Open Versus Arthroscopic Reduction for Perilunate and Lunate Dislocations: A Systematic Review and Network Meta-Analysis.

PURPOSE: Perilunate and lunate dislocations represent complex wrist injuries requiring urgent reduction and stabilization. While open reduction has been the gold standard, arthroscopic techniques have emerged as less invasive alternatives. However, comparative evidence remains limited, and the management strategies are controversial. METHODS: We conducted a systematic review and network meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Dedicated searches of major databases were performed up to June 16, 2025. Studies comparing open and

Triceps-Sparing Total Elbow Arthroplasty for Distal Humerus Fracture: The Lateral Paraolecranon Approach.

Distal humerus fractures in the elderly are a difficult problem to treat. Open reduction internal fixation has a high rate of complications, particularly because osteopenia compromises what is already tenuous fixation in a metaphyseal fracture. Total elbow arthroplasty is a more predictable outcome and easier recovery for these patients. However, most surgeons perform a low volume of total elbow arthroplasty. In addition, traditional exposure requires detachment of the triceps tendon. The lateral paraolecranon approach maintains the central tendon attachment to the olecranon while still facili

Injectable bone cement augmentation for the treatment of distal radius fractures: a review.

Fractures of the distal radius often occur as a result of low-energy trauma and are frequently seen in osteoporotic patients. Many biomechanical studies and clinical case series have been carried out to investigate the effects on the fractured bones of different fixation methods such as cast immobilization, percutaneous pins, external fixators, or open reduction with internal fixation. In addition, the use of different bone cements as stand-alone solutions or as an adjunct to the aforementioned fixation methods is described as one possible way of increasing the performance of the fixation by r

Nonunion of femoral neck fracture and trochanteric osteotomy after a pinned, slipped capital femoral epiphysis: a case report.

Femoral neck fracture as a complication of slipped capital femoral epiphysis (SCFE) is rare. Even rarer is a femoral neck nonunion as an additional complication. This is the first case reported in the literature of a failed valgus osteotomy for a femoral neck nonunion. A salvage operation involving a step-cut valgus/flexion/internal rotation osteotomy, open reduction and internal fixation, with a blade plate and cannulated screw, placement of an allograft femoral strut, and allograft bone grafting was successfully performed. Femoral neck fractures following SCFE fixation are more difficult to

[Psoas compartment block for surgery of the femoral neck (trochanteric) fracture in a patient with severe heart failure due to rheumatoid myocarditis].

A 72-year-old female with severe heart failure due to rheumatoid myocarditis underwent open reduction of the left femoral neck (trochanteric) fracture. We performed psoas compartment block (PCB) at L3/4 level in the lateral position with the fractured side up, using a 22 G Tuohy needle to inject 10 ml of normal saline and 20 ml of 2% mepivacaine. Anesthesia was maintained with propofol (1-2 mg.kg-1.hr-1) under spontaneous breathing with oxygen (2 l.min-1) through mask. The patient did favorably during and after the operation. We conclude that PCB is useful for surgery of the lower extremity in

Humerus shaft fractures, approaches and management.

Humeral shaft fracture is a common injury which can be treated either conservatively with functional bracing or with surgical fixation. Current evidence shows an increase in the rate of nonunion after conservative treatment, suggesting that indications for conservative treatment may need to be re-examined. This article updates trends in treatment for humeral shaft fracture. Indications for surgery, both for plate osteosynthesis with open reduction and internal fixation (ORIF) as well as for minimally invasive plate osteosynthesis (MIPO) and intramedullary nailing (IMN) are described. Recogniti

An ITP Patient Taking Eltrombopag Presented with Extreme Thrombocytosis after Orthopedic Surgery.

BACKGROUND: Primary immune thrombocytopenia (ITP) is an acquired autoimmune disease characterized by varying degrees of platelet count reduction, with or without skin and mucosal bleeding. Eltrombopag is a thrombopoietin receptor agonist (TPO-RA) that promotes platelet production by specifically binding to the transmembrane domain of thrombopoietin receptor (TPO-R). METHODS: Blood routine examination was performed to monitor the changes of platelets in this ITP patient during the treatment of eltrombopag. RESULTS: The ITP patient had an extreme thrombocytosis after open reduction and interna

Bipolar dislocation of the forearm (floating forearm).

Bipolar dislocation of the forearm (floating forearm) is an unusual injury and is therefore often overlooked. We report a 28-year-old male patient who presented at another center with a history of a fall while climbing a tree. The patient's left elbow was treated with closed reduction and immobilization with a long-arm cast brace due to elbow dislocation. However, the patient was admitted with pain and swelling of the wrist to our emergency department the following day. Physical and radiological examination revealed dorsal trans-scaphoid perilunate dislocation. A dorsal incision was performed

Management of a rare complication after screw fixation of a pediatric tibial spine avulsion fracture: a case report with follow-up to skeletal maturity.

