BACKGROUND: Supracondylar humeral fractures are the most common elbow fractures in children. In case of displacement and instability, the standard procedure is closed reduction and percutaneous Kirschner wire fixation. As Kirschner wire fixation requires postoperative cast immobilization, does not allow early mobilization, and is associated with the risk of damage of the ulnar nerve, innovative techniques should be evaluated. Therefore, the aim of the study was to assess both radiologic and functional outcome of supracondylar humeral fractures treated by elastic stable intramedullary nailing (

OBJECTIVES: Ipsilateral concomitant fractures of proximal extracapsular and distal femur are rare injuries and pose a great challenge for orthopaedics. In this study, we reviewed and examined the approaches and outcomes of this complex injury.
METHODS: From August 2002 to January 2010, seven patients (six males and one female) with a mean age of 39 years (range, 20-48 years) were involved in the study. They had suffered from ipsilateral concomitant fractures of proximal extracapsular and distal femur, with two cases of unstable intertrochanteric fractures, three cases of subtrochanteric fract

This paper analyses 80 cases of displaced proximal humerus fractures, with two or three fragments, treated by PHN nailing. The hold of the spiral blade was effective, and is particularly recommended for fractures with two fragments according to Neer's classification. The results, analysed with Constant's score and radiographic checkups, were satisfactory. Nearly all our patients (76 out of 80) had a mean age of 75 years and a total Constant score of 78.48. Only four (mean age 42.5 years) had a Constant score of 86. Shoulder articulation was good and only a few complications occurred.

The results of closed and open reduction via posterior approach with percutaneous pinning of posteromedial displaced supracondylar humerus fractures in children were evaluated. Fifty-five consecutive patients with Gartland type III fractures were reviewed. The mean follow-up period was 22 months (12-48 months). The closed reduction group consisted of 32 patients and the open reduction group with the posterior approach using the triceps-sparing method consisted of 23 patients. Both groups were stabilized with cross Kirschner wire fixation and followed the same protocol. In comparison with close

UNLABELLED: Leg lengthening by external fixation is associated with various difficulties. We evaluated eight adolescent patients who underwent leg lengthening with a motorized intramedullary lengthening device. We asked whether this method could reduce the time of hospitalization and rehabilitation and whether the incidence of complications commonly associated with external fixators could be reduced. We compared our preliminary results with those from other reports, with a focus on leg length achieved, time of rehabilitation, and rate of complications. The average leg-length discrepancy was 3.

TYPE OF STUDY: A 2 to 4-year clinical and radiologic prospective study was conducted for 39 patients over the age of 65 treated by lumbar arthrodesis.
OBJECTIVES: Evaluate the quality of lumbar arthrodesis results in elderly patients, in which the bone osteoporosis (poor quality of the spine's anatomic components) and general factors (frequent comorbidity) are likely to make this surgery difficult if not dangerous and evaluate a dedicated instrumentation for osteoporotic bone.
MATERIALS AND METHODS: Thirty-nine patients over 65 years of age (average 73 y) underwent arthrodesis surgery in 200

PURPOSE: To assess the results of treatment for flexion-type supracondylar humeral fracture in children.
METHODS: The treatment of 14 children with flexion-type supracondylar humeral fracture was reviewed. Severity was classified according to the Gartland system for extension-type fractures. Type-I fractures were treated with immobilisation in an extension cast. For type-II and -III fractures, closed reduction was first attempted followed by percutaneous pinning. If closed reduction failed, open reduction and internal fixation was performed.
RESULTS: Patients were followed up for at least on

This study was performed to determine if picture archiving communication systems can provide a more accurate method of determining implant length for intramedullary tibial nailing. Postoperative radiographs of 40 patients who underwent intramedullary nailing of their tibial shaft fractures using picture archiving communication systems were retrieved. In phase one and two of this investigation, tibial nail lengths were measured using "measuring distance" and "measure calibration" tools displayed on the respective digital systems. Phase 3 of this study involved 5 tibial Sawbones (Pacific Researc

UNLABELLED: We analyzed the relationship between knee pain after tibial nailing and nail prominence. We identified 70 patients in our trauma registry with healed fractures initially treated with intramedullary nails. Subjective pain and function were measured with visual analog pain scales and Lysholm knee scores at a mean of 20 months after fracture. These scores were compared with nail prominence measured on postoperative radiographs. More than 49% of patients had knee pain. Subjective knee pain was more common in women and patients with a smaller plateau width. Anterior nail prominence was

