Successful management of female office workers with "repetitive stress injury" or "carpal tunnel syndrome" by a new treatment modality--application of low level laser.

Female office workers with desk jobs who are incapacitated by pain and tingling in the hands and fingers are often diagnosed by physicians as "repetitive stress injury" (RSI) or "carpal tunnel syndrome" (CTS). These patients usually have poor posture with their head and neck stooped forward and shoulders rounded; upon palpation, they have pain and tenderness at the spinous processes C5-T1 and the medial angle of the scapula. In 35 such patients we focused the treatment primarily at the posterior neck area and not the wrists and hands. A low level laser (100 mW) was used and directed at the tip

Injuries in short track speed skating.

BACKGROUND: Little is known about the pattern of injury in short track speed skating. PURPOSE: To investigate the incidence and characteristics of injuries in short track speed skating. STUDY DESIGN: Retrospective study. METHODS: Ninety-five of 150 elite-level skaters (63.3%) were surveyed to collect information on training and competition load as well as on injuries sustained during the 1999-2000 competitive season. Injuries were characterized in terms of anatomic location, type of injury, time loss from training and competition, and circumstance of injury (acute onset during competition,

An investigation into complaints of wrist pain and swelling among workers at a factory manufacturing motors for refrigerators.

In 1987, a group of workers complained through their union that some of them developed wrist pain and swelling from the manual handling of heavy stators. The complaints were from the morning shift of a particular assembly line in a factory making motors for refrigerators. The precipitating factor appeared to be the change in the weight of the model assembled. The new stator weighing 5.8 kg was about twice as heavy as the previous model. Each worker on the line handled 300 to 1,000 stators per shift. The work involved including lifting the stators onto and off the conveyor belts and pushing and

Clinical and functional outcomes of distal biceps tendon repair using all-suture anchors.

BACKGROUND: Various surgical approaches and fixation methods are available for distal biceps tendon repair. Although all-suture anchors offer theoretical advantages and are widely used in orthopedic surgery, clinical data on their use in distal biceps tendon repairs remain limited. This study aims to evaluate the clinical and functional outcomes of distal biceps tendon repair using two all-suture anchors via a single-incision approach. We hypothesized that this technique would result in effective restoration of supination strength, excellent patient-reported outcomes, and a low rerupture rate.

Outcomes of Glenoid Labral Repair Using All-Suture Anchors.

BACKGROUND: In recent years, innovation in arthroscopic glenoid labral repair techniques has included the development of all-suture anchors. There are multiple potential advantages of all-suture anchors, including decreased bone removal, anchor migration, synovitis/chondral injury, and bone reaction. Despite these potential advantages, few studies have examined clinical outcomes of glenoid labral repair with all-suture anchors. PURPOSE: To evaluate patient-reported and return-to-sport outcomes of arthroscopic glenoid labral repairs using all-suture anchors. STUDY DESIGN: Case series; Level o

[Analysis of brachial plexus injuries reported to MRM].

Upper-arm weakness (paresis) or paralysis in the newborn (Erb's palsy) usually indicates peripheral-nerve damage to the brachial plexus. Its location lateral to the lower cervical spine (C5-T1) renders it susceptible to injury by pressure or traction during pregnancy, labor or delivery. The Medical Risk Management (MRM), a "Madanes" group company, routinely receives adverse events reports from medical centers covered by its medical malpractice insurance. In the current study, the authors analyzed 536 reports of Erb's palsy in the newborn, with varying degrees of severity, which were reported t

Intraoperative Nerve Conduction Studies During Open Carpal Tunnel Release: A Pilot Study.

BACKGROUND: Severe traction injury to the brachial plexus (BP) during childbirth can induce the formation of intraneural scar tissue or neuroma in continuity. This can lead to nerve entrapment and demyelination, which inhibit axonal transport to the target muscles. Secondary complications include muscle weakness, contractures, joint deformity, and altered limb growth with prolonged and persistent disability. These patients find difficulty performing activities of daily living. BP neurolysis is one of the multistage reconstructive procedures in patients with obstetric brachial plexus injury (OB

Complex Labral Tears Are Not Predicted by Number of Preoperative Dislocations nor Time From Injury to Surgery in Patients Receiving Arthroscopic Anterior or Posterior Labral Repairs.

PURPOSE: To evaluate whether complex glenoid labral tears (i.e., those involving >120°of the glenoid labrum) are related to demographics, injury patterns, and surgical procedures/timing in patients undergoing primary arthroscopic labral repair. METHODS: This retrospective chart review included patients with traumatic shoulder instability who underwent primary arthroscopic labral repair between 2012 and 2020. Patients with atraumatic multidirectional instability, clinical ligamentous laxity, or previous shoulder surgeries were excluded. Variables analyzed included age, sex, body mass index, co

Comparison of arthroscopic and open anterior shoulder stabilization. A two to six-year follow-up study.

