Health Equity: Foundation of Serious Illness Care.

Health equity is foundational to the delivery of high-quality serious illness care. Despite advances in palliative care, substantial disparities persist in symptom burden, quality of life, caregiver distress, access to specialty services, and end-of-life outcomes across racial, ethnic, socioeconomic, geographic, and other structurally marginalized populations. These inequities are driven not by individual patient characteristics alone, but by structural, institutional, and social factors including discrimination, language discordance, disability, transportation barriers, housing instability, f

Distance, Disparity, and Duty: An Empirical Bioethical Analysis of Adolescent Sexual and Reproductive Health Access in Rural Uganda.

Disparities in adolescent sexual and reproductive health (SRH) access in rural Uganda are commonly attributed to geographic distance, resource scarcity, and service availability. These explanations, while important, remain ethically incomplete unless they also account for duty: the social and economic responsibilities that constrain adolescents' capacity to prioritise health needs. Drawing on empirical findings from a study of 408 adolescents aged 13-19 years in the Bugweri and Iganga districts of eastern Uganda, this paper integrates Rawlsian distributive justice and the capability approach t

Posthumous health data donation: an emerging framework for equitable healthcare AI.

OBJECTIVE: To examine posthumous health data donation as a pathway toward more representative datasets used to develop healthcare artificial intelligence (AI). PERSPECTIVE: Healthcare AI systems are increasingly deployed in clinical settings, yet the datasets underlying these systems remain narrow, fragmented, and systematically underrepresenting key patient populations. Current consent frameworks, designed for living patients, create structural barriers to assembling diverse training data. Death meaningfully shifts the ethical risk-benefit calculus, substantially reducing harms, employment d

Structure-Guided Discovery of Neuroprotective Kromycin-Type Macrolides from Streptomyces narbonensis sp.

Six new kromycin-type macrolides, kromycins B-G (1-6), and with the known kromycin A (7), were isolated from Streptomyces narbonensis sp. CPCC 206470 guided by the Global Natural Products Social Molecular Networking (GNPS) analysis. Their structures were comprehensively established by extensive spectroscopic analyses, time-dependent density functional theory electronic circular dichroism (TDDFT-ECD) comparison, and single-crystal X-ray diffraction analysis. These compounds represent a structurally distinct subclass of 14-membered macrolides. Compound 7 exhibited substantial neuroprotective act

Exploring the lived experience of food insecurity through the camera lens: A photo elicitation study.

Healthy eating remains unattainable for many due to limited access to affordable, nutritious foods. In the United Kingdom, over 11% of the population is estimated to be food insecure. This study explored the lived experiences of adults living with food insecurity and examined how these experiences shape their dietary health behaviours. Using a photo-elicitation approach, food insecure adults (N = 9) photographed their food environments. The photos were used to guide semi-structured interviews which were transcribed and analysed using reflexive thematic analysis. Participants viewed healthy foo

Is Chronic Emptiness a Transdiagnostic Suicide Risk Factor?

Previous research suggests associations between suicidality (i.e., suicidal ideation and attempt) and chronic emptiness in patients with borderline personality disorder (BPD). However, approximately one-third of clinical patients without BPD endorse feelings of emptiness. The current study examined the relationship between chronic emptiness and suicidal behavior in 3,284 psychiatric outpatients. Patients were evaluated with the Structured Interview for DSM-IV Personality (SIDP-IV), the Structured Clinical Interview for the DSM-IV (SCID-IV), and the Schedule for Affective Disorders and Schizoph

A qualitative analysis of health information seeking among African American adults in South Carolina.

Structural racism impacts education, economics, insurance coverage, and overall health. As a result, African American adults often have poorer access to quality healthcare, which is interconnected to social determinants of health. This study used a health and financial literacy approach to qualitatively determine how African American Adults in South Carolina find out about health information, how they decide the information is true, and how to pay for it (n = 20). Results indicated that participants place great value on the opinions of family and friends as well as health professionals and onl

Structured speech tasks reveal temporal facial variability as an objective measure in the de-novo Parkinson's disease patients.

Diminished facial expression, a hallmark symptom of Parkinson's disease (PD), significantly impacts non-verbal communication and social interactions. This study explores the use of temporal facial variability (TVV) as a objective measure for motor impairment severity in de-novo PD patients. A cohort of 42 participants performed structured speech tasks, and their facial dynamics were captured using high-resolution 3D sensors. Advanced geometric and statistical analyses, including radial curve tensors and temporal volume variation metrics, were applied to quantify facial muscle activity. Cluster

A Systematic Review on Assessing Assessments: Unveiling Psychometric Properties of Instruments for Reactive Attachment Disorder and Disinhibited Social Engagement Disorder in Minors under Protective Measures.

BACKGROUND: Reactive attachment disorder (RAD) and disinhibited social engagement disorder (DSED) manifest in individuals facing attachment system challenges, particularly observed in minors under protective measures. The lack of standardized tools for assessing these difficulties and uncertainty about the most effective instruments from a psychometric perspective prompted this study. AIM: Using the COSMIN checklist, we systematically reviewed instruments assessing RAD, adhering to PRISMA. METHODOLOGY: Examined tools included the Disturbance Attachment Interview, Preschool Age Psychiatric As

Language, Social, and Face Regions Are Affected in Toddlers with Autism and Predictive of Language Outcome.

Identifying prognostic early brain alterations is crucial for autism spectrum disorder (ASD). Leveraging structural MRI data from 166 ASD and 109 typical developing (TD) toddlers and controlling for brain size, we found that, compared to TD, ASD toddlers showed larger or thicker lateral temporal regions; smaller or thinner frontal lobe and midline structures; larger callosal subregion volume; and smaller cerebellum. Most of these differences were replicated in an independent cohort of 38 ASD and 37 TD toddlers. Moreover, the identified brain alterations were related to ASD symptom severity and

Top five considerations for improving outcomes in black patients with heart failure: A guide for primary care clinicians.

