Successful treatment of cutaneous and subcutaneous zygomycosis in an immunosuppressed patient with aplastic anaemia.

Zygomycosis in patients with persistent neutropenia had been associated with poor outcomes despite aggressive surgical and antifungal therapy. We describe the case of a 10-year-old girl with aplastic anaemia and persistent neutropenia who developed cutaneous and subcutaneous zygomycosis of her right thigh that was successfully treated with extensive surgical debridement, intravenous liposomal amphotericin B, later changed to oral posaconazole for long-term suppressive therapy and granulocyte colony stimulating factor.

Assessment of a new selective chromogenic Bacillus cereus group plating medium and use of enterobacterial autoinducer of growth for cultural identification of Bacillus species.

A new chromogenic Bacillus cereus group plating medium permits differentiation of pathogenic Bacillus species by colony morphology and color. Probiotic B. cereus mutants were distinguished from wild-type strains by their susceptibilities to penicillin G or cefazolin. The enterobacterial autoinducer increased the sensitivity and the speed of enrichment of B. cereus and B. anthracis spores in serum-supplemented minimal salts medium (based on the standard American Petroleum Institute medium) and buffered peptone water.

Pediatric cervicofacial actinomycosis: a case report.

Cervicofacial actinomycosis affects many soft tissue and bony structures in the head and neck, and has both granulomatous and suppurative features. Pathogenesis of actinomycosis is still unclear, but trauma provides a portal of entry for the infection. It usually presents as a diffuse swelling with multiple sinus tracts containing macroscopic colonies of the organism known as "sulphur granules." Cervicofacial actinomycosis in children is rare. This article reports a case of actinomycosis in a 10-year-old-boy overlying the left ramus of the mandible.

Pulmonary toxicity associated with nitric oxide in term infants with severe respiratory failure.

We prospectively analyzed airway specimens from 24 newborn infants. Inhaled nitric oxide (< or = 20 ppm for 1 to 4 days to 12 infants) did not affect the concentrations of the lipid peroxidation product, the surface activity, or the cytokines (interleukin-1, granulocyte-macrophage colony-stimulating factor, interleukin-1 receptor antagonist). Nitrotyrosine was detected after 10 days of life in the two infants requiring prolonged ventilation, suggesting toxicity of endogenous nitric oxide.

Triaga Brasilica: renewed interest in a seventeenth-century panacea.

Theriacs (electuaries prepared by mixing extracts of many plants) were known from antiquity until the eighteenth century as remedies for all kinds of envenomation, above all those due to the bites and stings of venomous animals, especially snakes. In colonial Brazil, the 'Brazilian theriac' was developed by Jesuit priests by gradually substituting native plants for components of their European model. Most of these ingredients, mentioned in an old manuscript, can be identified by their common names, which have survived the centuries.

Fungal meningitis and intracerebral hemorrhage complicating prosthetic valve endocarditis.

A 50 yr old man presented with meningitis 163 days following aortic valve replacement for congenital bicuspid aortic stenosis. One week later he died of intracerebral hemorrhage and autopsy revealed fungal prosthetic valve endocarditis. Colonies of branching septate hyphae were adherent to the oculomotor nerves and left posterior communicating artery. This pattern of fungal basal meningeal involvement complicating prosthetic valve endocarditis has not been previously described.

GM-CSF therapy for drug-induced agranulocytosis.

Five patients with drug-induced agranulocytosis were treated with recombinant granulocyte-macrophage colony-stimulating factor (GM-CSF)). With the exception of one patient who experienced a very short period of neutropenia, GM-CSF did not appear to alter the natural course of the disease. From our experience and from a critical review of the literature we conclude that the efficacy of GM-CSF in agranulocytosis is far from certain, and that the use of growth factors in this condition remains investigational.

Urinary tract infections in children.

A five-year review of pediatric patients with urinary tract infections shows that absolute colony counts are not satisfactory as the sole criterion for urinary tract infection. Prior to culture results, patients can be treated empirically if the urine is nitrite-positive or if there is more than an occasional organism per high power field on unspun urine. In this series, no diagnostic or therapeutic benefit resulted from localizing studies. Patients with enuresis and infection should undergo radiologic evaluation for an anatomic abnormality.

Cell-mediated immunity to measles virus in subacute sclerosing panencephalitis.

Four patients with subacute sclerosing panencephalitis were investigated for their specific cellular and humoral immunities against measles virus. Lymphocytotoxicity mediated by peripheral blood lymphocytes was evaluated with the colony inhibition test of target cells having measles antigen. The lymphocytes from two patients of subacute sclerosing panencephalitis (SSPE) specifically destroyed the carrier cells; however, no significant lymphocytotoxicity was observed in the other two patients. The result suggests the heterogeneity in cellular immune states in SSPE patients.

Tumor-Associated Macrophage Promotes the Survival of Cancer Cells upon Docetaxel Chemotherapy via the CSF1/CSF1R-CXCL12/CXCR4 Axis in Castration-Resistant Prostate Cancer.

Castration-resistant prostate cancer (CRPC) is an advanced stage of prostate cancer that can progress rapidly even in patients treated with castration. Previously, we found that tumor-associated macrophages (TAM) can be recruited by CSF-1 secreted by docetaxel-treated prostate cancer cells and promote the survival of cancer cells in response to chemotherapy. The inhibition of CSF-1R can impede this effect and significantly prolong survival in xenograft mice. However, the actual mechanism of how TAM improves cancer cell survival still remains elusive and controversial. Here, for the first time,

Unimpaired macrophage differentiation and activation in mice lacking the zinc finger transplantation factor NGFI-A (EGR1).

