Impact of scheduled laparoscopic cholecystectomy in patients with acute cholecystitis, following percutaneous transhepatic gallbladder drainage.

BACKGROUNDS/AIMS: Frequently encountered in practice, the first-line treatment for acute cholecystitis is early or urgent cholecystectomy, with laparoscopic cholecystectomy (LC) being the preferred method. Percutaneous transhepatic gallbladder drainage (PTGBD) is considered as a safe alternative therapeutic option for resolving acute cholecystitis in surgically high-risk patients. We evaluated the surgical outcomes of acute cholecystitis, focusing on the differences between emergent LC without PTGBD, and scheduled LC following PTGBD. METHODS: Between March 2010 and December 2014, 294 patients

Optimal treatment strategy for acute cholecystitis based on predictive factors: Japan-Taiwan multicenter cohort study.

BACKGROUND: Although early laparoscopic cholecystectomy is widely performed for acute cholecystitis, the optimal timing of a cholecystectomy in clinically ill patients remains controversial. This study aims to determine the best practice for the patients presenting with acute cholecystitis focused on disease severity and comorbidities. METHODS: An international multicentric retrospective observational study was conducted over a 2-year period. Patients were divided into four groups: Group A: primary cholecystectomy; Group B: cholecystectomy after gallbladder drainage; Group C: gallbladder drai

Comparing how patients value and respond to information on risk given in three different forms during dental check-ups: the PREFER randomised controlled trial.

BACKGROUND: This study aims to compare patient preference for, and subsequent change in, oral health behaviour for three forms of risk information given at dental check-ups (verbal advice compared to verbal advice accompanied by a traffic light (TL) risk card; or compared to verbal advice with a quantitative light fluorescence (QLF) photograph of the patient's mouth). METHODS: A multi-centre, parallel-group, patient-randomised clinical trial was undertaken between August 2015 and September 2016. Computer-generated random numbers using block stratification allocated patients to three arms. The

Nursing and midwifery practice for maintenance of vascular access device patency. A cross-sectional survey.

BACKGROUND: Up to 85% of hospital in-patients will require some form of vascular access device to deliver essential fluids, drug therapy, nutrition and blood products, or facilitate sampling. The failure rate of these devices is unacceptably high, with 20-69% of peripheral intravenous catheters and 15-66% of central venous catheters failing due to occlusion, depending on the device, setting and population. A range of strategies have been developed to maintain device patency, including intermittent flushing. However, there is limited evidence informing flushing practice and little is known abou

Eliminating circulating donor passenger leukocytes during clinical ex vivo lung perfusion does not attenuate inflammation.

PURPOSE: During Ex Vivo Lung Perfusion (EVLP), it is standard practice to incorporate a leukocyte filter to remove donor passenger leukocytes and attenuate their inflammatory effects. The widely used leukocyte filter, LeukoGuard 6, seems to be ineffective in reducing circulating leukocytes during EVLP. Therefore, we performed contemporary leukocyte filtration using BioR 02 plus leukocyte filters to reduce these leukocytes and investigate whether inflammation could be reduced. METHODS: EVLP was performed for a minimum of 180 min with 15 bilateral donor lungs. Perfusate samples were taken from

Radiation exposure and fluoroscopy time in AVNRT ablation: role of 3D electroanatomical mapping.

BACKGROUND: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most frequent regular supraventricular tachycardia in adults. Catheter ablation of the slow pathway is considered first-line therapy, offering high success with low complication risk. Conventional fluoroscopy-guided ablation, however, exposes both patients and operators to ionizing radiation. Three-dimensional (3D) electroanatomical mapping systems have emerged as a promising alternative to reduce radiation while maintaining efficacy. METHODS: We retrospectively reviewed 170 patients who underwent AVNRT ablation between Ju

The beneficial effects of clazosentan on the perioperative management of patients with subarachnoid hemorrhage.

Objective clazosentan has been approved in Japan for the prevention of cerebral vasospasm in patients with aneurysmal subarachnoid hemorrhage (aSAH) and has contributed to the significant suppression of angiographic vasospasm in clinical practice. We herein report the beneficial effects of clazosentan on the perioperative management of patients with aSAH. Methods The study population comprised 102 consecutive patients with aSAH: 49 received conventional treatment (fasudil with triple-H therapy) before May 2022, and 53 were treated with the current protocol (clazosentan management) thereafter.

Diagnostic value of uric acid to high-density lipoprotein cholesterol ratio in abdominal aortic aneurysms.

BACKGROUND: Abdominal aortic aneurysm (AAA) is highly lethal upon onset of acute aortic diseases (AAD) or rupture. Dyslipidaemia and hyperuricaemia are important risk factors for the development of AAA and AAD as well as aortic disease-related death. The aim of this study was to explore whether uric acid (UA) to high-density lipoprotein cholesterol (HDL-C) ratio (UHR) can be used as an independent predictor of the presence of AAA or AAD. METHODS: Three hundred subjects, including 100 AAA patients (AAA group), 100 AAD patients (AAD group) and 100 controls (CON group), were recruited in this st

Effectiveness of a patient decision aid for patients with superficial basal cell carcinoma: A pre- and post-implementation study.

OBJECTIVE: This study evaluates whether using a patient decision aid (PDA) for patients with superficial basal-cell carcinoma (sBCC) results in a decreased decisional conflict level and increased knowledge. METHODS: In a prospective multicentre study, patient groups were included before and after implementation of a PDA. Decisional conflict levels were compared directly after making the treatment decision, measured once as the mean score on the decisional conflict scale (DCS). Higher scores correspond with higher conflict levels (0-100). Secondary outcomes were knowledge on treatment options,
