Congenital complete heart block in an infant with free plasma carnitine deficiency.

A newborn male presenting with congenital complete heart block was found to have low levels of free plasma carnitine. Among the many complications of carnitine deficiency is a structural disruption of myocardial tissue. A deficiency of carnitine may be causally related to the presence of cardiac dysrhythmias in this patient on the basis of mechanical disturbance to the cardiac conducting system.

Bronchial hypersensitivity to methacholine in monkeys with beta-adrenergic blockade.

In monkeys with beta-adrenergic blockade caused by administration of propranolol, we found that the conductance of the total respiratory system markedly decreased following intravenous administration of methacholine. The presence of beta-adrenergic blockade was judged from inotropic effect on the heart, levels of blood glucose and lactic acid, and the reaction of eosinophils after adrenalin injection.

Caring at the crossroads: Exploring end-of-life challenges for advanced heart failure patients in saudi emergency departments.

BACKGROUND: Patients with advanced heart failure often present to hospital emergency departments during acute health crises near the end of life. While emergency departments focus on medical stabilization, they may not adequately address the emotional, spiritual, and personal needs of patients during this critical period. METHODS: This qualitative study was conducted at a major tertiary hospital in Saudi Arabia. Semi-structured interviews were conducted with twelve patients diagnosed with advanced heart failure to explore their experiences of end-of-life care in the emergency department. Them

Isochronal late activation mapping in ischemic scar VT: an independent validation using intraoperative mapping.

BACKGROUND: VT ablation is often limited by hemodynamic instability. Substrate-based ablation strategy is often useful, and isochronal late activation mapping (ILAM) in sinus rhythm (SR) to identify deceleration zones (DZ) has been reported to be effective. AIM: We aimed at independently validating ILAM concept using data from comprehensive intraoperative VT mapping. METHOD: Panoramic simultaneous endocardial intraoperative mapping was performed using a 112 bipolar electrode balloon. The late potentials in SR were mapped in eight isochrones, each of 15-30ms duration and correlated to the dia

Prevalence and characteristics of atrioventricular nodal reentrant tachycardia with a bystander concealed nodoventricular/nodofascicular pathway.

BACKGROUND: The concealed nodoventricular/nodofascicular (NV/NF) pathway is mostly a bystander, retrograde bypass tract connecting the right ventricle/right bundle branch (RBB) and slow pathway that is observed in patients with atrioventricular nodal reentrant tachycardia (AVNRT). However, its prevalence and characteristics in response to pacing maneuvers have not been fully evaluated. OBJECTIVE: This study investigated the prevalence and characteristics of AVNRT with a bystander NV/NF pathway. METHODS: We retrospectively reviewed 153 consecutive patients undergoing catheter ablation of AVNR

Whole-Heart Histological and Electroanatomic Assessment of Postinfarction Cardiac Magnetic Resonance Imaging Scar and Conducting Channels.

BACKGROUND: Cardiac magnetic resonance imaging (CMR)-defined ventricular scar and anatomic conduction channels (CMR-CCs) offer promise in delineating ventricular tachycardia substrate. No studies have validated channels with coregistered histology, nor have they ascertained the histological characteristics of deceleration zones (DZs) within these channels. We aimed to validate CMR scar and CMR-CCs with whole-heart histology and electroanatomic mapping in a postinfarction model. METHODS: Five sheep underwent anteroseptal infarction. CMR (116±20 days post infarct) was postprocessed using ADAS-3

Dynamic QT response to cold-water face immersion in long-QT syndrome type 3.

BACKGROUND: Abnormal dynamics of QT intervals in response to sympathetic nervous system stimulation are used to diagnose long-QT syndrome (LQTS). We hypothesized that parasympathetic stimulation with cold-water face immersion following exercise would influence QT dynamics in patients with LQTS type 3 (LQT3). METHODS: Study participants (n = 42; mean age = 11.2 years) comprised 20 genotyped LQTS children and 22 healthy children. The LQTS group was divided into LQT3 (n = 12) and non-LQT3 (n = 8) subgroups. Provocative testing for assessing QT dynamics comprised a treadmill exercise followed by

Are wall thickness channels defined by computed tomography predictive of isthmuses of postinfarction ventricular tachycardia?

