Focal lesions area feature of chronic inflammatory demyelinating polyneuropathy (CIDP).

In a study designed to identify the neuropathological features typical of chronic inflammatory demyelinating polyneuropathy (CIDP), we reviewed the sural nerve biopsy findings in 105 patients with this disorder. The patients' mean age at biopsy was 49 years. In 65% of patients the disease had a progressive and in 35% a relapsing-remitting course. In 47% of cases the disorder was idiopathic; the remainder had various concurrent conditions. All sural nerve biopsy specimens showed varying amounts of active demyelination associated with onion bulbs (48% of cases), endoneurial edema (55%) and infla

New method to measure central motor conduction time using transcranial magnetic stimulation and T-response.

Measuring central motor conduction time (CMCT) is one of the useful methods to detect an impaired level of the spinal segment in cervical myelopathy patients. We modified a new technique to calculate the CMCT using tendon reflex latency (T-response) and investigated its accuracy. Motor-evoked potentials (MEPs) following transcranial stimulation were recorded in 19 patients with cervical myelopathy caused by a single level of spinal cord compression. CMCT was measured by subtracting the peripheral conduction time, which was calculated by using the T-response for the biceps brachii muscle (Bicep

Anti-Hu-associated paraneoplastic sensory neuropathy with peripheral nerve demyelination and microvasculitis.

A patient had a paraneoplastic autonomic, then sensory, then sensory-motor neuropathy with small cell lung carcinoma and a high titer Anti-Hu antibody to nuclei of neuronal cells. As an unusual finding there was electrophysiological and pathological evidence of demyelination and a peripheral nerve microvasculitis. The relationship of microvasculitis and peripheral nerve demyelination is discussed and their occurrence with paraneoplastic anti-Hu-associated sensory neuropathy is suggested not to be by chance.

[Quantitative determination of the function of thin nerve fibers].

Quantitative assessment of thermal and pain sensitivity (Marstock method on a SOMEDIC Thermotest, Somedic AB Stockholm, Schweden) was made and the function of autonomic nervous system (sympathetic skin response and R-R interval variation) indicating the function of small nerve fibres (A-delta and C) was determined in 44 patients with symptoms of the presumed sensory neuropathy. The function of the large nerve fibres was evaluated by the classic nerve conduction study. The methods of small nerve fibres evaluation exhibited greater sensitivity as the classic nerve conduction study. The dysfuncti

Correlation between the histopathologic, genotypic, and phenotypic features of hereditary peripheral neuropathies in childhood.

In recent years, there have been remarkable advances in the understanding of the molecular genetic basis of the hereditary polyneuropathies. Linkage of the genes for Charcot-Marie-Tooth disease to chromosomes 1 and then 17 was followed by the discovery that the commonest form of Charcot-Marie-Tooth disease (CMT1A) was due to a duplication of DNA at 17p11.2-12. This duplication was shown to contain the gene for peripheral myelin protein PMP22. The finding that mutations of the myelin protein PMP22 gene were present in some Charcot-Marie-Tooth disease cases lacking the duplication confirmed the

Carpal tunnel syndrome in 100 patients: sensitivity, specificity of multi-neurophysiological procedures and estimation of axonal loss of motor, sensory and sympathetic median nerve fibers.

This prospective study meets all six criteria recently recommended by a quality assurance committee of the AAEM and defines criteria of abnormality, sensitivity and specificity of 19 sensorimotor and sympathetic parameters in 100 patients who were suspected on clinical grounds of having carpal tunnel syndrome (CTS), and in 70 control subjects. Nine parameters reached a specificity of 97%, permitting the electrodiagnosis of CTS in 87% of the patients studied. The results in this study confirm that median sensory nerve conduction studies are more frequently abnormal than are studies of motor ner

Mitochondrial dysfunction in Rett syndrome. An ultrastructural and biochemical study.

We report the ultrastructural and biochemical alterations of muscle mitochondria in two girls with Rett syndrome. Our findings suggest the presence of an energy metabolism impairment the primary or secondary role of which in the pathogenesis of this syndrome cannot be defined at present.

The effect of injury on the properties of afferent fibres in the lingual nerve.

The receptor properties of mechanosensitive afferent fibres in the lingual nerve have been studied using electrophysiological techniques in cats. In normal animals some fibres responded only to mechanical stimulation of filiform or fungiform papillae but others also responded when a cold stimulus was applied to the receptive field. Twelve weeks after crushing the lingual nerve, the regenerated fibres had slower conduction velocities but the receptor properties were not significantly different from normal. Twelve weeks after sectioning the lingual nerve, there was a greater reduction in conduct

Evaluation of proximal facial nerve conduction by transcranial magnetic stimulation.

A magnetic stimulator was used for direct transcutaneous stimulation of the intracranial portion of the facial nerve in 15 normal subjects and in patients with Bell's palsy, demyelinating neuropathy, traumatic facial palsy and pontine glioma. Compound muscle action potentials (CMAPs) thus elicited in the orbicularis oris muscle of controls were of similar amplitude but longer latency (1.3 SD 0.15 ms) compared with CMAPs produced by conventional electrical stimulation at the stylomastoid foramen. No response to magnetic stimulation could be recorded from the affected side in 15 of 16 patients w

Regeneration and repair of myelinated fibers in sural-nerve biopsy specimens from patients with diabetic neuropathy treated with sorbinil.

