Neuro-Ophthalmological Manifestations of Obstructive Sleep Apnea: Current Perspectives.

Obstructive sleep apnea (OSA) is a disease of obstructed airways during sleep that significantly impacts the quality of life and increases the risk of various systemic diseases. OSA has been studied as a risk factor for a number of neuro-ophthalmic conditions and a strong relationship has been established with non-arteritic anterior ischemic optic neuropathy (NAION). The incidence of glaucoma and stroke have also been significantly associated with OSA and are conditions that may also be seen by neuro-ophthalmologists. Patients with NAION have a significantly higher incidence of OSA and OSA dia…

Encephalitis lethargica epidemic milestones in early sleep neurobiology researches.

Around 100 years ago, the outbreak of peculiar encephalitis promoted knowledge advancement regarding sleep and psychomotricity control. This epidemic is believed to have disappeared ten years after it started, and it remained from 1916 to 1927. Since then, only a few sporadic cases have been reported, but previously, they happened in occasional and epidemics forms. Two pioneers in describing the cases were Jean-René Cruchet and his collaborators, and Constantin Von Economo. The firsts described diffuse symptomatology, "sub-acute encephalomyelitis." However, the reports by the Austrian aristocr…

Post-Concussion Syndrome and Sleep Apnea: A Retrospective Study.

BACKGROUND: Concussion symptoms typically resolve within 7-10 days, but 10%-25% of patients do not fully recover. They can develop post-concussion syndrome (PCS), which includes sleep abnormalities such as obstructive sleep apnea. It is unclear how specific sleep problems manifest in PCS and how it relates to cognition and symptomology. METHODS: A retrospective chart review was conducted on PCS patients seen at the University Health Network (UHN) Concussion Clinic and sent for sleep study. Neuropsychology tests, concussion features, PCS symptoms, and demographics were abstracted from clinical…

Significance of hyposmia in isolated REM sleep behavior disorder.

OBJECTIVE: To determine if hyposmia in isolated REM sleep behavior disorder (IRBD) predicts short-term conversion to any α-synucleinopathy and declines with time. METHODS: Olfaction was tested using the University of Pennsylvania Smell Identification Test (UPSIT-40) in 140 consecutive patients with polysomnography-confirmed IRBD and in 77 matched controls. Patients were followed-up during 5.6 ± 3.9 (range 0.2-13) years. Twenty-one patients underwent serial UPSIT-40 evaluations at 1-3 and 4-6 years after baseline. RESULTS: UPSIT-40 score was lower in patients than in controls (20.2 ± 6.5 vs. …

Spontaneous and apnea arousals from sleep in preterm infants.

BACKGROUND: The significance of arousal in apnea termination in preterm infants is not known. METHODS: We investigated the appearance of arousals from sleep with polysomnography for 21 preterm infants at a median age of 36 gestational weeks. RESULTS: The polysomnographic appearance of sleep was fragmented by frequent arousals. The number of spontaneous arousals unrelated to apneas was 18 per hour in sleep; higher in rapid eye movement (REM) sleep than in non-REM sleep (p < 0.001). Eighty-two percent of arousals were regarded as spontaneous, and 18% were related to apneas. In turn, arousal fo…

Central Sleep Apnea Predicts Pulmonary Complications After Cardiac Surgery.

BACKGROUND: Postoperative major pulmonary complications (MPCs) continue to be leading causes of increased morbidity and death after cardiac surgery. Although various risk factors have been identified, reports on the association between sleep-disordered breathing (SDB) and postoperative MPCs remain inconclusive. RESEARCH QUESTION: What is the incidence of the composite end point postoperative MPCs? What are predictors for postoperative MPCs in patients without SDB, with OSA, and with central sleep apnea (CSA) who undergo cardiac surgery? STUDY DESIGN AND METHODS: In this subanalysis of the on…

Alzheimer's Disease in Patients with Obstructive Sleep Apnea Syndrome.

BACKGROUND: Obstructive sleep apnea syndrome (OSAS) is a disorder with high prevalence among adults and is an independent risk factor for various diseases, especially those affecting the central nervous system (CNS). Continuous positive airway pressure (CPAP) is usually the optimal choice of treatment for OSAS. Alzheimer's disease (AD) is a neurodegenerative disease affecting a large proportion of the elderly population. The purpose of this study was to collect information concerning the two pathological entities and investigate the effectiveness of CPAP in the treatment of AD. MATERIALS AND …

Respiratory and sleep-related complications of multiple system atrophy.

