Even complex vesicovaginal fistulas may be repaired transvaginally. In difficult repairs adjunctive measures have been described to increase the success rate of surgery, including Martius flaps, labial flaps or gracilis muscle flap. We describe a new technique to repair difficult vesicovaginal fistulas via a transvaginal approach. A flap of peritoneum is developed and used to buttress the fistula repair. We report on 2 debilitated male patients who had penile urethral complications of transurethral catheter drainage. One patient had a urethral diverticulum, and the other had a Foley catheter misplaced through a urethral perforation. Both lesions mimicked primary intrascrotal masses on physical examination and were therefore evaluated by scrotal sonography. Nontraumatic perforation of the small intestine is very rarely found as a cause of acute abdominal disease. A series of 15 adult patients admitted to our hospital from 1973 to 1983 is reviewed.
