Extracellular volume fraction by cardiac computed tomography (ECVCT): opportunities and challenges for clinical implementation.

Cardiac computed tomography (CT)-derived extracellular volume (ECVCT) estimates myocardial interstitial expansion by quantifying the distribution of iodinated contrast within the extracellular space, providing a CT marker of focal and diffuse fibrosis as well as amyloid infiltration. In single-energy CT, ECVCT is calculated from the change in myocardial and blood-pool attenuation between non-contrast and delayed scans, adjusted for haematocrit. Spectral CT, including photon-counting CT (PCCT), can quantify ECVCT from energy-resolved data and may reduce reliance on a separate true non-contrast

IgA Subclasses and Free Light Chains in Celiac Disease: A Pilot Study.

Celiac disease (CD) is an autoimmune enteropathy of the small intestine affecting genetically susceptible individuals, characterized by an aberrant immune response to gliadin and sustained IgA-driven inflammation. IgA exists in two main subclasses, IgA1 and IgA2, which differ in distribution and function, but their profile in CD remains poorly characterized. Circulating free light chains (FLCs) are markers of B-cell activation and immune dysregulation, yet their role in CD has not been fully explored. The aim of this study was to characterize IgA subclasses and FLC profiles in newly diagnosed

NLRX1 is a critical neutrophil checkpoint modulating ferroptosis and suppressing NETosis to attenuate joint damage in arthritis.

Neutrophil extracellular traps (NETs) are key drivers of inflammation and tissue damage in rheumatoid arthritis (RA), yet the mechanisms that restrain NETs overproduction remain unclear. Here, we identify the mitochondrial NOD-like receptor NLRX1 as a critical checkpoint limiting NETs production. The expression of NLRX1 is dysregulated in RA patients. NLRX1 deficiency leads to a profound expansion of the neutrophil population, and neutrophil-specific NLRX1 deficiency results in broad production of cytokines, chemokines, autoantibodies and complement accompanied by markedly accelerated arthriti

Innovations in the management of urethral stricture disease: emerging and regenerative strategies.

Urethral stricture disease remains a challenging clinical entity within contemporary urology, requiring interventions that achieve sustained urethral patency while preserving functional outcomes. Established modalities such as urethral dilation, endoscopic urethrotomy, and urethroplasty using autologous grafts provide effective treatment for many patients. However, they are constrained by morbidity, graft contraction, and suboptimal success in complex or recurrent strictures. These inherent limitations have stimulated growing investigation of therapeutic strategies. Among device-based innovati

Subarachnoid hemorrhage induces expansion of pro-inflammatory lymphocyte subsets in human cerebrospinal fluid: a pilot study.

Despite recent advances in treatment, subarachnoid hemorrhage from a ruptured brain aneurysm (aSAH) remains a significant cause of morbidity and mortality worldwide. While some early effects of aneurysm rupture (direct trauma to brain tissue, for example) are unrelated to the immune response, research suggests that some early and many delayed effects of aSAH such as vasospasm, delayed cerebral ischemia, chronic neurological decline, are at least partially immune mediated. Cytokines, neutrophils, and other immune markers are known to correlate with these delayed sequelae. Based on our prior wor

Intercalated cells contribute to the immune responses in renal autoimmune disease.

Autoimmune diseases are a major cause of acute and chronic kidney injury, driven by complex mechanisms involving autoantibodies and cellular immune responses that result in tissue damage. Intercalated cells (ICs), specialized renal epithelial cells responsible for proton secretion, are strategically positioned at the epithelial-immune interface, making them ideal sensors of stress signals and potential regulators of immune responses. To investigate the molecular mechanisms by which ICs crosstalk with immune cells during autoimmune kidney injury, we depleted Foxp3+ regulatory T-cells (Tregs) by

The Gut-Skin Axis in Atopic Dermatitis and Inflammatory Bowel Disease: Mechanisms, Microbiota, and Therapeutic Implications.

