This case report describes a new surgical approach for the treatment of cyclodialysis. The treatment of cyclodialysis cleft associated with hypotony is in a period of evolution. In this report, a patient with traumatic cyclodialysis associated with hypotony maculopathy was treated with contact transscleral diode laser therapy to the cleft areas. The contact diode laser therapy resulted in closure of the clefts, reversal of hypotony, and resolution of maculopathy with improvement in vision. The results of this study show that this technique can be less invasive than other surgical options.

PURPOSE: The aim of this study was to investigate the incidence of unexpected malignant uveal melanoma in the age of ultrasound diagnostics and to highlight the reasons for misdiagnosis.
PATIENTS AND METHODS: All eyes were surgically removed and histologic examination was performed between 1981 and 1995. The eyes were investigated for the incidence of uveal melanoma, and the history of the unexpected malignant melanoma of the uvea or ciliary body highlighted.

Both filtering surgery with Mitomycin-C and diode laser cyclophotocoagulation have proved to be effective alternatives in cases of glaucoma with poor surgical prognosis. The right eyes of 40 pigmented rabbits were randomly divided into 2 groups: Mitomycin-C group underwent filtering surgery with application of 0.4 mg/ml of Mitomycin-C whereas the diode laser (DL) group received 15 applications of 1.8 J (1800 mW. 1000 ms) distributed in 270 degrees. Mean IOP showed statistical differences in two periods between days 3-13 and 45-60-Mann-Whitney U test -with a higher IOP lowering in the DL group.

A role for NO in aqueous humor dynamics, and thus regulation of intraocular pressure, has been postulated. In order to help clarify how much Neodymium:YAG transscleral cyclophotocoagulation (Nd:YAG CPC) treatment should be given to maximize effectiveness and minimize complications in the treatment of refractory glaucomas, we retrospectively reviewed and compared the results achieved with an initial 180-degree noncontact Nd:YAG CPC treatment with those achieved with an initial 360-degree treatment. We describe herein a technique for locating the ciliary sulcus for suturing a posterior chamber intraocular lens. Transscleral illumination with a light-guide fiber placed underneath the iris reveals the ciliary sulcus as a bright area near the limbus, and it allows for a straight needle with 10-0 polypropylene fiber to be easily inserted through the sulcus after making two pairs of radial scleral incisions to fixate the 10-0 polypropylene fiber. We think that this procedure can minimize complications of suturing a posterior chamber intraocular lens and shorten the operation time.
