Surgical palliation of small bowel obstruction due to metastatic carcinoma.

Seventy-three patients with small bowel obstruction due to metastatic carcinoma were seen in the years 1960 to 1979. Twenty-nine patients were seen in the first decade and 44 patients in the second. The most common primary tumor causing metastatic small bowel obstruction was colonic carcinoma, followed by gastric carcinoma. Plain x-ray examinations supplemented by an upper gastrointestinal series with small bowel follow-through were the most useful diagnostic tests. Seventy per cent (51/73) of these patients were initially treated with intravenous fluids and gastrointestinal decompression usin

An Abnormal Presentation of a Popliteal Artery Mass and a Unique Cause of Claudication.

Multiple hereditary exostosis (MHE) is a rare autosomal-dominant disorder marked by multiple osteochondromas arising from the metaphyses of long bones. Parameniscal cysts, often linked to meniscal tears, can also expand and compress nearby vessels, posing a potential vascular risk. We present a 28-year-old male with MHE and a parameniscal cyst causing popliteal artery compression. The patient's intermittent claudication resolved completely following surgical cyst decompression and meniscal repair. This case report highlights the importance of considering vascular compression in patients with M

Extracranial Meningiomas of the Left Neck.

Extracranial meningiomas can present in atypical locations and may have distinct symptoms that require specific treatment strategies based on tumor size, location, and effects on adjacent structures. Herein, we present a case of extracranial meningioma on the left side of the neck in a 64-year-old female. The diagnosis was confirmed via histological examination and immunohistochemical staining. The patient underwent a surgical procedure that included histological biopsy and resection of the neck soft tissue lesion combined with peripheral nerve decompression, which is the standard treatment fo

Five-step unilateral biportal endoscopic surgery for central lumbar canal stenosis: "Z" technique nuance.

The treatment for lumbar spinal stenosis has advanced through the use of minimally invasive surgery techniques. Endoscopic methods go even further, with studies showing that both uniportal and biportal endoscopic techniques have outcomes comparable to traditional approaches. However, there is limited knowledge of the step-by-step decompression process when using the unilateral biportal endoscopic (UBE). To address this, the authors introduce the five steps in the "Z" sequence, which aims to reduce surgical time and complications. The video can be found here: https://stream.cadmore.media/r10.31

Conservative management of gastric pneumatosis following left gastric artery embolisation.

Gastric pneumatosis, the presence of air within the stomach wall, is a very rare occurrence with poor outcomes. One of the most common mechanisms for gastric pneumatosis is gastric ischaemia, also a rare entity. Although patients with gastric ischaemia may require surgical intervention, they can often be treated with conservative measures such as a proton pump inhibitor, broad-spectrum antibiotics, nasogastric tube decompression, fluid resuscitation and total parenteral nutrition. We report a rare case of gastric ischaemia and pneumatosis following therapeutic left gastric artery argon plasma

Diagnostic challenges in cerebral tuberculoma presenting with seizures in pregnancy.

Tuberculoma is an uncommon presentation of tuberculosis and is found in regions with a high prevalence of tuberculosis. This is rarely diagnosed during pregnancy. The presentation can mimic other etiologies such as eclampsia or cerebral venous sinus thrombosis so the diagnosis can be challenging, particularly when presenting with seizures in pregnancy. Described here is a woman in her first pregnancy who presented with seizures mimicking eclampsia and was suspected to have a brain tumour on neuroimaging. She was diagnosed to have a intracerebral tuberculoma on histopathological examination fol

Iatrogenic Vertebral Fracture During Intramedullary Nailing for Femoral Fracture: A Case Report.

CASE: A 73-year-old woman with advanced ankylosing spondylitis (AS) underwent closed reduction and internal fixation using antegrade intramedullary nailing because of midshaft fracture of her right femur. After the surgery had been performed, a fracture and dislocation at T12-L1 was detected. Therefore, emergency spinal decompression and posterior instrumentation placement from T11 to L5 were then performed. CONCLUSION: Patients with advanced AS have a high risk of vertebral fracture, so special care must be taken not to transmit excessive stress on the spine for surgical reduction and manipu

Often Overlooked Diagnosis: Median Arcuate Ligament Syndrome as a Mimicker of Crohn's Disease.

Median arcuate ligament syndrome occurs when the celiac artery and/or the celiac plexus nerves is compressed by the median arcuate ligament during expiration causing a variety of gastrointestinal symptoms. Here, we present a case of median arcuate ligament syndrome in a persistently symptomatic 35-year-old man that presented as a mimicker of Crohn's disease. Symptomatology, computed tomography angiography, and abdominal ultrasound Doppler were consistent with celiac artery compression syndrome. After surgical decompression of the ligament and removal of the celiac ganglion, he reported a defin

Elbow Arthroscopy for Treatment of Valgus Extension Overload.

Valgus extension overload syndrome (VEO) is the result of supraphysiologic stresses placed across the posterior elbow during pitching. Following failure of nonoperative measures, surgical options consist of arthroscopic or limited incision posteromedial decompression. Although technically challenging, arthroscopic treatment offers many advantages over open treatment, including improved joint visualization, decreased soft-tissue dissection, decreased postoperative pain, and quicker rehabilitation. Arthroscopic treatment of VEO consists of soft tissue and bony debridement, loose body removal, an

Overview and History of Trigeminal Neuralgia.

