Development of tolerance to clobazam in fully kindled rats: effects of intermittent flumazenil administration.

The effects of acute and chronic treatment with the 1,5-benzodiazepine, clobazam, were studied on fully kindled amygdaloid seizures in rats. After acute dosing, clobazam significantly reduced all parameters of kindled seizures (seizure severity, seizure duration, duration of amygdalar afterdischarges) at doses of 7.5 or 10 mg/kg i.p. 'Active' plasma concentrations of clobazam ranged between 300-800 ng/ml. The elimination half-life of clobazam in plasma was about 1 h. Only very low (10-75 ng/ml) levels of the major metabolite, N-desmethylclobazam, were detected in rats. Administration of N-desm

Patient Acceptance of Invasive Treatments for Tinnitus.

PURPOSE: The field of neuromodulation is currently seeking to treat a wide range of disorders with various types of invasive devices. In recent years, several preclinical trials and case reports in humans have been published on their potential for chronic tinnitus. However, studies to obtain insight into patients' willingness to undergo these treatments are scarce. The aim of this survey study was to find out whether tinnitus patients are willing to undergo invasive neuromodulation when taking its risks, costs, and potential benefits into account. METHOD: A Visual Analog Scale (VAS, 0-10) was

Sensing in Sacral Neuromodulation: Objective, Individualized Stimulation Response Thresholds Directly From the Lead.

OBJECTIVES: Sacral neuromodulation (SNM) therapy today relies on visually observed motor responses (e.g., anal bellows contraction, toe flexion) and subjective patient feedback (pelvic region sensations) to optimize lead placement and perform device programming. Measures of sacral nerve activation used in current practice include motor and sensory thresholds, described as the stimulation amplitudes associated with first observed or reported stimulation responses. This study evaluated the utility of sacral evoked responses (SERs), recorded directly from implanted commercial SNM leads, as object

Assessing the Pharmacotherapy and Clinical Outcomes After Deep Brain Stimulation for Treatment-Refractory Obsessive-Compulsive Disorder: A Case-Cohort Study.

Background: In the search for effective treatments for refractive obsessive-compulsive disorder (OCD), deep brain stimulation (DBS) serves as an alternative option for those with minimal response to pharmacotherapy. The rarity of reports regarding DBS use for OCD is attributed to the invasive nature of the procedure: placement of electrodes within targeted areas of the brain to provide neuromodulation. This treatment of last resort may decrease functional impairment and pharmacologic complications for a debilitating mental illness. This study compares the pharmacotherapy utilization and treatm

Late electrically-evoked compound action potentials as markers for acute micro-lesions of spiral ganglion neurons.

Cochlear implants (CIs) are the treatment of choice for profoundly hearing impaired people. It has been proposed that speech perception in CI users is influenced by the neural health (deafferentation, demyelination and degeneration) of the cochlea, which may be heterogeneous along an individual cochlea. Several options have been put forward to account for these local differences in neural health when fitting the speech processor settings, however with mixed results. The interpretation of the results is hampered by the fact that reliable markers of locally restricted changes in spiral ganglion

A novel assistive method for rigidity evaluation during deep brain stimulation surgery using acceleration sensors.

OBJECTIVE Despite the widespread use of deep brain stimulation (DBS) for movement disorders such as Parkinson's disease (PD), the exact anatomical target responsible for the therapeutic effect is still a subject of research. Intraoperative stimulation tests by experts consist of performing passive movements of the patient's arm or wrist while the amplitude of the stimulation current is increased. At each position, the amplitude that best alleviates rigidity is identified. Intrarater and interrater variations due to the subjective and semiquantitative nature of such evaluations have been report

Non-invasive assessment of left atrial activation using lead V8 during Bachmann bundle pacing.

BACKGROUND AND AIMS: Bachmann bundle pacing (BBP) improves atrial synchrony by advancing LA activation, but quantifying inter-atrial synchronization remains challenging. This study evaluated whether lead V8 reflects posterolateral LA activation time and whether lead V8 can be used to assess the effect of BBP on LA activation. METHODS: Consecutive patients undergoing an electrophysiology study and ablation (Cohort 1) or BBP lead implantation (Cohort 2) were prospectively enrolled. Surface ECGs were recorded with the V5 electrode repositioned to the V8 location (2 cm below the inferior left sca

Case Report: Spinal cord stimulation for neuropathic pain from spinal cysticercosis.

RATIONALE: Spinal cord cysticercosis, an exceptionally rare form of neurocysticercosis (NCC), often leads to refractory neuropathic pain and neurological deficits. Current treatments (e.g., antiparasitics or decompressive surgery) may fail to alleviate symptoms, necessitating alternative strategies. Spinal cord stimulation (SCS) is well-established for chronic pain but has rarely been reported for NCC-related pain. PATIENT CONCERNS: A 69-year-old female presented with an 8-year history of progressively worsening trunk and limb pain (burning sensation, VAS 8/10), sensory abnormalities, and low

Spinal cord stimulation restores locomotion in a Parkinson's disease patient and rodents.

