Longitudinal omics data and preclinical treatment suggest the proteasome inhibitor carfilzomib as therapy for ibrutinib-resistant CLL.

Chronic lymphocytic leukemia is a malignant lymphoproliferative disorder for which primary or acquired drug resistance represents a major challenge. To investigate the underlying molecular mechanisms, we generate a mouse model of ibrutinib resistance, in which, after initial treatment response, relapse under therapy occurrs with an aggressive outgrowth of malignant cells, resembling observations in patients. A comparative analysis of exome, transcriptome and proteome of sorted leukemic murine cells during treatment and after relapse suggests alterations in the proteasome activity as a driver o

Outcomes following exposure to drug interactions with ibrutinib in patients with chronic lymphocytic leukaemia.

Ibrutinib is metabolized by cytochrome P450 3A (CYP3A) and poses challenges with drug interactions. We conducted a population-based cohort study of Ontario residents aged ≥66 years who initiated ibrutinib for chronic lymphocytic leukaemia (CLL) to evaluate the frequency of potential drug interactions involving moderate/strong CYP3A inhibitors and inducers and their association with overall survival (OS). Secondary analyses employed a nested case-control design examining hospitalizations for haemorrhage or infection as potential markers of ibrutinib toxicity. Among 642 ibrutinib recipients (med

Real-World Treatment Patterns and Outcomes of Zanubrutinib in Chronic Lymphocytic Leukemia and Small Lymphocytic Leukemia (CLL/SLL).

Zanubrutinib is a next-generation selective Bruton's tyrosine kinase inhibitor (BTKi) with superior efficacy and lower cardiotoxicity rates seen in clinical trials over first-generation ibrutinib in CLL/SLL patients. We present real-world treatment patterns based on a practice change from ibrutinib to zanubrutinib in CLL/SLL patients in an integrated community oncology practice. We retrospectively analyzed adult patients who filled at least 3 months of zanubrutinib at Kaiser Permanente Northern California. Treatment patterns, adverse events (AE), and mortality were reported. Two hundred eighty

Impact of availability of a highly effective cystic fibrosis treatment (elexacaftor/tezacaftor/ivacaftor) on lung transplant waitlist and lung transplantation trends in the US.

Cystic fibrosis (CF) is a genetic disease that often leads to progressive lung disease and lung transplantation. CF transmembrane conductance regulator modulators (CFTRm) improve lung function in people with CF. The US Scientific Registry of Transplant Recipients (SRTR) data were used to assess rates of lung transplant waitlisting, waitlist removal, pre-transplant mortality, and lung transplantation in people with CF (CF cohort) compared to those with other respiratory conditions (non-CF cohort) across three time periods: (i) prior to approval of any CFTRm (Pre-CFTRm era); (ii) from approval o

Pharmacokinetic assessment of elexacaftor/tezacaftor/ivacaftor and their metabolites in maternal blood, cord blood, the neonate, and breastmilk of a cystic fibrosis carrier mother/affected fetus dyad.

There are a paucity of data regarding the pharmacokinetics (PK) of elexacaftor (ELX)/tezacaftor(TEZ)/ivacaftor(IVA)(ETI) in pregnant and/or lactating mothers and their offspring. We conducted a PK assessment of ETI and their metabolites in maternal/neonatal/cord blood and breast milk from a cystic fibrosis (CF) carrier mother/affected fetus dyad, collecting specimens at delivery, 1 and 4 weeks after birth. The infant received on-label direct dosing after 1 month of life; all PK samples were collected prior to initiation of neonatal dosing and analyzed using LC-MS/MS. Measured metabolites were

Impact of Elexacaftor/Tezacaftor/Ivacaftor on Microbiology and Antibiotic Utilization in People With Cystic Fibrosis.

BACKGROUND: Cystic fibrosis (CF) is a multisystem disease characterized by persistent lung infection. Treatment with elexacaftor/tezacaftor/ivacaftor (ETI) improves respiratory-related quality of life and reduces rates of infection and antibiotic treatments. Reduced antibiotic use may alter bacterial drug resistance patterns. METHODS: This was a single center, retrospective, observational study analyzing respiratory cultures obtained from people with CF (pwCF) before and after starting ETI therapy. Antibiotic courses and culture data, including susceptibilities, were obtained from the electro

Effect on neutrophil migration and antimicrobial functions by the Bruton's tyrosine kinase inhibitors tolebrutinib, evobrutinib, and fenebrutinib.