Although the symptoms associated with trigeminal neuralgia have been well documented, the root cause of this disease initially eluded most surgeons. Although early remedies were haphazard because of a lack of understanding about the condition, near the 20th century both medical and procedural therapies were established for the treatment of trigeminal neuralgia. These treatments include a variety of medications, chemoneurolysis, radiofrequency lesioning, percutaneous ablative procedures, stereotactic radiosurgery, and open rhizotomy and microvascular decompression. This report recounts the hist

Traumatic Lumbosacral Spondylolisthesis: A Case Report.

INTRODUCTION: Traumatic lumbar spondylolisthesis is rare disease and in the literature, different surgical approaches, including anterior, posterior, or combined approaches (posterior and anterior) are used to treat the lesion. CASE PRESENTATION: We treated a case of traumatic lumbosacral spondylolisthesis using posterior approach and the patient showed a satisfactory outcome. At the final follow-up, he was completely asymptomatic, and radiographic images revealed normal lumbar alignment and a solid interbody fusion. CONCLUSIONS: Traumatic lumbosacral spondylolisthesis can be treated using p

Penetrating intracranial gunshot wound transecting the right transverse sinus.

A 23-year-old man sustained a gunshot wound to the posterior head. Imaging demonstrated a transection of the right transverse sinus, a retained bullet fragment and significant cerebellar oedema. The patient emergently underwent suboccipital decompression associated with brisk bleeding from the transverse sinus. Reported examples of surgical management of cerebral venous sinuses include: packing, grafting, patching and ligation. Our patient had a codominant transverse sinus and underwent successful unilateral ligation. His postoperative course was uneventful, however, he did require a ventricul

Intraneural hematoma after nerve stimulation-guided femoral block in a patient with factor XI deficiency: case report.

Intraneural hematoma after a peripheral nerve block is a theoretical cause of postoperative nerve injury that has not been previously documented in humans. We report an intraneural hematoma after nerve stimulation-guided femoral block, which occurred in a patient with undiagnosed factor XI deficiency and a slightly prolonged activated partial thromboplastin time. Onset of paralysis several days after surgery prompted early surgical exploration and removal of an intraneural hematoma at the femoral nerve. Improvement in motor function was noted after epineurolysis of the femoral nerve. Early ner

Axillary sheath hematomas causing neurologic complications following arterial access.

Iatrogenic brachial plexus injuries secondary to expanding hematomas and pseudoaneurysms have been described in limited nature in previously published data. We present the case of a 55-year-old woman who developed neurologic deficits because of a compressive hematoma after axillary arteriography. She underwent emergent exploration of her left arm with decompression of the axillary sheath and brachial artery repair with complete recovery. We describe the presentation, relevant anatomy, and importance of this condition and stress the need for early recognition and surgical intervention to preven

Spontaneous resolution of nontraumatic acute spinal subdural hematoma.

Spinal subdural hematoma (SSDH) is an extremely uncommon condition. Causative factors include trauma, anticoagulant drug administration, hemostatic disorders, and vascular disorders such as arteriovenous malformations and lumbar punctures. Of SSDH cases, those that do not have any traumatic event can be considered cases of nontraumatic acute spinal subdural hematoma, which is known to have diverse clinical progress. Treatment typically consists of surgical decompression and cases in which the condition is relieved with conservative treatment are rarely reported. We report two nontraumatic acut

Tubercular epidural abscess in children: report of two cases.

Tubercular spinal epidural abscess (SEA) is an uncommon infectious occurrence. SEA was first described in 1761. Clinical prospects of tubercular SEA are graver, if not promptly diagnosed and treated appropriately. We present two cases of spinal epidural abscess of tubercular etiology who progressed to paraplegia over the course of disease. MRI pointed towards an epidural abscess in the dorsal spine. Histopathological analysis revealed SEA, tubercular in nature in both cases. Patients responded to surgical decompression and anti-tubercular therapy (ATT). Such type of clinical entities has least

Two cases of tetralogy of Fallot with absent pulmonary valve repaired by modified Kreutzer's technique.

Two severely symptomatic infants of tetralogy of Fallot with absent pulmonary valve successfully repaired by a modified Kreutzer's technique are described. In addition to the standard repair of tetralogy of Fallot, our surgical technique consists of extensive pulmonary reduction arterioplasty, which allows tracheobronchial decompression. Both patients tolerated surgery and showed significant relief of airway compression and clinical symptoms, although they required prolonged mechanical ventilatory support. The modified Kreutzer's technique is relatively simple and could be sufficiently efficac

Nerve injuries in the throwing elbow.

The unique anatomy of the elbow combined with the angular velocity and stresses placed across this hinge joint while throwing can cause a large number of pathologic changes associated with nerves. Although the ulnar nerve is the most commonly injured, neuropathies are also seen with the branches of the median and radial nerves. These neuropathies are typically responsive to rest, activity modification, ice, splinting, and anti-inflammatories. A graduated return to throwing is then needed before returning to play. When conservative measures fail, surgical decompression is warranted, but results

[The treatment of acute hematogenous osteomyelitis in children and young adults].

On the basis of own experiences and literature from last three years authors presented the role of surgical treatment in children above two years and young people, suffering from acute haematogenous osteomyelitis. Conservative treatment should be limited to these cases, where we can see a quick recovery, reducing of the pain and decreasing of the level of CRP and ESR. The presence of subperiosteal abscessus, positive bacteriological culture from blood and other complications (i.e. staphylococcal pneumonia), are the absolute indications to decompression of the medullary cavity, and local applic
