Long-term Outcome of Patients With Nonoperated Prosthetic Valve Infective Endocarditis: Is Relapse the Main Issue?

In nonoperated prosthetic valve endocarditis (PVE), long-term outcome is largely unknown. We report the follow-up of 129 nonoperated patients with PVE alive at discharge. At 1 year, the mortality rate was 24%; relapses and reinfection were rare (5% each). Enterococcal PVE was associated with a higher risk of relapse.

[New frontiers in aortic surgery].

In cardiac surgery, aortic diseases represent an important chapter, and include treatments from the aortic valve to the descending aorta. The infra-diaphragmatic abdominal aorta, generally and historically, is the domain of vascular surgery. Despite the excellent and consolidated results obtained in the treatment of thoracic aortic disease, surgical mortality and morbidity are still relevant, also due to the presence of older patients with more extensive and complex aortic disease. In the last decades, the better knowledge of the aortic issues and the availability of new grafts have resulted i

Surgical management of prosthetic valve obstruction with the Sorin tilting disc prosthesis.

BACKGROUND AND AIMS OF THE STUDY: Thrombotic occlusion is a potentially fatal complication of heart valve replacement surgery. The purpose of this report is to present our experience in the treatment of this group of patients, with emphasis on valve debridement as an effective surgical cure. MATERIAL AND METHODS: Of 299 operative survivors undergoing valve replacement with Sorin Carbocast tilting disc prostheses at our institution 270 could be followed up: 18 of them (6.7%) developed thrombosis within 26 months. All thrombotic blocks occurred in the mitral position. Anticoagulation was sub-th

The Monostrut Björk-Shiley valve. Seven years' experience.

The results of cardiac valve replacement with the Monostrut Björk-Shiley prosthesis (Shiley, Inc., Irvine, Calif.) during a 7-year period are presented. A total of 984 valves were implanted in 820 patients from May 1983 to April 1990. Aortic valve replacement was performed in 378 patients, mitral replacement in 294, and multiple replacement in 148. In addition, 180 patients (22%) underwent associated procedures. Mean age was 52.6 +/- 11 years. Operative (30 days) mortality was 5.9% (49 patients): 3.9% (15 patients) for aortic, 7.8% (23 patients) for mitral, and 7.4% (11 patients) for multiple

Surgical Management of Group A β-Hemolytic Streptococcal Aortic Root Abscess.

This case report presents a pediatric patient with an aortic root abscess caused by group A β-hemolytic Streptococcus. After aggressive debridement of infected tissue, the continuity between the left ventricular outflow tract and aorta was reconstructed with autologous pericardium, and the aortic valve was replaced with a mechanical prosthesis using the Konno procedure with concomitant mitral valve plasty. We believe that radical resection of the abscess is an effective way to eradicate such a life-threatening infection.

Mobilization of an anomalous retroaortic coronary artery to prevent myocardial ischemia after aortic valve replacement.

Abnormal distribution may put coronary arteries at risk of damage during valvular surgery, especially if it is not identified during a preoperative examination. In this case report, we describe an approach to an anomalous right coronary artery during aortic valve replacement.

Anticoagulation for coexisting bioprosthetic aortic valve thrombosis and anticoagulant-related bleeding: "A double edge sword".

The choice of anticoagulant agents for newly implanted bioprosthetic valve varies significantly, particularly in the presence of postoperative atrial fibrillation with increasing use of nonvitamin K oral anticoagulation (NOACs) in recent years. We reported a challenging case with a coexisting bioprosthetic aortic valve thrombosis and significant anticoagulant-related bleeding. Clinical management strategy and brief literature review were presented.

Flat fibrin thrombus deposition on tissue valve after aortic valve replacement.

Bioprosthetic valves have a lower thrombogenicity than mechanical valves, thus long-term systemic anticoagulation can be avoided. Herein, we present an extremely rare case of fibrin thrombus formation on tissue valve, which was observed during the second operation 2 years after the initial operation by chance. This report gives us a word of caution regarding the thrombus formation on the bioprosthetic valves that could be unaware of the presence for the long term.

[Cardiac manifestations in osteogenesis imperfecta. A case report and therapeutic implications].

Osteogenesis imperfecta is an hereditary defect in the synthesis of collagen fibres. The disease can cause cardiac defects, such as aortic and mitral incompetence. Surgical repair of these defects is complicated by tissue friability and bleeding problems. We describe a case where an incompetent aortic valve was successfully replaced by a mechanical prosthesis.

Evidence of impending embolization of a calcific cusp fragment from a mitral porcine xenograft.

The use of porcine bioprostheses has been associated with a low rate of embolic complications. The case here presented provides evidence that systemic emboli in porcine valve recipients may not necessarily be due to intracardiac or prosthetic thrombosis, but to detachment and migration of fragments of degenerated cusp tissue. Therefore, urgent assessment of xenograft function is recommended in patients with a porcine bioprosthesis who experience embolic episodes late postoperatively.

Prosthetic valve endocarditis caused by Histoplasma capsulatum.

Previously reported cases of Histoplasma capsulatum infection of prosthetic heart valves have been first diagnosed at autopsy. A patient had an H capsulatum infection that was diagnosed by serologic means 1 1/2 years before involvement of the patient's prosthetic mitral valve was found at surgery. Numerous previous attempts to demonstrate organisms by culture and histologic study of bone marrow and liver biopsy specimens were unsuccessful. Various serologic tests were used in the diagnosis and evaluation of treatment of H capsulatum endocarditis. The management of this case was compared with 2
