Orbital cellulitis presenting as a first sign of incomplete kawasaki disease.

A 6-year-old boy was referred to our hospital with orbital cellulitis. He had a history of 7 days of fever despite antibiotherapy. At first, he only had pharyngitis and conjunctivitis, but then an orbital mass evolved which restricted the movement of his right eye and there was also periorbital inflammation resembling orbital cellulitis. Examination at presentation revealed conjunctivitis with secretion, periocular inflammation and edema, right-preauricular lymphadenopathy and restriction of upgaze in the right eye. Laboratory findings included a white blood cell count of 19,000 cells per mm3,…

Orbital follicular hyperplasia in common variable immune deficiency syndrome.

Common variable immune deficiency (CVID) is characterized by reduced serum immune globulins and impaired or absent antibody responses. Patients become more susceptible to infections and to lymphoproliferation and granulomatous inflammation. Ophthalmic manifestations of CVID are rare. The authors describe a case of orbital follicular hyperplasia in a 15-year-old girl with CVID syndrome causing proptosis and exposure keratopathy.

A novel technique for ventral orbital stabilization: the masseter muscle flap.

Loss of the caudal maxilla and ventral orbit after tumor resections can have negative functional and esthetic influences on the eye involved. This article reports on a case of a caudal maxillary acanthomatous ameloblastoma involving the ventral orbit that was resected and stabilized with a masseter muscle flap. The masseter muscle flap was generated from the superficial belly of the masseter muscle in order to close a defect in the orbital rim, created by a caudal maxillectomy. None of the published complications such as enophthalmos, excessive lacrimation, globe deviation, or strabismus were …

Rhino-orbital Mucormycosis Treated Successfully with Posaconazole without Exenteration.

Mucormycosis is a rare and often fatal opportunistic angioinvasive infection seen mostly in immunocompromised patients, such as those with diabetes mellitus, cancer, or renal failure. Ophthalmic manifestations of orbital mucormycosis include ocular pain, periocular oedema, visual loss, ophthalmoplegia, proptosis, and ptosis. Although therapy for orbital mucormycosis consists of maximally tolerated doses of antifungal agents (e.g., amphotericin B) and extensive surgical debridement, treatment remains ineffective in up to 20% of cases. We describe two patients with rhino-orbitalmucormycosis who …

Complications of orbital floor repair with silastic sheet: the skin fistula.

Treatment of orbital floor fracture is a subject of great interest in maxillofacial surgery. Many materials have been described for its reconstruction. In this article, the authors report a case of a patient who, 7 years from a previous orbital floor fracture and treatment with silastic sheet, presented herself to their clinic for the failure of the material used for its reconstruction and a skin fistula.Orbital floor repair with silastic sheet is an old method that no one uses anymore, but we still observe cases of late complications with this material. So a fine knowledge of silastic sheet co…

Orbital trauma with a large plant foreign body: a case report.

We report a case of a large plant foreign body in the orbit. A 30-year-old male presented to our hospital 6 h after a penetrating orbital injury in his left superior eyelid when he skidded from his motorcycle while riding. When the patient reported to us, he already had an inability to open his left eye and a continuous pain in the left orbit. On examination, there was a large plant foreign body penetrating the nasal orbit through the left upper eyelid. Orbital CT revealed a low density linear foreign body measuring 3.6 x 0.5 cm in the left orbit. An emergency operation was performed to remove…

[Pediatric orbital cellulitis without sinusitis: report of four cases].

INTRODUCTION: Pediatric orbital cellulitis is most often caused by ethmoid sinusitis. We present a description of 4 atypical cases of orbital cellulitis without sinusitis. PATIENTS AND METHODS: A 4-day-old girl presented with medical canthal swelling and preseptal cellulitis caused by bilateral congenital dacryocystoceles. The second patient was an 8-year-old boy seen for infectious conjunctivitis complicated by preseptal cellulitis without sinusitis. Conjunctival cultures revealed Neisseria gonorrheae. The next patient, a 5-month-old boy, presented with lid swelling, fever, proptosis and epi…

Late orbital floor implant migration presenting as recurrent episcleritis.

A 53-year-old patient presented with recurrent epicleritis of the right eye for 6 months and progressive diplopia. He had a past history of orbital floor repair over 30 years. A CT scan showed a radiodense migrated orbital floor sheet implant in the maxillary sinus causing obstruction of the maxillary sinus ostium, secondary maxillary sinusitis with retrobulbar involvement. The silastic orbital implant was removed via a Caldwell Luc approach and resolution of the patient's symptoms and signs quickly ensued. We discuss this late complication of orbital floor fracture repair presenting initially…

Inflammatory mass formation secondary to hydroxyapatite orbital implant leakage.

A 28-year-old woman with a history of an hydroxyapatite implantation performed 2 years earlier, presented with complaints of a poorly fitting prosthesis, and fullness of the left upper eyelid accompanied by eyelid erythema. Computed tomography scan showed a mass adjacent to the implant medially. Surgical removal of the implant and mass was performed. Histological examination of the implant revealed degradation on the medial side of the implant while the mass consisted of fibrous tissue with chronic inflammation and lymphoid follicle formation around a foreign material.

Case report of orbital cellulitis and necrotizing fasciitis from melioidosis.

A 48-year-old Thai male farmer presented with progressive swelling of the right eyelid and high-grade fever. His visual acuity declined and the right side of his forehead developed a necrotic skin lesion with a purulent discharge. CT of the orbits suggested orbital cellulitis with subperiosteal abscess. Intravenous ceftriaxone and clindamycin were given empirically but then switched to vancomycin and meropenem because of rapid deterioration together with clinical sepsis. Burkholderia pseudomallei was isolated from the blood 3 days after the treatment, and the antibiotics were then switched to …

Blowout fracture of the orbital floor secondary to vigorous nose blowing.

Orbital floor fracture due to vigorous nose blowing in the absence of mechanical trauma is rare, only four cases having previously been reported. In each of these cases, predisposing factors have been identified; preceding URTI in three and a history of sino-nasal surgery in the fourth case. We present the case of a 49-year-old woman who developed a maxillary sinus fracture and orbital emphysema after blowing her nose.

Evaluation of orbital disorders and cranial nerve innervation of the extraocular muscles.

A wide range of orbital disorders, including an orbital mass, infection, inflammation, systemic disease, or intracranial lesions, may be encountered with imaging. Evaluation of orbital disorders requires the combination of accurate and relevant clinical information with an understanding of anatomy and pathologic processes. An imaging approach to an orbital differential diagnosis includes assessment for alteration of a normal orbital structure, a lesion that does not belong in the orbit, or alteration of the orbit from bone or periorbital disorders. This approach, combined with key elements of …
