Graves ophthalmopathy.

Graves ophthalmopathy (GO) is an autoimmune disease that typically affects the thyroid gland and the orbit. The roles of the thyroid stimulating hormone receptor and cellular and humoral immunity in the pathogenesis of GO are being investigated. Investigators are gaining insights into the epidemiology of GO as it relates to other ocular and autoimmune diseases. Several possible tools for assessing GO disease activity are being studied. Medical and radiation therapy for GO recently have been evaluated prospectively. Orbital surgeons continue to refine orbital decompression techniques to improve

Restoration of vision by radiotherapy for severe, acute, steroid-refractory, congestive thyroid orbitopathy.

We report the case history of a patient treated with radiotherapy for severe acute thyroid orbitopathy that failed to respond to high-dose corticosteroid therapy. The clinical response to irradiation was rapid, dramatic and sustained after extended follow-up. Although radiotherapy has been used in dysthyroid eye disease, there remains debate over the precise indications for and the timing of its utilization. A review of the literature on the use of radiotherapy for acute thyroid orbitopathy has been undertaken, emphasizing its steroid-sparing potential and its role in avoiding the necessity fo

[Correction of kyphosis in tuberculous spondylitis in children].

Various modifications of radical reparative surgery were made in 145 children with tuberculous spondylitis were comparatively analyzed. Greatest correction of kyphosis was achieved by combining anterior spinal repair with posterior plunged contractors, in additional resection of the apical vertebral arch in particular. There is a direct correlation between the degree of compression of the dural sac and the severity of neurological disorders. Anterior decompression of the spinal cord is an obligatory surgical component in tuberculous spondylitis, eliminates central stenosis of the spinal canal;

Spinal epidural lipomatosis as a complication of prolonged corticosteroid therapy.

We present the case of a 42-year-old man who complained of low back pain radiating into the posterior aspect of both thighs. There were no neurological deficits, while general medical examination revealed a Cushing's syndrome due to corticosteroid therapy for rheumatic polyarthritis. Myelography showed irregular and circumferential narrowing of the dural sac from L-3 to S-1 caused by abundant epidural fat as demonstrated by MRI. Surgical decompression produced immediate relief of symptoms. The literature of epidural spinal lipomatosis is reviewed and therapeutic considerations are discussed.

Long-term results of treatment for axillary subclavian vein thrombosis.

Thrombolytic therapy is an effective method for reopening an occluded venous segment, but little is known about the long-term results of thrombolysis via direct intravenous route for axillary subclavian vein thrombosis (ASVT). Our experience with 13 cases of primary ASVT illustrates the possible advantages of a protocol consisting of lytic therapy, surgical decompression and percutaneous balloon angioplasty if a high-grade stenosis persists. If accompanied by adjunctive therapy to correct the underlying cause, thrombolytic therapy for ASVT results in acceptable long-term symptom relief and vei

[A case of Phalen-Dickson syndrome].

A case of dysplastic spondylolisthesis with a complete slipping of the L5 vertebrae accompanied by neurological disorders and characteristic contracture of hamstrings (Tight Hamstrings Syndrome) has been described. This disease entity, described in literature, has a name of Phalen-Dickson Syndrome. Neurological disorders were caused by compression of radices at the level of the slipping. Surgical treatment were: laminectomy of the arcus of L5, decompression of the compressed radices with postero-lateral fusion of the L5-S1 segment without reduction of the slipping. The final, functional and ra

Modified gastrojejunostomy tube: percutaneous placement for gastric decompression and jejunal feeding.

A four-lumen, 20-F surgical gastrojejunostomy tube designed to incorporate separate lumens for both jejunal feeding and gastric decompression was modified for guide-wire use. This modification permits radiologic rather than intraoperative tube placement. The tube was successfully placed in six patients. Slow gastric leakage that developed at the stoma site in one patient immediately after tube insertion resolved after 24 hours. No catheter dislodgments or other complications occurred. At 4-month follow-up, five patients had fully functional tubes; the sixth patient had died with a functional t

Meniere's disease 1978-1982.

Over 250 reports concerning or related to Meniere's disease, published in English from 1978 through 1982, are summarized. Reports have been classified into the categories of incidence of occurrence, analysis and reporting results, pathophysiology, etiologic theories, diagnostic methods, medical management, and surgical procedures. Major emphases in reporting from 1978 though 1982 have been in the areas of pathophysiologic studies, expansion of diagnostic procedures, and the use of sac decompression and neurectomy procedures. This literature review reveals the continuing disagreement regarding

Pseudocyst of the pancreas complicating distal splenorenal shunt. A report of three cases with successful percutaneous drainage.

Three cases of pseudocysts of the pancreas complicating the operative procedure of distal splenorenal shunt are reported. In all of these patients, gross changes in the pancreas, consistent with chronic pancreatitis were found. Attention is called to the possibility of this complication occurring in association with this particular operation, specifically, when it is carried out in patients who may prove to have a considerable degree of pancreatic and retroperitoneal fibrosis secondary to alcoholic pancreatitis. All three pseudocysts were satisfactorily managed by percutaneous drainage under u

Abnormalities of the cranio-vertebral junction with cervico-medullary compression. A rational approach to surgical treatment in children.

