The sequence of the treatment of combined fractures of the pelvis, spine, and extremities in polytraumatized patients.

'Damage control' is the therapeutic strategy in the treatment of polytraumatized patients and aims at securing vital functions and controlling bleeding with a favorable effect on the post-traumatic immune response. The post-traumatic immune dysfunction is based on a disturbed balance between immunostimulatory and anti-inflammatory mechanisms. The extent of the immunological 'second hit' can be limited by delaying deferable surgical therapies until organ stabilization has been achieved by the treating surgeon. Pelvic sling is easy to apply and noninvasive with effective pelvic reduction. Pelvic

Origin and Composition of Nanoparticles Induced in a Complex Plastic-Brittle Transition Process of Xiaomei Shear Zone, Hainan Island, China.

Micro and nano structures of quartz schist, plagioclase granite, and granitic gneiss in the Xiaomei Shear Zone located in southeastern Hainan Island, China are observed using Scanning Electron Microscope (SEM). Ultramicroscopic analyses indicate that three types of nanoparticles were found in the samples, including spherical nanoparticles, agglomerated nanoparticles and nanofibers. These nanoparticles are mainly developed in the fracture zones. The more fractures there are, the more nanoparticles are developed. Energy dispersive spectrometer (EDS) and X-ray diffraction (XRD) are used to ascert

Arthroscopic Fixation of Osteochondral Fracture of Head of Femur. Case Study.

This article describes the case of an 18-year-old male after hip dislocation caused by snowboard injury, which resulted in osteochondral fracture of the head of femur with displacement of a loose body. CT as well as MRI confirmed the presence of a loose osteochondral fragment (20x14x4mm) fractured from the weight-bear-ing superoanterior part of head of femur (Pipkin type II). The surgical management undertaken was only arthroscopic, this representing a very rare case when com-par-ed to the literature. The article describes in detail the technical aspects of the procedure. At 4 months post-surg

Results after total hip arthroplasty with a large head and bipolar arthroplasty in patients with displaced femoral neck fractures.

The authors studied the short-term outcomes of total hip arthroplasty (THA) performed using large diameter femoral heads or bipolar arthroplasty (BA) in physiologically active elderly patients with displaced intracapsular femoral neck fractures. The THA group included 14 males and 66 females with a mean age of 75.5 years, and the BA group included 16 males and 73 females with a mean age of 77.6 years. Surgical procedures were performed by one surgeon using a modified Hardinge approach. Mean operation times were significantly longer in the THA groups. Pain, mobility, and walking ability scores

Second generation locked plating of proximal humerus fractures--a prospective multicentre observational study.

Surgical treatment of most displaced proximal humerus fractures is challenging due to osteoporosis. Locking plates are intended to provide superior mechanical stability. In a prospective multicentre study 131 patients were treated with second generation locked plating (NCB-PH, Zimmer, Inc.). The open procedure (n = 78) was performed using a deltopectoral approach; the minimally invasive technique (n = 53) involved percutaneous reduction and an anterolateral deltoid split approach. Clinical and radiological follow-up was obtained. Improvement in function (ROM) was statistically significant. Fra

Localized hand burns with or without concurrent blast injuries from fireworks.

This is a retrospective study of 32 cases with localized hand burns from fireworks. All cases occurred during two national festivals of our country. The majority (54%) were children between 5 and 14 years, and 94% were males. All patients had localized hand burns. Patients were divided into two groups. Group I (n=10) patients had isolated burn injuries and Group II (n=22) patients had other concurrent hand injuries from the 'blast' of the fireworks such as tendon avulsion, nerve injury, fracture, dislocations, and amputations. Primary management of concurrent injuries along with dressing to th

Treatment of condylar fractures in children and youths: the clinical value of the occlusal plane orientation and correlation with facial development (case reports).

The relative position of the plane of occlusion to the cranial base determines the direction of the forces generated in the cranium during occlusal function. When the plane of occlusion is level and when the neuromuscular system is in harmony, the vectors of forces created by the closing muscles are directed to the central area of the cranium in a symmetrically balanced way. Unfortunately, TMJ fractures may alter completely this balance with loss of the support to the mandible against the temporal component and loss of the functional effect of the lateral pterygoid muscle on the mandible. The use of a hook-plate in the management of acromioclavicular injuries. Report of ten cases.

A hook-plate is a clavicular small fragment AO plate with a hook engaging below the acromion. It is primarily used to secure the ligament repair in the treatment of displaced acromioclavicular joint dislocations. We have used the hook-plate in conjunction with a Weaver-Dunn procedure to secure the repair in seven patients. In another three we used this plate to reduce and stabilize distal clavicular fractures. Satisfactory results were obtained in all patients; the deformity disappeared, full pain-free shoulder movement was regained with no motor weakness, with a mean follow-up of 11 months (6

Subchondral contusion of the knee caused by axial loading from dashboard impact: detection by magnetic resonance imaging.

We studied the occurrence of "bone bruises" of the knee resulting from dashboard impaction and detected by magnetic resonance imaging (MRI). We chose 21 knees of 20 front seat occupants in head-on motor vehicle collisions. To ensure all knees had received a significant axial load, patients selected had ipsilateral posterior hip dislocations and/or posterior wall acetabular fractures. Anteroposterior and lateral knee radiographs were negative for fracture in all cases. T1-weighted axial and sagittal MRI of each knee with a 1.5-T magnetron scanner revealed signal changes consistent with subchond

Combined Kirschner wire fixation in the treatment of Colles fracture. A prospective, controlled trial.

