation, and it is radiolucent because it does not contain barium. Peterson suggested steps be taken to prevent migration of the Cranioplast, as migration of the interposition material might allow for reformation of the physeal bar. Although Peters

Percutaneous pins versus volar plates for unstable distal radius fractures: a biomechanic study using a cadaver model.

PURPOSE: A biomechanic study using a cadaver model of a dorsally unstable distal radius fracture was used to compare the stability of percutaneous pinning and volar fixed-angle plating. Among the many surgical options for treating distal radius fractures are percutaneous pinning and internal plate fixation. Although percutaneous pin fixation requires less soft-tissue trauma and has low complication rates, plate fixation allows for early active movement with good clinical results. The biomechanic stability of these 2 methods was studied by using a cadaver model of a dorsally unstable intra-arti

Step ostectomy as a treatment for varus deformity of a metatarsophalangeal joint in a 4.5-month-old colt.

A 4.5-month-old quarter horse colt was presented with severe right hind metatarsophalangeal varus, present since birth. On radiographs, it was determined that there was a 16 degree deviation with the pivot point at the distal physis of the 3rd metatarsal bone. A step ostectomy in the sagittal plane corrected the deformity.

Plate and screw design in fractures of the hand and wrist.

UNLABELLED: As internal fixation of fractures of the hand and wrist has evolved, so too have the implants used for the same purpose. Implants used for the first (1/2) of the 20th century were modeled on implants used to fix long bone fractures. Consequently, they were often bulky. In addition, they often were made of materials which were ill suited to the unique functional and biomechanical requirements of the hand and wrist. As a result, outcomes of internal fixation frequently were suboptimal and complication rates were high. In the past 40 years there has been an evolution in metallurgy, bi

Comminuted fractures of the radial head: comparison of resection and internal fixation. Surgical technique.

BACKGROUND: Satisfactory internal fixation of comminuted radial head fractures is often difficult to achieve, and radial head resection has been the accepted treatment. In this study, we compared the results of radial head resection with those of open reduction and internal fixation in patients with a comminuted radial head fracture. METHODS: Twenty-eight patients with a Mason type-III radial head fracture (some with associated injuries) were enrolled in the study. Fifteen patients underwent radial head resection as the initial treatment (Group I), and thirteen patients underwent open reducti

Percutaneous insertion of a proximal humeral locking plate: an anatomic study.

This cadaveric study sought to evaluate the feasibility of applying a locking proximal humerus plate with a novel minimally invasive technique. A unique pre-contoured locking plate was placed on cadaveric proximal humeri through a described minimally invasive approach. Proximity of the plate and screws to the axillary nerve and their respective surgical tracks were quantified. Safe screw hole placement with respect to the axillary nerve was determined. Risk of entrapment of the nerve beneath the plate was evaluated. Three of the holes near the middle of the locking plate consistently intersect

Locking compression plate fixation of radial and tibial fractures in a young dog.

A six-month-old, male Bernese Mountain Dog in which radius-ulna and tibia-fibula concomitant fractures were treated each with a 3.5 mm Locking Compression Plate (LCP) is presented. Both fractures were approached and plated medially. The tibial fracture had to be revised with a 4.5 mm intramedullary nail and a new 3.5 mm LCP at the second post-operative day because of fixation breakdown. The follow-up radiographs taken at days 14 and 53 revealed uneventful healing of both fractures. Implants were removed 53 days after surgery.

Open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus.

BACKGROUND: Controversy persists concerning the preferred treatment of displaced fractures of the proximal part of the humerus. The present study was undertaken to evaluate the results of open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus and the functional limitations of patients in whom avascular necrosis of the humeral head develops as a complication of this fracture. METHODS: We assessed the intermediate and long-term results for sixty patients with a three or four-part fracture of the proximal part of the humerus who had undergone op

A newly designed miniplate staple for high tibial osteotomy.

A biomechanical study was made to compare the mechanical performance of the newly designed Miniplate staple to the conventional Coventry staple in high tibial osteotomy (HTO). Using twenty fresh porcine tibiae, the fixational strength of the two different types of staples in HTO was compared. To minimize the error due to the specimen-to-specimen individuality, the bone mineral density of the tibiae was measured with bone densitometry and those with 0.8 to 1.2 gm/cm2 at the proximal tibia were used in the biomechanical test. Testing was performed on a material testing system with aid of a comme

Wurzburg lag screw plate versus four-hole miniplate for the treatment of condylar process fractures.

PURPOSE: The purpose of this study was to compare the biomechanical stability of rigidly fixed condylar neck fractures using either a conventional four-hole miniplate system or the Wurzburg lag screw plate system. MATERIALS AND METHODS: Ten identical synthetic mandibles were used for this study. Simulated bilateral condylar neck fractures were fixed with the lag screw plate system on one side and the miniplate system on the other. The mandibles were loaded with an MTS servohydraulic testing machine at a rate of 1 cm/min until failure was reached. Data for resistance to motion and ultimate str

Fractures of the os calcis: improved results with internal fixation.

A prospective study of 24 patients with displaced intra-articular fractures of the os calcis was performed to compare operative with non-operative treatment. Twelve patients were treated by internal fixation followed by early mobilization, and 12 were managed non-operatively. The patients were assessed 15 months later using eight parameters. Of the 12 patients who were treated by internal fixation, eight had returned to work. Walking distance was significantly longer (4 km) in this group and only one patient needed a change in shoe size. Of the 12 patients who were managed non-operatively, onl

[The surgical treatment of thoracic wall instability].

Cardinal symptom of the instability of thorax with paradoxical respiration is the irreversible respiratory insufficiency. The lethality is over 50%. For the treatment often besides the longterm respiration with the well known complications the operative stabilisation of the chest wall by osteosynthesis of the ribs by small plates. Thereby the mortality of this severe injury can be reduced significantly, if there is a good indication for osteosynthesis.

[Is a radial nerve paralysis in a humeral shaft fracture an indication for surgery? Indications].

Surgery for humeral shaft fractures. A series of 78 operative treated humeral shaft fractures was studied retrospectively. Sixteen times we found a radial nerve palsy, preoperatively. In this group we evaluated the type of fracture, the operation indication, the time of treatment and the postoperative recovery of the radial nerve palsy. Fourteen times we did an early exploration. The indication to operate was 9 times the conducting polytrauma, 5 times the radial nerve palsy itself and 2 times a pseudarthrosis. In 13 of the 16 patients nerve function recovered either clinically and/or electromy