8 children with cranio-vertebral abnormalities and neurological deficits are reviewed. The specific treatment is dependent on etiology, mechanisms of compression, and whether the bony abnormality can be reduced to its normal position. Irreducible ventral compression of the cervico-medullary junction was relieved by transoral removal of the odontoid-clivus complex in 3 patients. Primary posterior decompression was done in 4 children with dorsal encroachment. A reducible C1-C2 dislocation in 1 child with juvenile rheumatoid arthritis underwent primary posterior fusion. A detailed surgical physio

Anatomy of the achondroplastic lumbar canal.

The narrowed bony canal in the achondroplastic spine cases varied neurologic problems. The specific anatomic areas seem not to have been directly measured in an autopsy specimen. An achondroplastic spine was examined radiographically and found to show decreased interpediculate distance, thick pedicles, inferior facet thickness, and for animal stenosis in the lumbar canal. These findings have direct application to surgical treatment insofar as simple laminectomy for achondroplastic dwarfs may be insufficient to produce neural canal decompression. The problem is likely to require thorough exposu

Risk factors of neurological deficit and pulmonary cement embolism after percutaneous vertebroplasty.

BACKGROUND: The risk factors, incidence, and clinical management of pulmonary cement embolism and neurological deficit during percutaneous vertebroplasty (PVP) were evaluated. METHODS: Three thousand one hundred and seventy-five patients with symptomatic osteoporotic vertebral compression fractures (OVCFs) treated with PVP were retrospectively reviewed in a single institution. Clinical parameters such as age, gender, number of fractures, and time from fracture to vertebroplasty were recorded at the time of surgery. Image and surgical parameters including the amount of cement, the vertebral le

Small-diameter percutaneous decompression for osteonecrosis of the shoulder.

Core decompression of the humeral head has previously been used as a joint-preserving procedure for treatment of symptomatic osteonecrosis of the shoulder. In this article, we describe a new decompression technique, which involves multiple small-diameter (3-mm) percutaneous perforations. In our study population (early-stage disease), shoulder arthroplasty was avoided in all 15 patients (26 shoulders) for a mean follow-up of 32 months (range, 24-41 months). Of the 26 shoulders, 25 had successful clinical and functional outcomes (University of California Los Angeles shoulder score, >24 points),

Minimally Invasive Decompression versus Open Laminectomy in Multilevel Lumbar Stenosis: A Systematic Review and Meta-Analysis.

OBJECTIVE: This meta-analysis compares clinical outcomes of minimally invasive decompression (MID) versus open laminectomy surgery (OLS) for multilevel lumbar spinal stenosis. METHODS: A systematic search was conducted in PubMed, Embase, and the Cochrane Central databases to identify studies comparing MID to OLS in patients with multilevel lumbar spinal stenosis. Primary outcomes were length of hospital stay, operation time, complication rate, intraoperative blood loss, reoperation due to recurrence (RDR), and low back pain 1 year after surgery. RESULTS: Of 3695 articles screened, 4 studies

Acute thoracic disc herniation with severe spinal cord compression: a case report.

This case report highlights the rare presentation of an acute thoracic disc herniation in a 27-year-old male. Thoracic disc herniations are uncommon, accounting for less than 1% of all disc herniations, and acute presentations have scantly been recorded in literature. The patient, a mechanic, presented with a sudden onset of dorsal pain and bilateral lower limb weakness after lifting heavy equipment, leading to a sudden cease of most motor functions in the patient's lower limbs. Magnetic resonance imaging revealed a severe T9/T10 herniation with significant spinal cord compression. Emergency s

Streptococcus gallolyticus-related spinal epidural abscess.

Spinal epidural abscess (SEA) is a surgical emergency if it causes paraplegia. Staphylococcus aureus and streptococci are the most common causes. Streptococcus gallolyticus has been reported to cause SEA only on three occasions earlier-all were associated with endocarditis or colonic malignancy. We report an older woman with diabetic ketoacidosis who presented with poorly localised back pain, fever and altered sensorium. Her lumbar puncture revealed frank pus, and MRI showed an SEA. She could not be weaned from mechanical ventilation post-surgical decompression, and she succumbed to ventilator

EANO guideline on the diagnosis and treatment of vestibular schwannoma.

The level of evidence to provide treatment recommendations for vestibular schwannoma is low compared with other intracranial neoplasms. Therefore, the vestibular schwannoma task force of the European Association of Neuro-Oncology assessed the data available in the literature and composed a set of recommendations for health care professionals. The radiological diagnosis of vestibular schwannoma is made by magnetic resonance imaging. Histological verification of the diagnosis is not always required. Current treatment options include observation, surgical resection, fractionated radiotherapy, and

Modified lateral orbital wall decompression in Graves' orbitopathy using computer-assisted planning.

Graves' orbitopathy, a condition seen in the autoimmune syndrome Graves' disease, affects the fatty tissue and muscles inside the orbit. Graves' orbitopathy is associated with increasing exophthalmos and sometimes leads to compressive dysthyroid optic neuropathy, resulting in progressive vision loss. Dysthyroid compressive optic neuropathy, functional problems, and cosmetic problems are the main indications for surgical decompression of the orbit, especially if conservative treatment has not led to a reduction in symptoms. Many surgical techniques are described in the literature. This article