The gut-skin axis is a bidirectional communication network linking the gastrointestinal tract, skin, microbiota, and immune system. As major barrier organs, the gut and skin harbor complex microbial communities that contribute to tissue homeostasis, immune regulation, and protection from environmental insults. Increasing evidence indicates that dysbiosis of the gut and skin microbiota is associated with inflammatory diseases through interconnected microbial, metabolic, and immune pathways. Clinical observations further reveal strong associations between gastrointestinal disorders, including in

Written in the Stars: Astrocyte Biology From Evolution to Disease.

In the 21st century, neuroglial research has entered a period of Renaissance, extending the views of prominent neuroanatomists and neurologists of the 19th and early 20th centuries, who assigned to glial cells numerous physiological functions and highlighted their fundamental role in the pathophysiology of nervous system diseases. Astrocytes are highly diversified in structure and function; they control brain homeostasis, support synaptic connectivity, and enable information processing in neural networks. Evolutionary diversification of astrocytes, initially emerging as supportive cells of pri

Molecular Determinants of Intravitreal Anti-VEGF Durability: Drug Architecture, Intraocular Pharmacokinetics, Target Biology and Treatment Resistance.

Intravitreal inhibition of vascular endothelial growth factor (VEGF) has transformed the management of neovascular age-related macular degeneration, diabetic macular oedema and macular oedema secondary to retinal vein occlusion, but frequent monitoring and retreatment remain major burdens. Ocular half-life is often used as shorthand for durability, although the clinical interval is produced by a wider molecular and biological system. This narrative review examines how dose, molecular format, hydrodynamic size, binding affinity, valency, ligand spectrum, target turnover, tissue distribution and

Obesity-Associated Inflammatory Reprogramming Impairs a Granulomatous Response to Leishmania infantum.

The demographics of leishmaniasis in Brazil have shifted in recent decades, coinciding with dietary changes and an increase in overweight/obesity. Cutaneous leishmaniasis becomes more treatment refractory in obese individuals, highlighting the impact of obesity on disease outcome. The impact of the overweight/obese state on infection with Leishmania infantum, which causes visceral leishmaniasis (VL), is unclear. We previously showed that an obesity-prone diet restricts L. infantum expansion in the livers of mice, but the immunological basis remained unexplored. Herein, we investigated whether

Recurrent Calciphylaxis With Discordant Parathyroid Hormone Trajectory in a Patient With HIV-Associated End-Stage Renal Disease: A Case Report.

Calciphylaxis, also termed calcific uremic arteriolopathy, is a rare and devastating complication of end-stage renal disease (ESRD). Its occurrence in patients with HIV-associated ESRD is sparsely characterized in the literature. We report a 36-year-old African American woman with ESRD secondary to longstanding hypertension and virologically suppressed HIV infection (CD4 count 348 cells/μL) who developed three discrete presentations of recurrent calciphylaxis over 18 months, with histopathologic confirmation at initial diagnosis and at first recurrence. At initial presentation (T1), intact par

Dahlia Virome Analysis Reveals Multiple Novel Viruses in Mixed Infections.

Dahlia is an economically important ornamental crop in the United States and worldwide. Dahlia is primarily propagated vegetatively through bulbs, which can result in vertical transmission of and constant accumulation of viruses. In this study, mosaic and yellowing symptoms were observed in Dahlia plants in a residential garden in California. Dahlia samples were collected and sequenced using high-throughput sequencing (HTS). HTS analysis was used to explore the Dahlia virome in the symptomatic plants, which revealed the presence of several contigs with homology to known Dahlia-infecting viruse

Interleukin-18 Constrains the Intestinal Tuft Cell-Driven Responses During Helminth Infection.

Type 2 immunity at mucosal surfaces is essential for helminth clearance, tissue repair, and barrier maintenance, processes governed by a robust feed-forward circuit between chemosensory tuft cells and group 2 innate lymphoid cells (ILC2s). However, the regulatory mechanisms that calibrate the magnitude and duration of this epithelial immune circuit remain incompletely defined. Here, we identify interleukin-18 (IL-18), a cytokine traditionally associated with type 1 immunity, as a key regulator of tuft cell-mediated type 2 responses during Nippostrongylus brasiliensis (N. brasiliensis) infectio

Thrombin-dependent nanoscale architecture and fibrinolytic remodeling of hydrated fibrin clots revealed by expansion microscopy.

