Following a review of the research literature on the psychophysiological effects of cocaine, a study is described of a group of 120 cocaine addicts. Of the 120 patients, 10 (8.33%) exhibited fleeting, unformed, organic delusions and hallucinations. Case reports of the 10 cases are presented. The quality of the adverse subjective effects of cocaine is emphasized, and the differential diagnosis between Cocaine Delusional Disorder and Paranoid Schizophrenia is discussed. Guidelines for a more accurate differential diagnosis are provided.

Widespread cocaine use has caused concern about possible harmful effects during pregnancy. Several studies have suggested an increased incidence of abruptio placentae and stillbirths with cocaine use during pregnancy. This study examined what percentage of pregnant women in a lower socioeconomic population use cocaine, as well as positive urine cocaine screens in those with abruptio placentae and intrauterine fetal death (IUFD). A case of optic neuropathy induced by thinner sniffing is reported. A 17-year-old girl, who had been sniffing a lacquer thinner for three months, suffered acute blindness followed by optic atrophy. Brain computed tomography revealed symmetrical low attenuation areas in the bilateral putamen. The hemodynamic changes associated with ingestion of cocaine may precipitate rupture of ectopic pregnancy. Two cases are presented in which this association was suspected. We believe this to be the first report of this clinical entity, representing a risk factor that should be identified in patients with suspected ectopic pregnancy.

Neuropsychological performance of 23-42-year-old males with diverse drinking histories was assessed. Comparisons were made among nondrinkers, social drinkers, short-term sober (less than 30 days) alcoholics and long-term sober (greater than 30 days) alcoholics. In 1972, a drug incidence survey was conducted at a private southern university. Pooled results of this random survey of 1,032 students revealed that on at least one occasion, 90% of the students had tried alcohol, 70% had used tobacco, and 61% had experimented with marijuana. The prevalence of hepatitis D virus infection was studied in 954 sera samples collected in Spain between 1974 and 1986 from 838 patients in a variety of epidemiological categories with HBsAg-positive liver disease, and from 116 haemophiliacs and drug addicts with hepatitis B antibodies. A sample of 1776 adult women in treatment for chemical dependency was divided into two age groups: 30 or under and over 30. The two groups were compared for substance use histories and other psychosocial variables. Significant differences emerged in the use of illicit drugs, social use patterns and familial histories. Findings suggest that differences may be related to generational as well as age effects. The study highlights the heterogeneity of women in treatment and emphasizes the need to assess and respond to women's chemical dependency in the context of sociocultural transitions.

By identifying some of the social correlates linked with completing day-care drug abuse treatment, the present study has sought to broaden understanding of how drug rehabilitations are effected. As the findings have demonstrated, completing care is a result of a complex array of causes and their interaction. Hyperprolactinemia and sexual dysfunction occurred in 7 of 10 chronic cocaine users. Two of these 7 patients who underwent rehabilitation for cocaine abuse and developed hyperprolactinemia and decreased libido are described. One patient developed galactorrhea. These adverse effects were reversed by bromocriptine. The pathophysiology of cocaine abuse and the central dopaminergic influence on sexual function are discussed.