INTRODUCTION: Dorsal column stimulation (DCS) of the spinal cord is emerging as a promising new technology to treat Parkinson's disease (PD). However, optimal stimulation settings that maximize its therapeutic effect on PD symptoms are yet to be determined. To optimize DCS therapy, it is necessary to understand its impact on pathological brain oscillations and to deliver stimulation triggered by neurophysiological biomarkers of PD. MATERIALS AND METHODS: We developed beta-triggered DCS (BT-DCS), where DCS was triggered by ongoing corticostriatal beta oscillations, and tested it in the bilater

DBS-STN&SNr combined stimulation versus DBS-STN monotarget stimulation for Parkinson's disease freezing: A comparative efficacy study.

BackgroundFreezing of gait (FOG) is a common and disabling symptom in patients with Parkinson's disease (PD), significantly impairing motor function and quality of life. While traditional deep brain stimulation (DBS) of the subthalamic nucleus (STN) provides some benefits, its efficacy in alleviating FOG remains limited. Combined stimulation of the STN and the substantia nigra pars reticulata (SNr) has recently emerged as a potentially superior approach.ObjectiveTo compare the efficacy of STN-only stimulation and combined STN + SNr stimulation in improving FOG symptoms and quality of life in p

A scoping review of neuromodulation techniques for controlling blood pressure: what are the ups and downs to this approach?

BACKGROUND AND OBJECTIVES: While there has been rapid progress in research aimed at developing device-based neuromodulation therapies for blood pressure (BP) disorders, there is a paucity of FDA-approved therapies. Currently, the only approved devices for treating resistant hypertension use renal denervation, however, this could soon change as clinical research progresses. With the evolution of interventional strategies for BP regulation, it is important to comprehend the developments to date in order to gauge directions for future research. The objective of this scoping review was to provide

Deep brain stimulation of nucleus basalis of meynert: Effect of stimulation mode and duration on learning in rat model of dementia.

Deep brain stimulation (DBS) of the nucleus basalis of Meynert (NBM) has been preliminarily investigated as a potential treatment for dementia. The degeneration of NBM cholinergic neurons is a pathological feature of many forms of dementia. Although NBM stimulation has been demonstrated to improve learning, the ideal parameters for NBM stimulation have not been elucidated. This study assesses the differential effects of varying stimulation patterns and duration on learning in a dementia rat model with cholinergic deficits. 192-IgG saporin (SAP) or Dulbecco's phosphate buffered saline was injec

Implementation of lead V8 as a simple non-invasive tool to improve patient selection in cardiac resynchronization therapy and guide left ventricular lead placement.

AIMS: Cardiac resynchronization therapy (CRT) improves outcomes in heart failure patients with electrical dyssynchrony. The QLV interval, a marker of delayed left ventricular (LV) activation, independently predicts CRT response. This study aimed to determine whether activation time measured in surface ECG lead V8 reflects posterolateral LV delay by correlating with the QLV interval during biventricular (BiV) CRT implantation. METHODS AND RESULTS: Consecutive patients with wide QRS (non-RBBB) undergoing BiV-CRT or generator change were prospectively enrolled. Surface ECGs were recorded with th

Percutaneous auricular neuromodulation to treat pain after ambulatory breast surgery: A randomized, double-masked, sham-controlled pilot study.

INTRODUCTION: Percutaneous auricular neuromodulation involves implanting electrodes around the ear and administering an electric current. A device is currently available that is cleared to treat symptoms from opioid withdrawal, and multiple reports suggest a possible postoperative analgesic effect. This randomized, controlled pilot study aimed to (1) assess the feasibility of a postoperative auricular neuromodulation protocol and (2) provide an estimate of its treatment effects on postoperative pain and opioid consumption. METHODS: Adults undergoing unilateral or bilateral ambulatory breast s

Thalamic Deep Brain Stimulation for Dystonic Head Tremor: A Long-Term Study of 18 Patients.

BACKGROUND: Dystonic head tremor is a particular manifestation of dystonia, which is difficult to treat. Although deep brain stimulation (DBS) of the globus pallidus internus has been established as a treatment for different phenotypes of dystonia, its role in dystonic tremor has been debated. Although thalamic targets have been used for treatment of dystonic tremor of the extremities, there is limited experience with dystonic head tremor. OBJECTIVES: Here, we present our experience with thalamic ventral intermediate (Vim) nucleus DBS in a consecutive series of patients providing long-term fo