BACKGROUND: Sixty-three consecutive patients with recurrent traumatic anterior shoulder instability underwent operative repair. The decision to select either arthroscopic Bankart repair or open capsular shift was based on the findings of an examination under anesthesia and the findings at the time of arthroscopy. Thirty-nine patients with only anterior translation on examination under anesthesia and a discrete Bankart lesion underwent arthroscopic Bankart repair with use of absorbable transfixing implants. Twenty-four patients with inferior translation in addition to anterior translation on ex

Heterotopic Ossification Leading to Ureteral Obstruction Resulting in Nephrectomy.

Background: Heterotopic ossification (HO) is the presence of bone in soft tissue where bone normally does not exist. This can be acquired or inherited with the acquired form most often seen with either trauma, spinal cord injury, or central nervous system injury. HO most commonly affects the flexors and abductors of the hip, medial knees, and the shoulders and rarely affects the genitourinary (GU) system. Case Presentation: We discuss a 67-year-old Caucasian male patient who presented with left-sided ureteral obstruction. He was involved in an airplane accident in 2001 resulting in a spinal co

Traumatic first rib fracture: is angiography necessary? A review of 730 cases.

The two most common sites of first rib fracture were at the subclavian sulcus and in the neck of the first rib, posteriorly. Five distinct mechanisms for rib fracture were identified and included: (i) posteriorly directed trauma to the upper thorax or shoulder girdle; (ii) a direct blow to the sternum and anterior chest wall; (iii) a blow fracturing the clavicle; (iv) a strong sudden contraction of the scalenus anticus muscle; and (v) radiographic findings of a first rib fracture without history of trauma. Isolated first rib fracture regardless of mechanism of injury, results in a low incidenc

Absorption of the bone fragment in shoulders with bony Bankart lesions caused by recurrent anterior dislocations or subluxations: when does it occur?

BACKGROUND: Recently, bony defects of the glenoid in patients with traumatic anterior shoulder instability have been increasingly noticed. The bone fragment of a bony Bankart lesion is often utilized for Bankart repair, but the fragment is at times smaller than the glenoid defect. The reason for this mismatch in size is unknown. HYPOTHESIS: The bone fragment of a bony Bankart lesion might gradually be absorbed over time. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 163 shoulders were prospectively examined by computed tomography. In shoulders with bony Bankart lesion

[Miniinvasive technologies in treatment of intraarticular splittered fractures of the proximal shoulder].

The algorithm of diagnostics, miniinvasive ostheosynthesis and postoperative rehabilitation of patients with the splittered fractures of caput femori was worked out. Treatment results of 24 patients were analyzed. All patients were treated with the use of the original apparatus of the external osteal fixation. The method allows early functional treatment and limb function restoration in cases, when classic immersinal ostheosynthesis is contraindicated.

Cerebral ischemia during shoulder surgery in the upright position: a case series.

We report 4 cases of ischemic brain and spinal cord injury after shoulder surgery in the beach chair position, using data from medical legal case reviews. We argue that the correlation between cardiovascular risk factors and cerebral ischemic complications for this type of surgery is poor in these middle-aged patients. Rather, our analysis suggests that the sitting position and the head position create specific physiological conditions that may be conducive to cerebral and spinal cord ischemia during this type of surgery. Thromboembolic events may be an additional cause of adverse neurologic o

Posttraumatic arteriovenous fistula and subclavian vein thrombosis: treatment by percutaneous arterial embolization and vein angioplasty.

A 71-year-old woman had edema and venous dilatation of her upper right limb that caused painful functional disability following a shoulder injury. Arteriograms demonstrated an arteriovenous fistula between the subclavian artery and vein associated with thrombosis of the vein at the same level. The arteriovenous fistula was found to have multiple arteriovenous communications. Because of associated distal venous thrombosis, venous drainage was retrograde through the brachial vein. The inflow arteries of the fistula were embolized and then the subclavian vein thrombosis was recanalized, dilated,

Coracoclavicular screw fixation for unstable fractures of the distal clavicle. A report of five cases.

Five consecutive unstable fractures of the distal third of the clavicle were treated by indirect open reduction and internal fixation using a temporary Bosworth-type screw. Coracoclavicular fixation provided and maintained reduction of the fracture. Healing occurred uneventfully within nine weeks in all cases. The screw was removed under local anaesthesia after healing of the fracture and there were no surgical complications. Shoulder function was restored to the pre-injury level. Temporary coracoclavicular screw fixation appears to be a valuable alternative for the treatment of type II fractu