PURPOSE OF REVIEW: The purpose of this article is to provide a contemporary review of sleep issues affecting patients with multiple system atrophy (MSA). RECENT FINDINGS: Prodromal symptoms of MSA may occur years prior to diagnosis, including autonomic dysfunction such as orthostatic hypotension, urogenital dysfunction, rapid eye movement (REM) sleep behavior disorder (RBD), and stridor. Patients may also develop sleep-related respiratory disorders such as obstructive sleep apnea (OSA), central sleep apnea (CSA), and stridor. The development of stridor is associated with a shortened lifespan …

Predicting Poor Sleep Quality in Fibromyalgia with Wrist Sensors.

Fibromyalgia is a musculoskeletal disorder characterized by chronic, widespread muscle pain. This condition is associated with disturbed sleep, which has a direct impact on patient quality of life. Patient-reported outcomes are frequently used to assess sleep quality, but show modest correlations with objective measures of sleep, such as polysomnography. Working towards our goal of an automated ambulatory system of assessing sleep quality, we use features from blood volume pulse (BVP) and electrodermal activity (EDA) collected with a wearable device during sleep. We compare these measurements …

Sleep quality and chronic neck pain: a cotwin study.

STUDY OBJECTIVES: Sleep quality and chronic neck pain (NP) are associated. However, the genetic influences on this association have not been explored. This study investigated the genetic and environmental influences on the association between sleep quality and chronic NP. METHODS: The sample comprised 2,328 individual twins from the Murcia Twin Registry (Spain). A bidirectional cotwin logistic regression analysis was performed (sleep quality assessed as the exposure and chronic NP as the outcome and vice versa). Analysis included 2 sequential stages: total sample analysis and within-pair twin…

Examining subjective sleep quality in adults with hoarding disorder.

Hoarding disorder (HD), characterized by difficulty parting with possessions and functionally impairing clutter, affects 2-6% of the population. Originally considered part of Obsessive-Compulsive Disorder (OCD), HD became a distinct diagnostic entity in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) in 2013. While sleep impacts OCD, little is known about sleep in HD. As HD patients often report poor sleep in clinical settings, understanding global subjective sleep quality and disturbances may lead to novel therapeutic targets. To address this gap, the authors used a sample o…

Inner ear function in patients with obstructive sleep apnea.

OBJECTIVE: Because of their high metabolic activity and low-resting oxygen tension, the organs of the inner ear are vulnerable to hypoxia, a condition that occurs repetitively in obstructive sleep apnea-hypopnea syndrome (OSAHS). The present study aimed to investigate the inner ear function of patients with OSAHS. METHODS: A total of 58 patients with OSAHS (116 ears) and 20 adults without OSAHS were enrolled in the present study. The clinical features, such as air-conduction thresholds, auditory brainstem response (ABR, 11 times/s and 51 times/s stimulation rates), and distorted products otoa…

Rehearsal initiates systems memory consolidation, sleep makes it last.

After encoding, memories undergo a transitional process termed systems memory consolidation. It allows fast acquisition of new information by the hippocampus, as well as stable storage in neocortical long-term networks, where memory is protected from interference. Whereas this process is generally thought to occur slowly over time and sleep, we recently found a rapid memory systems transition from hippocampus to posterior parietal cortex (PPC) that occurs over repeated rehearsal within one study session. Here, we use fMRI to demonstrate that this transition is stabilized over sleep, whereas wa…

Obstructive sleep apnoea as a late complication after tracheostomy.

BACKGROUND: Obstructive sleep apnoea occurs consequent to partial or complete upper airway obstruction, caused mostly by the collapse of upper airway musculature. Drug-induced sleep endoscopy represents the 'gold standard' in identifying the obstruction site, from the palatal level to laryngeal entry. Breathing impairment in sleep caused by the collapse of cervical trachea after previous tracheostomy has not yet been described in the literature. METHODS: This report presents two patients with severe obstructive sleep apnoea, in whom pre-operative drug-induced sleep endoscopy revealed upper ai…
