Functional MRI follow-up study of language processes in healthy subjects and during recovery in a case of aphasia.

BACKGROUND AND PURPOSE: The goal of this study was to develop a functional MRI (fMRI) paradigm robust and reproducible enough in healthy subjects to be adapted for a follow-up study aiming at evaluating the anatomical substratum of recovery in poststroke aphasia. METHODS: Ten right-handed subjects were studied longitudinally using fMRI (7 of them being scanned twice) and compared with a patient with conduction aphasia during the first year of stroke recovery. RESULTS: Controls exhibited reproducible activation patterns between subjects and between sessions during language tasks. In contrast,…

Atypical brainstem representation of onset and formant structure of speech sounds in children with language-based learning problems.

This study investigated how the human auditory brainstem represents constituent elements of speech sounds differently in children with language-based learning problems (LP, n = 9) compared to normal children (NL, n = 11), especially under stress of rapid stimulation. Children were chosen for this study based on performance on measures of reading and spelling and measures of syllable discrimination. In response to the onset of the speech sound /da/, wave V-V(n) of the auditory brainstem response (ABR) had a significantly shallower slope in LP children, suggesting longer duration and/or smaller …

[Aphasias for verbal and sign languages are due to lesions of nearly localised but not identical brain regions of the left hemisphere].

OBJECTIVES: To study the characteristics of verbal and sign language aphasia in a patient fluent in both languages, who had had a recent left hemisphere stroke as well as to localise the site responsible for Spanish sign language aphasia. PATIENT AND METHODS: 56 years old male, with risk factors for stroke, who presented an episode of sudden onset aphasia and right hemiplegia that partially recovered in a few hours. The residual deficit of language was explored with a detailed protocol that included comprehension, denomination, oral and phonetic praxis, propositional and automatic spontaneous…

Language lateralization in patients with temporal lobe epilepsy: a comparison of functional transcranial Doppler sonography and the Wada test.

This study prospectively investigates whether noninvasive functional transcranial Doppler sonography (fTCD) is a useful tool to determine hemispheric language lateralization in the presurgical evaluation of patients with medically intractable temporal lobe epilepsy (TLE). fTCD results were compared with the Wada test as the gold standard. Wada test and fTCD were performed in 13 patients suffering from TLE. fTCD continuously measured blood flow velocities in both middle cerebral arteries, while the patient was performing a cued word generation task. During the Wada test, spontaneous speech, com…

Abstraction on clinical data sequences: an object-oriented data model and a query language based on the event calculus.

In this work, we deal with temporal abstraction of clinical data. Abstractions are, for example, blood pressure state (e.g. normal, high, low) and trend (e.g. increasing, decreasing and stationary) over time intervals. The goal of our work is to provide clinicians with automatic tools to extract high-level, concise, important features of available collections of time-stamped clinical data. This capability is especially important when the available collections constantly increase in size, as in long-term clinical follow-up, leading to information overload. The approach we propose exploits the i

Schizophrenic language is conceived as a disorder in the implicitly dialogical structure of texts. This can be proven by these patients' inability to find communicative solutions for misunderstandings emerging during the interview.

Evaluation of memory and language function pre- and postthalamotomy with an attempt to define those patients at risk for postoperative dysfunction.

Memory and language dysfunction has been sporadically reported following stereotaxic thalamotomies. In order to determine which patients are at greatest risk and to better define the nature of this dysfunction, we have prospectively evaluated 18 patients undergoing stereotaxic thalamotomies for movement disorders (MDs). Patients were evaluated clinically, with computed tomography (CT) and with memory and language protocols (MLPs) pre- and postoperatively. Patients exhibiting postoperative deficits were again evaluated with the MLP on follow-up visits to the clinic. Significant changes in memor…

Context Matters: Assessing Sex and Diagnostic Differences in Autistic Children's Language Across Tasks With Differing Levels of Structure.

PURPOSE: Examine how task structure and participant sex influence social-word production in verbally fluent autistic and non-autistic youth … . CONCLUSION: Task structure impacts detection of diagnostic and sex-related differences in social language: less structured contexts reveal diagnostic group differences, while structured narrative elicitation attenuates them. Social word production in unstructured settings relates to clinician-rated social difficulties, particularly for autistic girls, h …

Low-cost fine-tuning of SmolVLA with smoothness regularization and chunk-aware auxiliary supervision for language-conditioned manipulation.

Vision-Language-Action (VLA) models offer a unified interface for language-conditioned manipulation. This paper reports a negative result: a low-cost post-training recipe for SmolVLA with LoRA, a temporal smoothness regularizer, and chunk-aware auxiliary supervision from action-variation pseudo labels does not improve manipulation success under a 30,000-step budget (approximately 1/67 of the original 2,000,000-step SmolVLA recipe). Because the auxiliary classifier is used only during training and the runtime chunk length remains fixed at k = 8, we denote the configuration SmolVLA+LoRA+S&C (smo…

Individual differences in motor and auditory activation in second language speech learning: Roles of domain-general auditory processing.

While existing models emphasize the critical roles of auditory and motor systems in speech perception, it remains unclear how individual learners differentially rely on these systems and how their relative weighting relates to language learning. This study investigates the neural mechanisms through which individual differences in domain-general auditory processing influence reliance on these networks during second language (L2) phonetic learning. Focusing on the acquisition of English [æ] and [ʌ] by 207 Japanese learners, we examined participants with varying auditory precision profiles using …

Venlafaxine modulates white matter integrity in dorsal and ventral language pathways after subcortical stroke: a longitudinal TBSS study.

BACKGROUND: Subcortical aphasia results from disrupted cortico-subcortical language networks after left-hemispheric infarction. Venlafaxine, an SNRI, may accelerate language recovery, but its effects on white matter plasticity remain unclear. We used diffusion tensor imaging (DTI) and tract-based spatial statistics (TBSS) to assess early venlafaxine effects on dorsal and ventral language pathways in subacute stroke. METHODS: Thirty-two patients with first-ever left subcortical infarction and mild-to-moderate aphasia were randomized to venlafaxine (n=16; 75 mg/day, day 7-week 4) or control (n=…

How you read the text matters: Caregiver oral reading prosody during shared reading relates to preschoolers' language skills.

Acoustic features of child directed speech (CDS) in caregiver-child conversational interactions are known to capture young children's attention to language input and facilitate language learning. Emerging evidence suggests that caregivers' use of CDS-like features when reading to their child, known as caregiver oral reading prosody, plays an important role in supporting preschoolers' storybook comprehension. Do associations between caregiver oral reading prosody and preschoolers' language extend beyond real-time listening comprehension to broader language skills? Forty-one caregivers read a ch…