BACKGROUND: Wall thickness (WT) in post-myocardial infarction scar is heterogenous, with channels of relatively preserved thickness bordered by thinner scar. OBJECTIVE: This study sought to determine whether 3-dimensionally-reconstructed computed tomography (CT) channels correlate with electrophysiological isthmuses during ventricular tachycardia (VT). METHODS: We retrospectively studied 9 postinfarction patients (aged 57 ± 15 years, 1 female) with 10 complete VT activation maps (cycle length 429 ± 77ms) created using high-resolution mapping. Three-dimensionally-reconstructed WT maps from CT

Contact force facilitates the achievement of an unexcitable ablation line during pulmonary vein isolation.

AIMS: Contact force (CF) catheters provide catheter-tissue contact information to improve outcome of pulmonary vein isolation (PVI) in paroxysmal atrial fibrillation (PAF). We evaluated different target-CF values for achievement of the additional endpoint of an unexcitable ablation line. METHODS: A total of 106 patients undergoing PVI were randomized into three groups (G) (G1: target-CF 15 g, G2: target-CF 10 g, G3: CF concealed from operator). The PVI encircling line was divided into predefined sections. Excitable tissue along the PVI-line identified by high output pacing (10 V, 2 ms) was ta

Catheter ablation of paroxysmal atrial fibrillation in patients with sick sinus syndrome.

Sick sinus syndrome (SSS) frequently coexists with atrial fibrillation (AF). The results of AF ablation in patients with SSS have not been fully evaluated. We retrospectively investigated 65 patients with paroxysmal AF (PAF) and SSS who underwent AF ablation using either radiofrequency (n = 50) or cryoballoon ablation (n = 15) in our institute. Forty-nine (75.4%) patients had a median of 5.6 (4.8-6.0) s of documented sinus pauses prior to the procedure (42 patients on antiarrhythmic drugs), and were observed when AF terminated in 47 patients. Successful pulmonary vein isolation was achieved in

Lack of prognostic value of atrial arrhythmia inducibility and change in inducibility status after catheter ablation of atrial fibrillation.

BACKGROUND: Previous studies have suggested a role of atrial arrhythmia inducibility as an endpoint of catheter ablation of atrial fibrillation (AF). The prognostic value of noninducibility after ablation and of a change in inducibility status has not been investigated in large studies. OBJECTIVE: The purpose of this study was to evaluate the prognostic role of noninducibility and of a change in inducibility status after ablation of AF. METHODS: We studied 305 consecutive patients with AF (66% paroxysmal) undergoing antral pulmonary vein (PV) isolation plus non-PV triggers ablation. All pati

[Catheter ablation of persistent atrial fibrillation : Where do we go?].

Despite the very promising initial results of clinical studies, catheter ablation of persistent atrial fibrillation (AF) remains a challenge in modern electrophysiology. On the basis of the hypothesized pathophysiological mechanisms, a variety of ablation strategies have been developed over the course of time. The current ablation strategies range from pulmonary vein isolation (PVI) alone as first-line therapy, through ablation of multiple linear lesions, ablation of complex fractionated atrial electrograms (CFAE), rotor ablation, isolation of fibrotic and scar tissue up to isolation of the le

Diagnostic Accuracy of Several Electrocardiographic Criteria for the Prediction of Atrioventricular Nodal Reentrant Tachycardia.

BACKGROUND AND AIMS: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common form of paroxysmal supraventricular tachycardia (SVT) whose diagnosis can be strongly suspected based on the surface eletrocardiogram alone. The purpose of this study is to determine the diagnostic accuracy of several electrocardiographic (ECG) criteria for the prediction of AVNRT. METHODS: Between November 2010 and January 2014, a total of 256 patients who underwent electrophysiological testing (EP) with regular, paroxysmal and narrow QRS complex tachycardia were prospectively enrolled. We classified

Determining the Site of Accessory Pathways in Orthodromic Reciprocating Tachycardia by Using the Response to Right Ventricular Pacing.

BACKGROUND: Postpacing interval (PPI) after right ventricular (RV) pacing entrainment minus tachycardia cycle length (TCL) with a correction for atrioventricular (AV) node delay (corrected PPI-TCL) was useful to differentiate atrioventricular node reentrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT). However, the value of corrected PPI-TCL in determining the site of the accessory pathway (AP) in ORT has not been investigated. The purpose of this study was to assess whether the corrected PPI-TCL is useful in differentiating ORT using a left-sided AP from a right-sided