Multiple sclerosis (MS) is a neurodegenerative, autoimmune disease that is still incurable. Nowadays, a variety of new drugs are being developed to prevent excessive inflammation and halt neurodegeneration. Among these are the inhibitors of Bruton's tyrosine kinase (BTK). Being indispensable for B cells, this enzyme became an appealing therapeutic target for autoimmune diseases. Recognizing the emerging importance of BTK in myeloid cells, we investigated the impact of upcoming BTK inhibitors on neutrophil functions. Although adaptive immunity in MS has been thoroughly studied, unanswered quest

Elevated serum lipase in infants with cystic fibrosis exposed to prenatal and postnatal elexacaftor/tezacaftor/ivacaftor.

Cystic fibrosis transmembrane conductance regulator (CFTR) dysfunction leads to progressive exocrine pancreatic insufficiency, resulting in difficulty in the secretion of digestive enzymes and subsequent malabsorption of nutrients. Case reports have described preserved pancreatic function with prenatal elexacaftor/tezacaftor/ivacaftor (ETI) exposure. However, little is known about pancreatic function and injury as ETI exposure decreases postnatally. Here, we discuss four infants with cystic fibrosis (CF) who were prenatally exposed to ETI, with pancreatic function ranging from preserved to mil

Efficacy and safety of JAK inhibitors in patients aged > 60 years with moderate-to-severe atopic dermatitis: a 52-week multicenter, real-life study-IL AD (Italian Landscape Atopic Dermatitis).

Atopic dermatitis (AD) prevalence in elderly patients is increasing. Clinically, elderly AD may present with atypical phenotypes, making the diagnosis difficult. Moreover, treatment challenges arise due to treatment-resistance, comorbidities, polypharmacy, and contraindications to existing therapies. Janus kinase (JAK) inhibitors (abrocitinib, baricitinib, upadacitinib) may offer a valuable alternative. However, their use in elderly populations remains unclear, as older patients are often excluded from clinical trials, and several concerns have been raised about their safety in this category o

Emerging translocator protein-positron emission tomographic imaging improves detection of focal cortical dysplasia.

OBJECTIVE: The identification of epileptic lesions is crucial for improving surgical outcomes. Nevertheless, substantial focal cortical dysplasia (FCD) may be invisible on magnetic resonance imaging (MRI). We aimed to characterize the expression pattern of 18-kDa translocator protein (TSPO) in FCD and to evaluate the effectiveness of this inflammation-reflective molecular imaging technique for detecting FCD. METHODS: Patients clinically diagnosed with FCD, based on clinical features, interictal electroencephalographic (EEG) findings, and MRI characteristics, underwent positron emission tomogr

Analysis of postmarketing neuropsychiatric adverse events of avapritinib based on the FDA adverse event reporting system.

Neuropsychiatric adverse events (AEs) significantly impact the quality of life of patients using avapritinib. However, the majority of current data comes from pre-marketing, with limited real-world studies. Our research aimed to explore post-marketing data of avapritinib. We evaluated the signals of avapritinib-related neuropsychiatric AEs by data mining using the FDA Adverse Event Reporting System (FAERS). Reporting odds ratio (ROR) and information component (IC) were employed to quantify the signals from the first quarter of 2020 through the fourth quarter of 2023. Subsequently, stratified a

Cellular hierarchies of embryonal tumors with multilayered rosettes are shaped by oncogenic microRNAs and receptor-ligand interactions.

Embryonal tumor with multilayered rosettes (ETMR) is a pediatric brain tumor with dismal prognosis. Characteristic alterations of the chromosome 19 microRNA cluster (C19MC) are observed in most ETMR; however, the ramifications of C19MC activation and the complex cellular architecture of ETMR remain understudied. Here we analyze 11 ETMR samples from patients using single-cell transcriptomics and multiplexed spatial imaging. We reveal a spatially distinct cellular hierarchy that spans highly proliferative neural stem-like cells and more differentiated neuron-like cells. C19MC is predominantly ex