Avulsion of the tibial spine is functionally equivalent to rupture of the anterior cruciate ligament in an adolescent athlete. It therefore presents to general orthopaedists as well as a wide variety of orthopaedic subspecialty surgeons, including traumatology sports medicine, and pediatrics. Restoration of normal knee kinematics is dependent on anatomic reduction and fixation of the avulsed fragment. Because this injury is typically sustained by the skeletally immature patient, epiphyseal fixation is ideal to avoid physeal injury, which can lead to angular limb deformity. We present a case, t

Geriatric Acetabular Fractures and Femoral Head Protrusio: Methods of Fixation and the Option of Total Hip Arthroplasty.

Currently, the literature is unclear regarding the optimal treatment algorithm for geriatric acetabular fractures. In a recent epidemiological study, 70% of all acetabular fractures in patients older than 65 years were classified as either associated both column or anterior column/posterior hemitransverse. Within the subset of these fractures, variants with significant femoral head protrusio, which is defined as the displacement of the femoral head medial to the ilioischial line, present with unique challenges. Goals of treatment in these cases should include surgical techniques that minimize

Outcome of intramedullary fixation of clavicular fractures.

We conducted a retrospective, single-center (tertiary referral center with associated level I trauma center) review to evaluate the outcome of open reduction and internal fixation (ORIF) with intramedullary (IM) clavicle pin of displaced clavicular fractures. Sixty-eight displaced midshaft clavicle fractures in 68 patients underwent ORIF with IM clavicle pins. Patients were identified through a perioperative database by searching for Current Procedural Terminology (CPT) codes. Union was the primary outcome. Secondary outcomes included time to union, pain, incidence of nonunion and delayed unio

Displaced comminuted midshaft clavicle fractures: use of Mennen plate fixation system.

BACKGROUND: Surgical treatment of displaced comminuted midclavicular fractures includes open reduction and stabilization with wires, pins, or screwed plates. However, it is often difficult stabilizing small fragments, and there are several complications. The aim of this study was to assess an alternative technique using a Mennen plate. METHODS: From 1999 to 2004, we treated a cohort of 43 patients with a displaced three- or four-part midclavicular fracture. All patients were followed up at an average of 1 year after the injury: they had a functional evaluation and were assessed radiographical

The treatment of isolated ulnar fractures in adults: a systematic review.

The treatment of isolated ulnar fractures remains controversial, with different authors recommending both surgical and non-surgical management. We undertook a systematic review of the current literature in order to assess whether any conclusions can be drawn. A total of 33 series involving 1876 patients were identified as suitable for analysis. Data from these papers were combined by method of treatment. The results of the non-operative treatment of minimally displaced ulnar fractures with a stable configuration were uniformly good. Below elbow plaster cast, functional brace and early mobilisa

Supracondylar fractures of the femur: results of treatment of 61 patients.

A retrospective study of 61 fractures (60 patients) involving the supracondylar and intercondylar areas of the femur was made. All patients were younger than 60 years and sustained their injuries in motor vehicle accidents. Follow-up was from 1 to 9 years (mean 30 months). Clinical results of operative and nonoperative treatment were compared with respect to knee motion, pain and function. Extra-articular (type A) fractures had superior clinical ratings if treated operatively (45% excellent and 30% unsatisfactory). Intra-articular (type B) fractures had a substantial number of unsatisfactory r

Health related quality of life in patients with supination-external rotation stage IV ankle fractures.

BACKGROUND: While open reduction of displaced ankle fractures generally is accepted as the standard of care, relatively little is known about the health related quality of life after treatment. It is generally accepted that clinical results of treatment for supination-external rotation stage IV ankle fractures are favorable. The goal of this investigation was to determine the relationship between clinical results and health-related quality of life outcome measures in a consecutive series of patients treated for closed supination-external rotation stage IV ankle fractures. METHODS: Twenty-six

A Rare Clinical Dilemma: Humeral Shaft Fracture in a Full-Term Pregnant Patient - A Case Report with Review of Literature.

INTRODUCTION: Orthopedic trauma complicates 6-7% of pregnancies and remains a leading cause of non-obstetric maternal and fetal morbidity. While musculoskeletal injuries during pregnancy are not uncommon, fractures requiring surgical intervention in the third trimester are rare and present unique management challenges. Among these, humeral shaft fractures are especially rare, and their management must carefully balance maternal recovery with fetal well-being. CASE REPORT: We report the case of a 30-year-old primigravida at 38 + 4 weeks of gestation who sustained a left diaphyseal humerus frac
