Doxepin cream for eczema?

One of the more disruptive symptoms of eczema is itching. Traditionally, it is treated with emollients, topical corticosteroids or oral antihistamines. A topical form of the antidepressant doxepin (doxepin 5% cream; Xepin-Bioglan) is now marketed for "the relief of pruritus associated with eczema" in adults and children aged over 12 years. Here, we review the efficacy and safety of this product and assess its role in the management of eczema.

Onycholysis in a case of atopic eczema treated with PUVA photochemotherapy.

Onycholysis following the ingestion of psoralens and subsequent exposure to natural sunlight has been reported on several occasions and was first reported following photochemotherapy in 1978 by Ortonne and Baran from France and in 1979 by Mackie from Scotland. Mackie commented that she hoped to stimulate further reports of onycholysis induced by PUVA photochemotherapy in order to establish whether or not it was a definite complication of such treatment. Since then, there has been a dearth of similar reports. We describe a patient with severe atopic eczema and alopecia totalis who developed ony

Isolation and Purification of Th9 Cells for the Study of Inflammatory Diseases in Research and Clinical Settings.

Th9 cells are associated with atopic and inflammatory diseases, and their increased levels and function correlate with the severity of symptoms in various inflammatory disorders including asthma, food allergy, atopic dermatitis, ulcerative colitis, and psoriatic arthritis. Thus, clinical trials are warranted to evaluate the role of Th9 cells in allergic diseases with the goal of controlling these ailments.Circulating T cells (naïve or memory CD4+ T cells) purified from human blood and expanded using anti-CD3 and anti-CD28 antibodies can be treated with appropriate cytokines in order to polariz

Netherton's syndrome: a syndrome of elevated IgE and characteristic skin and hair findings.

Netherton's syndrome is a rare symptom complex characterized by greatly elevated IgE levels with atopic manifestations, an ichthyotic skin disorder (ichthyosis linearis circumflexa and/or congenital lamellar ichthyosis), and the characteristic hair abnormality trichorrhexis invaginata. It may be confused with atopic dermatitis but does not respond to topical corticosteroid treatment. Netherton's syndrome is therefore of considerable importance to allergists. In this article we report the results of the clinical and immunologic evaluation of a previously unreported patient who responded to trea

Immunohistochemical study of elevated expression of squamous cell carcinoma (SCC)-related antigens in erythrodermic epidermis.

Using immunohistochemical staining with the monoclonal antibodies (mAb) of squamous cell carcinoma-related antigens (SCC-RAg), the expression of SCC-RAg in erythrodermic epidermis, which included senile erythroderma following eczema, atopic erythroderma, psoriatic erythroderma, and Sézary syndrome was examined. In senile erythroderma and atopic dermatitis, the most intense staining with SCC-RAg mAb was evident in the upper epidermis. The cytoplasm of epidermal cells showed particularly strong staining. Although serum SCC-RAg in the Sézary syndrome was not as high as in other erythroderma patie

Use of Dupilumab to Treat Cutaneous Complications of Continuous Prostacyclin Infusion in Pulmonary Hypertension: A Case Report and Review of Literature.

Adhesive reactions are common complications of continuous treprostinil infusions, limiting their recommended use as a treatment for severe pulmonary hypertension. We present the case of a 10-year-old male with recurrent adhesive-related skin reactions, which compromised both subcutaneous and intravenous continuous treatment. Due to comorbid atopic dermatitis (AD), dupilumab was initiated to decrease skin reactions to adhesives. Within 48 h of initiation, skin reactions completely resolved. With sustained dupilumab treatment, he remains symptom-free and is successfully tolerating intravenous tr

Eczema Herpeticum: Would You Know It If You Saw It?

Eczema herpeticum is an easily missed entity most commonly seen in the pediatric population and carries the risk of systemic compromise and a 10% mortality rate. Clinicians should maintain high clinical suspicion when encountering children or young adults with a history of atopic dermatitis or other erosive dermatoses and who present with vesicular lesions, punched-out erosions, and systemic symptoms. We present 3 severe cases of eczema herpeticum that were potentially overlooked and demonstrate the need for elevated awareness to avoid potential pitfalls.

Routine lipid profiles in allergic disease: a mini review.

Allergic diseases are traditionally interpreted through epithelial barrier dysfunction, type 2 immune activation, and downstream inflammatory mediators, whereas routine serum lipid measures have usually been considered markers of cardiometabolic risk. However, emerging clinical and mechanistic evidence suggests that high-density lipoprotein (HDL), low-density lipoprotein (LDL), and triglycerides (TG) may provide additional information about systemic inflammatory and metabolic contexts in allergic diseases. This review synthesizes current evidence on routine lipid profiles across asthma, atopic

Ocular Adverse Events Associated With Dupilumab: Analysis From a Real-World Database.

PURPOSE: Dupilumab is a human monoclonal antibody used to treat various inflammatory conditions. The purpose of this study was to characterize the cumulative proportion of documented ocular adverse events after dupilumab initiation and compare outcomes between patients with atopic dermatitis (AD) and those treated for non-AD indications. METHODS: This retrospective cohort study used the TriNetX electronic health record network. Patients who initiated dupilumab and had at least 2 years of available follow-up after treatment initiation were included. Patients with documented ocular adverse even

Causal relationship between Faecalibacterium abundance and risk of Faecalibacterium-related diseases: a two-sample bi-directional Mendelian randomisation study.

Faecalibacterium is an essential probiotic in the human gut; changes in its abundance are associated with various disease states in many studies. However, the causal nature of such associations remains obscure. Therefore, we aimed to thoroughly investigate the causal relationships between Faecalibacterium and its related diseases. A two-sample bi-directional Mendelian randomisation analysis was conducted using publicly available genome-wide association studies summary statistics for Faecalibacterium and its related diseases. We found that Faecalibacterium was negatively correlated with the ris

Pan-JAK Inhibition Across Chronic Hand Eczema and Other Cutaneous Diseases: An Updated Review on Topical Delgocitinib.

Chronic hand eczema is a highly prevalent inflammatory skin disorder associated with substantial impairments in quality of life and social interactions. Topical corticosteroids and calcineurin inhibitors are conventional treatments often limited by the achievement of suboptimal disease control and concerns regarding long-term safety. Delgocitinib 2% cream, the first topical pan-Janus kinase inhibitor approved in Europe for adult patients with moderate-to-severe chronic hand eczema, represents a significant therapeutic advance. By simultaneously targeting multiple cytokine pathways implicated i

IRAK4 inhibition dampens pathogenic processes driving inflammatory skin diseases.

Innate immunity not only shapes the way epithelial barriers interpret environmental cues but also drives adaptive responses. Therefore, modulators of innate immune responses are expected to have high therapeutic potential across immune-mediated inflammatory diseases. IRAK4 is a kinase that integrates signaling downstream of receptors acting at the interface between innate and adaptive immune responses, such as Toll-like receptors (TLRs), interleukin-1R (IL-1R), and IL-18R. Because effects of IRAK4 inhibition are stimulus, cell type, and species dependent, the evaluation of the therapeutic pote