BACKGROUND: Fibrin network architecture governs clot stability, permeability, and susceptibility to fibrinolysis, yet nanoscale characterization of hydrated fibrin structures remains challenging. OBJECTIVE: To apply expansion microscopy (ExM) for quantitative subdiffraction-resolution imaging of fixed, hydrated fibrin architecture and determine how fibrin structure varies with thrombin, regulates clot permeability, and remodels during fibrinolysis. METHODS: Fibrin structure in human platelet-free plasma clots was quantified across thrombin concentrations (0.3-1.5 U/mL) and during tissue-type

Seasonal dynamics of vitamin D metabolism in the oviduct of the Chinese Brown frog (Rana dybowskii).

The R. dybowskii oviductal tissue undergoes significant expansion during pre-brumation rather than the pre-spawning period, reflecting a unique physiological adaptation for brumation. While vitamin D is known to support growth and reproductive functions in various organisms, its role in seasonal oviductal hypertrophy remains unclear. In this study, the vitamin D metabolic pathway's role in regulating this process was investigated. Morphometric measurements revealed a significant increase in oviductal weight and luminal diameter during pre-brumation compared to pre-spawning, while histological

Clonal dynamics on the road to hematological malignancy (MBL, MGUS, CH).

Monoclonal gammopathy of undetermined significance (MGUS), monoclonal B-cell lymphocytosis (MBL), and clonal hematopoiesis (CH) of indeterminate potential represent highly prevalent, age-associated precursor states characterized by detectable serological, cellular, or genetic alterations in otherwise asymptomatic individuals. The advent of high-sensitivity technologies, including mass spectrometry, multiparametric flow cytometry, and next-generation sequencing, has revealed that these conditions are far more widespread than previously appreciated, yet only a minority of affected individuals pr

GITRL enhances cytotoxicity and persistence of CAR-T cells in cancer therapy.

Chimeric antigen receptor (CAR)-T cell therapy has achieved remarkable clinical success in treating hematological malignancies. However, its clinical efficacy in solid tumors is less satisfactory, partially due to poor in vivo expansion and the limited persistence of CAR-T cells. Here, we demonstrated that the overexpression of GITR ligand enhances the anti-tumor activity of CAR-T cells. Compared to prostate-specific membrane antigen-BB-Z (PSMA-BB-Z) CAR-T, PSMA-BB-Z-GITRL CAR-T cells have much more interferon (IFN)-γ, TNF-α, and interleukin (IL)-9 secretion, a higher proportion of central mem

Novel screening approach for stem cell selective inhibitors and their possible translational therapeutic potential for endometriosis.

Endometriosis is an estrogen-dependent benign disease characterized by growth of the endometrial tissue outside the uterine wall. Several reports suggest the possibility of the pathogenesis and recurrence of endometriosis being related to functions of stem/progenitor cells of the endometrium. The drawback of the widely used method of using Hoechst 33342, a fluorescent dye, to collect stem cell-like populations, is the requirement of an ultraviolet (UV) excitation source not commonly provided on standard flow cytometers. Here, we aimed to overcome this hurdle by establishing a novel method that

Pyroptosis in acute pancreatitis: from pathogenesis to treatment.

Acute pancreatitis (AP) is characterized by unpredictable inflammation and potentially fatal outcomes. Pyroptosis is a type of lytic cell death mediated by the pore-forming gasdermin protein that supports inflammation and organ damage by releasing proinflammatory mediators and recruiting inflammatory cells. This review aims to comprehensively evaluate the recent advances in pyroptosis as another possible pathogenesis of AP, focusing on its mechanism and medicinal potential. Our findings suggest that pyroptosis is a finely regulated process, and overactivation of pyroptosis leads to uncontrolle