There is reason to believe that diabetic neuropathy may be related to the accumulation of sorbitol in nerve tissue through an aldose reductase pathway from glucose. Short-term treatment with aldose reductase inhibitors improves nerve conduction in subjects with diabetes, but the effects of long-term treatment on the neuropathologic changes of diabetic neuropathy are unknown. To determine whether more prolonged aldose reductase inhibition reverses the underlying lesions that accompany symptomatic diabetic peripheral polyneuropathy, we performed a randomized, placebo-controlled, double-blind tri

Wernicke-Korsakoff-like syndrome in patients with colorectal carcinoma treated with high-dose doxifluridine (5'-dFUrd).

5'-dFUrd (doxifluridine) is a new fluoropyrimidine that may have a higher antitumor effect and less side effects than 5-FU. In this study 15 patients with advanced colorectal carcinoma, one with renal carcinoma, one with carcinoid and one with advanced carcinoma of unknown origin, were randomly assigned to receive 5'-dFUrd 5 g/m2 or 3 g/m2 as a 1-h infusion for 5 days in 3 cycles at intervals of 4 weeks. The patients had repeated neurological and neurophysiological examinations before and during the treatment. Ten patients developed symptoms of toxicity of the central nervous system (CNS), wit

[Hereditary motor and sensory neuropathy. I. Principles of classification and clinical picture].

The clinical picture was analysed in two types of hereditary motor-sensory neuropathy isolated on the ground of electrophysiological criteria. Type I comprised 34 patients with the conduction velocity in median nerve below 38 m/sec. Type II 19 patients with the conduction velocity above 38 m/sec. The age of onset was similar in both types and cases with onset below the age of 5 years prevailed. The assessment of the clinical picture using a acoring system failed to show any significant differences between type I and type II. Cases of type I shows, however, a considerable variability of the cli

[Motor, sensory electroneurographic and electromyographic results as well as somatosensory evoked potentials in comparison to clinical findings following nerve suture].

A group of 37 patients (total 41 nerves) with a traumatic transection of median or ulnar nerves at the wrist were reinvestigated clinically and electrophysiologically 4-59 months after primary or secondary suture or grafting. Clinically there was no relation between the time after the operation, and sensory recovery determined according to the schedule of Nicholson and Seddon (1957), two-point discrimination, vibration threshold. There was also no relationship between the time after suture, and the motor latencies as well as amplitudes of evoked muscle action potentials from the abductor polli

Rett syndrome--clinical studies and pathophysiological consideration.

Eleven female patients with Rett syndrome were evaluated for detecting the pathogenesis. Clinical symptoms were characterized by their orderly sequence of occurrence of particular symptoms at particular ages starting from early infancy, and their progression. Increment of head circumference tapered from late infancy, resulting in microcephalus which corresponded with the clinical features. Surface EMG revealed the tremulous rhythmic discharge underlying the characteristic stereotyped movement of the hands. Serial polysomnographical examinations showed abnormalities of the tonic and phasic comp

Clinical and genetic variations in the syndrome of adult GM2 gangliosidosis resulting from hexosaminidase A deficiency.

Six patients from three families developed adult GM2 gangliosidosis resulting from severe beta-hexosaminidase A deficiency. The clinical picture varied between and within families and included spinocerebellar, various motor neuron, and cerebellar connection syndromes. Psychosis appeared in each family. Involvement of three generations was recorded in one family. The phenotype of adult GM2 gangliosidosis is variable and cannot form a basis for genetic classification. Detailed hexosaminidase determinations in eight patients from five families revealed all patients to have minor quantities of hex

Early auditory evoked potentials: developmental aspects and validity in neuropaediatric and audiologic disorders.

Auditory evoked potentials and in this context especially five waves in the first 10 ms (early auditory evoked potentials = EAEP) are a diagnostic aid in topodiagnosis of posterior fossa diseases. This is due to waves I to V which arise along the acoustic nerve and in brain stem structures such as medulla, pons and mid-brain. Besides an indication about the site of a lesion in the posterior fossa, wave V allows an objective threshold determination. The present results were gained in normal children aged 1-3 years and in children with neuropaediatric and audiologic disorders.

Somatosensory evoked responses in the diagnosis of thoracic outlet syndrome.

A study was made of 11 patients with cervical rib, and one patient with Klippel-Fiel syndrome and enlarged transverse processes to determine whether evoked potentials recorded from both Erb's points and the cervical spine in response to median and ulnar nerve stimulation provided information additional to that obtained by EMG and peripheral conduction studies. It was found that in seven patients who had pain and paraesthesias but no objective neurological signs both the peripheral and central conduction studies were within normal limits. By contrast, of five patients who had objective signs, c

Neurophysiological approaches to chronic pain following spinal cord injury.

Pain occurring in patients with spinal cord injury can be classified on clinical grounds into five types: peripheral, central, visceral, mechanical and psychic. An attempt has been made to correlate each type of pain with present neurophysiological knowledge. Mechanisms as to how unpleasant sensations reach the conscious level can be deduced when clinical and neurophysiological data are pooled. Eight case histories are presented which typify each class. The authors' evaluation and treatment offered is presented for each type.

Neurophysiological studies in adrenomyeloneuropathy. A report on five cases.

Neurophysiological studies were performed in 5 patients in two families suffering from adrenomyeloneuropathy (AMN). The diagnosis was supported by electron microscopy of nerve twigs in all cases and by the demonstration in 2 cases of increased levels of saturated very long chain fatty acids in cultured fibroblasts (Moser, personal communication). Measurements of the sensory-motor conduction velocities demonstrated the variability of the peripheral nerve damage in AMN, further confirmed by quantitative studies of sensory nerve biopsies. Somatosensory evoked potentials (SEP) were abnormally dela