UNLABELLED: The Wichita Fusion Nail (WFN) is a knee arthrodesis stabilization system that employs compression via an intramedullary rod. It was designed for use in the salvage of the irretrievably failed total knee arthroplasty and other severe knee pathologies. Questionnaires covering the fusion success rate, fusion time, and complication rate were obtained from 33 surgeons who were among the first to use the device. Data from these questionnaires were analyzed to determine if the rate of successful fusion was close to 100%, which was the primary hypothesis of this study. The average time req

The purpose of this article is to present the results of Desmanet's intramedullary pinning for distal radius fractures, and also to present modifications of the cut-end management of pins; the cut-ends were shaped like a tear-drop for the radial pin and like a curtain-hook for the ulnar pin. One hundred and two operated patients (23 men and 79 women; mean age at operation, 61.0 +/- 17.5 years) were followed up for 9.6 +/- 12.9 months. The clinical results were satisfactory according to the demerit point system of Saito; 83 were assessed as excellent and 19, as good. On the X-ray measurements,

Avascular necrosis with or without resultant limb-length discrepancy and problems with bony union is not uncommon after femoral neck fractures in the pediatric population. This rare case of infection after closed reduction and percutaneous pinning of an intracapsular fracture of the hip in a child illustrates the treatment difficulties posed by joint sepsis and the value of modern scanning techniques.

The management of femoral shaft fractures by retrograde intramedullary nailing is becoming more widespread. There have been no reported intraoperative neurovascular injuries to the surrounding anatomy using the retrograde femoral nailing technique. We report a case of injury to a branch of the profunda femoris artery during placement of the anteroposterior proximal locking screw.

A consecutive series of 23 patients with reamed retrograde femoral nails was reviewed. Nails were placed through the intercondylar notch with a minimal incision. Nineteen patients with retrograde femoral nails were available at an average follow-up of 19.3 months. The union rate was 100% with no infections or malunions. No second surgeries were required for union. Knee range of motion averaged 109 degrees and was greater in those patients with shaft fractures (117 degrees) than in those with supracondylar-intercondylar fractures (91.3 degrees) (P=.02). Pain (0-3 scale) averaged 0.36. Hospital

The senior author has used 41 femoral recon nails of one design in 41 consecutive patients with 30 impending and 11 pathologic femur fractures. All nails were inserted with minimally invasive surgical technique. The hospital stay was shorter (average, 5.6 days) and discharge to home was more likely (77%) after stabilization of impending pathologic fractures compared with fixation of completed fractures (length of hospital stay averaged 7.8 days, with a 36% discharge-to-home rate), as patients with completed fractures required a higher level of postoperative care. Fixation prior to fracture occ

Twenty-five wrists with comminuted, displaced, intra-articular fractures of the distal radius were prospectively treated with an adjustable external fixator for an average of 51 days. Twenty fractures (80% [20 of 25] were available for follow-up) in 16 adults (mean age, 34 years) were treated with the Wrist Jack external fixator system (Hand Biomechanics Lab, Sacramento, CA) and evaluated at a mean follow-up period of 25 months. Ten patients (12 fractures) sustained high-energy trauma with multiple injuries, while 6 patients (8 fractures) sustained isolated distal radius fractures. Percutaneou

Fractures around the elbow are common injuries in children. We studied 77 children who had sustained supracondylar fractures around the elbow during the period 1989-1993. The aim of this study was to compare the outcomes of supracondylar fractures following 3 modes of treatment, namely manipulation and reduction, open reduction with internal fixation and percutaneous pinning. In our study, we have found that the most common cause for injury in supracondylar fractures was a fall on the outstretched hand. A possible reason would be that children usually try to break the force of a fall by extend

A retrospective review of 115 patients treated surgically for displaced supracondylar humeral fractures was conducted. All patients had a minimum of 2 years' follow-up (range 2 to 7 years). According to a modified Gartland (19) classification system, 18.3% were type II (21/115), 77.4% were type III (89/115), and 4.3% (5/115) type IV supracondylar fractures. Twelve percent (14/115) of patients presented with neurapraxias, with the radial nerve being the most commonly affected. Sixty percent (69/115) of patients were treated with a crossed K-wire configuration, while another 30% (35/115) receive

This paper presents a report of a study of 14 hips in 12 non Caucasian patients treated for severe slipped capital femoral epiphysis. There were four Sudanese, two Syrians, and six Libyan patients. Three hips in two patients had severe acute slip and eleven hips in ten patients had severe chronic slip. Patients with severe acute slip were treated by closed reduction and internal fixation. Patients with severe chronic slip were treated by open replacement of the femoral epiphysis using the procedure advocated by Dunn and Angel[1]. One patient treated for acute severe slip developed over reducti