Black patients develop heart failure at younger ages and have worse outcomes such as higher mortality rates compared to other racial and ethnic groups in the United States. Despite significant recent improvements in heart failure medical therapy, these worse outcomes have persisted. Multiple reasons have been provided to explain the situation, including but not limited to higher baseline cluster of cardiovascular risk factors amongst Black patients, inadequate use of heart failure guideline directed medical therapy and delayed referral for advanced heart failure therapies and interventions. Gross anatomy of the longitudinal fascicle of Sapajus sp.

Opposing genetic and cultural-social explanations for the origin of language are currently the focus of much discussion. One of the functions linked to the longitudinal fascicle is language, which links Wernicke's area and Broca's area in the brain, and its size should indicate the brain increase in the evolution. Sapajus is a New World primate genus with high cognition and advanced tool use similar to that of chimpanzees. A study of the gross anatomy of the longitudinal fascicle of Sapajus using Kingler's method found it to differ from other studied primates, such as macaques and chimpanzees,

Factor Analysis of the Stigma Scale-Parent Version in Pediatric Disorders/Differences of Sex Development.

Stigma is a salient experience for both caregivers/parents and individuals with Disorders/Differences of Sex Development (DSD) as evidenced through qualitative and preliminary quantitative reports. However, few validated measures of associative stigma (i.e., vicarious stigma experienced through close association with someone who is socially stigmatized) for parents of children with DSD exist. The present study aims to (1) determine the factor structure of the adapted Stigma Scale - Parent, and (2) examine convergent validity of the factor structure with measures of parent psychosocial adjustme

Medical abortion through telehealth in India: a critical perspective.

The COVID-19 pandemic and nationwide lockdowns have significantly impacted access to abortions for millions of people in India. One of the proposed solutions to address the inaccessibility of abortion services is the use of teleconsultation or "telemedicine" where pregnant persons consult with Registered Medical Practitioners (RMPs) over voice or video calls. The RMPs then prescribe the necessary medication electronically. However, telehealth services bring their own set of social and legal challenges. Although they can allow for greater access to medical abortions, especially in situations wh

Towards Communicative Justice in Health.

This article approaches care from a different angle by looking ethnographically at how it is shaped by structural differences in the power to control the circulation of knowledge. I focus on an investigation conducted by people classified as "indigenous", of an epidemic that killed 38 children and young adults in a Venezuelan rainforest. I trace how health/communicative inequities structured clinical interactions, documents, epidemiological investigations, news stories, and dialogues with healers, thwarting the identification of the epidemic, clinically identified as rabies. Although the Boliv

Neurobiology of Pediatric Anxiety Disorders.

While the fear-based anxiety disorders (i.e., generalized anxiety disorder, social phobia and separation anxiety disorder) are among the most common psychiatric conditions in children and adolescents, only recently has an integrated understanding of the neurobiology of these disorders developed. In this regard, both structural and functional neuroimaging studies have demonstrated neuroanatomic and functional abnormalities within the amygdala and prefrontal cortex in youth with fear-based anxiety disorders, and have also suggested altered functional connectivity among components of the anterior

Examining negative effects of early life experiences on reproductive and sexual health among female sex workers in Tijuana, Mexico.

OBJECTIVE: To explore experiences during childhood and adolescence that influenced reproductive and sexual health among women who had entered the sex industry in adolescence. METHODS: A qualitative study was conducted using information provided by 25 female sex workers (FSWs) from Tijuana, Mexico, who reported entering the sex industry when younger than 18 years. In-depth, semi-structured interviews were conducted with all participants between January 31, 2011, and July 8, 2011. RESULTS: Four interrelated themes that shaped health experiences-early sexual abuse, early illicit drug use, ongoi

Mathematical tools for planning effective intervention scenarios for sexually transmitted diseases.

BACKGROUND: We studied the dynamics of sexually transmitted diseases in structured populations, deriving analogies from the theory of metapopulations. GOAL: The goal was to study the impact of potential interventions, such as reducing the probability of transmission (e.g., by condom use), reducing the duration of infectiousness (e.g., by early diagnosis and treatment), or reducing the number of new contacts. STUDY DESIGN: A structured SIS model was used for the study. RESULTS: We extended the concept of core groups to the concept of nodal epidemiologic contribution (NEC) by considering the

Neuropsychological correlates of early symptoms of autism.

Both the medial temporal lobe and dorsolateral prefrontal cortex have been implicated in autism. In the present study, performance on two neuropsychological tasks--one tapping the medial temporal lobe and related limbic structures, and another tapping the dorsolateral prefrontal cortex--was examined in relation to performance on tasks assessing autistic symptoms in young children with autism, and developmentally matched groups of children with Down syndrome or typical development. Autistic symptoms included orienting to social stimuli, immediate and deferred motor imitation, shared attention,

Psychometric Evaluation of the Motivators and Resources for Trauma Recovery (I- MOVE) Scale Among Gender-Based Violence Survivors.

Gender-based violence (GBV) is a pervasive public health issue with profound psychological, relational, and structural consequences. While survivors draw upon internal resources, external resources, internal motivators, and external motivators throughout the healing process, existing instruments typically assess these constructs independently rather than within a unified, survivor-centered framework. The Motivators and Resources for Trauma Recovery (I-MOVE) Scale was developed to address this gap through a strengths-based assessment of recovery facilitators among women survivors of GBV. The fo