The zinc finger protein NGFI-A (also called EGR1, Krox24, or zif268) is a candidate regulator of myeloid cell differentiation. Evidence supporting this hypothesis is twofold. First, NGFI-A antisense oligonucleotides prevent macrophage differentiation in HL-60 and U937 myeloid leukemia cell lines and in normal bone marrow cells. Second, enforced expression of NGFI-A blocks granulocytic differentiation and promotes macrophage differentiation in HL-60 cells and in the hematopoietic progenitor cell line 32D. We sought to determine the effect of NGFI-A deficiency on macrophage differentiation and f

INDICATIONS FOR REMOVAL OF A PORT SYSTEM IN A PATIENT WITH SUSPECTED SEPTICEMIA IN THE ABSENCE OF OTHER LOCAL SOURCES OF INFECTION.

The integrity of the skin is compromised during the placement of various implantable devices (arterial and venous catheters, port systems, tracheostomies, cholangiostomies, nephrostomies). Currently, there are no materials used for these devices that are completely biologically inert. Microorganisms adhere to their surfaces, forming individual colonies that relatively quickly (within 24 hours) develop into a continuous biofilm. Most microorganisms within the biofilm remain in a dormant state; however, localized foci of proliferation periodically arise, leading to the development of an infectio

Maxillary sinus actinomycosis: a diagnostic and therapeutic challenge.

The authors present a case of maxillary sinus actinomycosis in a young adult woman. This is a rare condition whose unspecific clinical presentation makes its diagnosis challenging. In this case, the diagnosis was given by the identification of Actinomyces colonies in samples of infected tissue.Treatment consisted of a combined medical and surgical approach. Endoscopic sinus surgery was performed to remove diseased mucosa and to aerate the involved sinuses, followed by long-term antibiotic therapy. No recurrence of the disease was observed during follow-up.

Cell Viability Assays for Candida auris.

Cell viability assays are useful for assessing the efficacy of antifungal therapeutics and disinfection strategies in vitro. In recent years these assays have been fundamental for the testing of conventional and novel therapies against the nosocomial fungal pathogen Candida auris. Here we provide detailed descriptions of methods for assessing cellular viability of Candida auris in vitro, such as metabolic assays (XTT and resazurin), colony-forming unit counting, live/dead quantitative PCR, and fluorescent staining for microscopic analyses.

Diffusion-Weighted Imaging is Key to Diagnosing Specific Diseases.

This article reviews diseases for which persistent signal abnormalities on diffusion-weighted imaging are the key to their diagnosis. Specifically, updated knowledge regarding the neuroimaging patterns of the following diseases is summarized: sporadic Creutzfeldt-Jakob disease, neuronal intranuclear inclusion disease, and hereditary diffuse leukoencephalopathy with axonal spheroids-colony-stimulating factor receptors/adult-onset leukoencephalopathy with axonal spheroids and pigmented glia. In addition, their differential diagnoses; clinical manifestations; and pathologic, genetic, and imaging

Onychomycosis caused by Aspergillus versicolor.

We report a case of onychomycosis caused by Aspergillus versicolor in a 66-year-old female patient. The infection was characterised clinically by yellowish pigmentation of the nail plate and mild nail bed hyperkeratosis of the first left toe. All other nails were normal. Three direct microscopical examinations of nail samples revealed the presence of hyaline hyphae as well as conidiophores. Pure colonies of A. versicolor were found in three cultures. The patient was successfully treated with oral itraconazole.

Isolation of Brucella melitensis from an abscess on the left foot of a 3-year-old infant.

An unusual case of a 3-year-old girl with a brucellar foot abscess is reported. Although direct microscopic examination of samples from the lesion did not reveal micro-organisms of any kind, a 7 day culture of caseous material yielded small colonies of Gram-negative cocobacilli in Löwenstein-Jensen medium. These were biochemically and molecularly identified as Brucella melitensis. The possibility of foot abscess being caused by Brucella should be considered in countries where brucellosis is endemic.

Efficacy of orally delivered cochleates containing amphotericin B in a murine model of aspergillosis.

Cochleates containing amphotericin B (CAMB) were administered orally at doses ranging from 0 to 40 mg/kg of body weight/day for 14 days in a murine model of systemic aspergillosis. The administration of oral doses of CAMB (20 and 40 mg/kg/day) resulted in a survival rate of 70% and a reduction in colony counts of more than 2 logs in lungs, livers, and kidneys. Orally administered CAMB shows promise for the treatment of aspergillosis.

Sweet syndrome associated with G-CSF treatment in a child with glycogen storage disease type Ib.

Acute febrile neutrophilic dermatosis (Sweet syndrome) is rare in children and is regularly associated with underlying malignancies or inflammtory diseases. A 5-year-old girl with glycogen storage disease type Ib, neutropenia, and recurrent infections developed characteristic skin eruption of Sweet syndrome after 2 years of granulocyte colony-stimulating factor (G-CSF) therapy. This association points to a possible role of G-CSF-induced granulopoiesis and granulodyte activation in the pathogenesis of Sweet syndrome.

Experimental infection of the conjunctival sac of lambs with Mycoplasma conjunctivae.

Ovine keratoconjunctivitis (OKC) identical to that seen in field cases was produced experimentally with 10(6) colony forming units of an isolate of Mycoplasma conjunctivae which had been passaged three times in vitro. Cytological studies showed that the peak neutrophil count in conjunctival exudate occurred 8 days after inoculation of the conjunctival sac. Rapid onset of clinical signs in the uninoculated eye showed the ease with which the organism could be transferred from affected to unaffected eyes.