For surgical treatment of the unstable Colles fracture we developed a new form of osteosynthesis, which consists in a modification of the dynamic Kirschner wire fixation described by Kapandji. It allows early motion without the typical risk of palmar dislocation noted with the Kapandji method. We prefer this method in elderly patients with reduced bone quality. The analysis of a collective of 110 fractures including a clinical and radiological follow-up examination of 72 patients revealed good anatomical reconstruction of the distal radius. The loss of motion was minimal on average and consist

Nerve root recovery in complete injuries of the cervical spine.

Thirty-six patients with complete quadriplegia were reviewed. Twenty-two underwent surgery, and 14 did not. There were 11 burst fractures and 3 extension fracture-dislocations, which were treated with anterior decompression and rigid plate fixation. There were 22 flexion injuries that were treated with posterior stabilization using Kirschner-wire tension band fixation, Harrington compression hooks, or Halifax laminar hooks. The non-operative group was treated with skeletal traction with skull tongs for 6-12 weeks followed by the application of a hard collar or halo vest for 3 months. Of the 22

Periprosthetic femoral fractures: more comminution and stem loosening with cemented implants-a retrospective cohort study.

BACKGROUND: Periprosthetic femur fractures (PFF) are a significant complication associated with Total Hip Arthroplasty (THA) and Hemiarthroplasty (HA). There remains a gap in the literature regarding a consensus on which method of stem fixation yields higher rates of periprosthetic fractures—cemented or cementless. The aim of this study is to analyze the rates of comminution, stem loosening and requirements for full femoral component revision in periprosthetic femoral fractures when comparing cemented implants to cementless implants. METHODS: A retrospective cohort study of all femoral PFFs be

Risk Factors and Preventive Strategies for Perioperative Distal Femoral Fracture in Patients Undergoing Total Knee Arthroplasty.

Background and Objectives Perioperative distal femoral fracture is rare in patients undergoing total knee arthroplasty (TKA). In such rare cases, additional fixation might be required, and recovery can be delayed. Several studies have focused on perioperative distal femoral fractures in TKA, but there remains a lack of information on risk factors. The purpose of this study was to investigate risk factors for perioperative distal femoral fractures in patients undergoing TKA and suggest preventive strategies. Materials and Methods: This retrospective study included a total of 5364 TKA cases in a

Preliminary report in treatment of proximal humeral fracture with closed reduction and DOS external fixation System: a multicentric study.

INTRODUCTION:: Proximal humerus fractures are the seventh most frequent fracture in adults, and the third in patients over 65 years old, 5.7% of whole diagnosed fractures. Most of these fractures can be treated conservatively and achieve good results. However, more and more frequently we are confronted with dislocated and multifragmentary fractures, and with elderly and high functional demanding patients. In patients with osteoporosis and poor general conditions external fixation can be performed as rapid and mininvasive procedure with good outcome and low complication rates. The authors inves

Screw elastic intramedullary nail for the management of adult forearm fractures.

BACKGROUND: The failure of the conventional nailing of both forearm bones or isolated fractures of radius and ulna pose a potential problem of nail migration and rotational instability, despite the best reduction. The purpose of this paper is to evaluate the results of screw elastic intramedullary nail for the treatment of adult diaphyseal fractures of both forearm bones, which effectively addresses the problems associated with the conventional nailing systems for the forearm fractures. Minimum Two-Year Outcomes of the Zimmer G7 Modular Dual Mobility Cup in Primary Total Hip Arthroplasty: Survivorship, Complications, Clinical and Radiographic Results.

Background/Objectives: Modular dual mobility (MDM) cups are constituted by a cobalt-chromium liner inserted into a standard acetabular shell, allowing for intraoperative indication and supplementary screw fixation of the acetabular component. MDM could face mechanical and biological issues, with the associated risk of elevated blood metal ions levels and adverse local tissue reactions. Methods: This is a monocentric retrospective study on a consecutive series of 105 patients who underwent primary unilateral THA with the G7 Dual Mobility Acetabular System cup (Zimmer Biomet, Warsaw, IN, USA) fr

Association between disease activity of rheumatoid arthritis and risk of complications following total hip arthroplasty: a retrospective cohort study.

BACKGROUND: Identifying rheumatoid arthritis patients at higher risk of complications after total hip arthroplasty could make perioperative management more effective. Here we examined whether disease activity is associated with risk of such complications. METHODS: We retrospectively analyzed data for 337 rheumatoid arthritis patients at our medical center who underwent primary total hip arthroplasty. Rheumatoid arthritis patients were categorized according to the simplified disease activity index (SDAI), the values of which at admission and follow-up were averaged together. Logistic regressio

Outcomes following primary total hip arthroplasty with pre-existing spinal fusion surgery.

AIMS: There is inconsistent evidence on whether prior spinal fusion surgery adversely impacts outcomes following total hip arthroplasty (THA). We conducted a systematic review and meta-analysis to assess the association between pre-existing spinal fusion surgery and the rate of complications following primary THA. METHODS: We searched MEDLINE, Embase, Web of Science, and Cochrane Library up to October 2019 for randomized controlled trials (RCTs) and observational studies comparing outcomes of dislocation, revision, or reasons for revision in patients following primary THA with or without pre-
