OBJECTIVES: We report a case of an association of an arachnoid cyst with heterotopic neuroglial tissue in the internal auditory canal.
MATERIAL AND METHODS: A 66-year-old woman consulted for cochleovestibular syndrome.
RESULTS: MRI demonstrated a lesion with spontaneous hypersignal on T1- and T2-weighted images, instigating surgical exploration. We discovered a hematic arachnoid cyst associated with heterotopic neuroglial tissue arising in the internal auditory canal.
CONCLUSION: An arachnoid cyst arising within the cerebellopontine angle or the internal auditory canal is a rare occurrence.

Three uncommon cases of ear trauma caused by a yucca plant leaf spine are presented. One patient presented with tympanic perforation and the second with mixed hearing loss after spontaneous closure. The third patient probably had a perilymphatic fistula with subsequent labyrinthitis and hearing loss. Although the yucca is a ubiquitous plant, to the best of our knowledge, such incidents have not been previously reported.

OBJECTIVES/HYPOTHESIS: In 1991, the translabyrinthine approach was modified by sealing the vestibule with bone wax, which allowed preservation of the hearing function in one patient. The present study aimed specifically at evaluating the effectiveness of the modified translabyrinthine approach in preserving hearing function in a group of patients with vestibular schwannoma that involved the internal auditory canal.
STUDY DESIGN: Prospective study.
METHODS: The series consisted of 12 patients with vestibular schwannoma (average age, 49.7 y). The schwannoma was smaller than 2 cm in all patient

The authors present a very rare case of concurrent longitudinal and transverse fracture of temporal bones. In this case, both the manifestations as well as the clinical image are quite typical of longitudinal and transverse fractures, the only difference being the lack of posttraumatic paralysis of the facial nerve, which is frequent in the case of transverse fractures. It must be noted that fractures of temporal bones generally occur as the result of road accidents or gunshots, since only a very substantial force may cause the fraction of the cranial basis. Nevertheless, in this case the frac

BACKGROUND AND PURPOSE: In patients with clinical symptoms suggestive of a retrocochlear disorder, contrast-enhanced T1-weighted spin-echo (SE) high-field-strength MR imaging is considered the criterion standard in assessing vestibular schwannoma. However, only 10-20% of its findings are pathologic. Our purpose was to prospectively compare the performance of low-field-strength MR imaging in screening for retrocochlear disorders, with high-field-strength MR imaging as the criterion standard.
METHODS: A total of 287 patients with suspected retrocochlear disease underwent axial 1.5-T MR imaging

Our aim in this study was to distinguish quantitatively between the localization accuracy of a commercially available stereotactic fixation device as claimed by the manufacturer and the target accuracy as measured by a user, applying neuroradiologic imaging in Gamma Knife planning and phantom irradiation. Missing the target is the most serious possible failure in Gamma Knife and Linac therapy. To reduce this risk, we developed a quality control algorithm and designed a phantom. To evaluate the accuracy of the targeting procedure with a Leksell Gamma unit, and to experience the possible errors

BACKGROUND AND PURPOSE: Although contrast-enhanced T1-weighted MR imaging is the standard of reference for diagnosing tumor in the cerebellopontine angle, high-resolution T2-weighted imaging may show more details of the seventh and eighth cranial nerve branches, resulting in more accurate tumor volume measurements. The purpose of this study was to compare two MR sequences for their ability to delineate internal auditory canal tumors.
METHODS: Twenty-seven ears in 21 patients with 16 confirmed schwannomas were studied with the 3D T2-weighted prototype segment-interleaved motion-compensated acq

Although many studies have suggested a relation between allergy and Ménière's disease, the pathophysiology of this condition remains controversial. The aim of this study was to clarify whether an anaphylactic reaction in the inner ear can disturb hearing and equilibrium, and whether such disturbances recur in response to repeated anaphylactic reactions. Increases in audiological threshold, nystagmus, and endolymphatic hydrops were observed in response to a single exposure to antigen administered to actively sensitized guinea pigs. The increase in audiological threshold was maximal 10 h after a

Degenerative changes of the cervical spine often cause the pains and vertigo as well as pathological results of otoneurological tests. The aim of the study was to estimate the frequency of the symptoms occurrence in the group of 146 patients, 63 male and 83 female, with degenerative changes of the cervical spine. Permanent or periodical headaches of various intensity occurred in 60% of the patients. Balance disturbances was observed in 74% of the patients, specially in women. In the cervical spine X-ray there were observed most frequently marginal degenerations with osteophytosis (16%), interv

A 44-year-old man with a documented 12-year history of progressive sensorineural hearing loss developed a generalized tonic-clonic seizure followed by a visual field deficit and apraxia. Six months later he developed a peripheral neuropathy and muscle fatigue followed by a slowly progressive aphasia and cortical blindness as well as increased seizure activity. A computed tomography scan showed bilateral basal ganglion calcification. The serum lactate level was elevated at 3.4 mEq/dL. A muscle biopsy enabled the diagnosis of mitochondrial myopathy. This disorder is presented as an unusual cause

The surgery of cerebellopontine angle tumors has shown remarkable progress over the last 20 years due to improved microsurgical techniques. However, the dissection of the facial nerve may lead to postoperative paresis as the result of the surgical trauma and the disruption of blood supply over a large distance. The functional status of the nerve can be intraoperatively monitored by means of intramuscular electromyography of facial muscles and direct electrical stimulation. In this study, the impact of monitoring techniques on the preservation of facial function was evaluated by comparing monit

Treatment of bilateral Menière's disease is a delicate task. Streptomycin has demonstrated its ability to successfully control the vertigo attacks, but in some cases it results in oscillopsia and transforms the periodic attacks to permanent instability. If the site of the active labyrinth can be determined, a more specific treatment can be tried. We have treated 14 patients with intractable bilateral Menière's disease with intratympanic gentamicin (Garamycin, 40 mg/ml) administered initially in either 1, 2, 3, or 4 injections. The patients were tested at frequent intervals and followed up for

The clinical diagnosis of organic brain syndromes (especially of primary degenerative dementia) in their early stages is uncertain or only tentative. We used a set of relatively simple tests to support the hypothesis that the disturbance of several operational brain functions precedes memory decline in primary degenerative dementia. Twenty-one patients with early stage, presenile onset of DAT, 14 patients with cardiovascular disease (CVD) and a questionable dementia, an age-matched control group (EC, n = 15), and a younger control group (YC, n = 16) were examined in the following tests: labyri

PURPOSE: To assess the value of MR in patients with sensorineural hearing loss (SNHL) caused by lesions other than acoustic neuromas.
METHODS: MR studies of 51 patients with SNHL were retrospectively reviewed; patients with acoustic neuroma were excluded to focus on the more uncommon causes.
RESULTS: Twenty patients had labyrinthine lesions. Six patients had viral labyrinthitis, one patient had bacterial labyrinthitis, and one patient had luetic labyrinthitis. Three patients had hemorrhage in the labyrinth, two posttraumatic and one spontaneous from an adjacent temporal bone tumor. Only one

A clinical and vestibulometric comparison of 10 patients following labyrinthectomy, 5 following vestibular nerve section, and 10 control subjects was undertaken in order to evaluate the long-term effectiveness of these procedures. Testing was performed a mean of 8.5 years after surgery. Vestibulometry assessed time constants, gain, and phase advance of the vestibulo-ocular reflex (VOR) in both horizontal and vertical planes of head movement. Loss of the anticipated increase in phase advance in the vertical VOR of the labyrinthectomy group, significantly different from the vestibular nerve sect
