[Perinatal asphyxia, known topic, new diagnostic prospects--advisability of troponin T in perinatal medicine].

Perinatal asphyxia is a major topic in neonatology. It has a high impact on neonatal mortality and morbidity and neurological and intellectual development of the infant. Hypoxia triggers compensatory mechanisms in the fetus and the newborn aimed to sustain adequate oxygen transport to the vital organs. Long periods of hypoxia and poor perfusion cause failure of mentioned compensative mechanisms and lead to irreversible damage in tissues. No clear-cut physiologic parameters exist which allow for an early identification of neonatal infants who are either at risk to develop brain damage or posthy

Subacute hepatic failure after the perinatal period with haemochromatotic siderosis at the age of 11 months: an unusual perspective on neonatal haemochromatosis.

UNLABELLED: Neonatal haemochromatosis is a rare disease characterized by the early onset of liver failure, a rapidly deteriorating clinical course and high mortality. An unusual case is presented of subacute hepatic failure in which autopsy findings at the age of 11 mo showed a haemochromatotic pattern of siderosis. Despite the similarity in the pattern of iron distribution to neonatal haemochromatosis, the clinical course was highly atypical in that the infant had later and milder presentation at 1 mo of age and slow progression into liver failure. This case illustrates the heterogeneous natu

Vaccination update. Hib, hepatitis, polio, varicella, influenza, pneumococcal and meningococcal disease.

Immunization of infants and children is the most effective strategy for decreasing the incidence of some infectious diseases. Most invasive disease due to Haemophilus influenzae type b occurs before age 5 years, and routine vaccination of infants for hepatitis B is currently recommended because selective immunization of high-risk persons has not been feasible. Decades of use of poliovirus vaccine has effectively eliminated cases of wild-virus infection, although some vaccine-related cases still occur. The newly approved varicella vaccine appears to be a cost-effective way to decrease infection

Recurrent meningitis and Mondini dysplasia.

Three infants with cerebrospinal fluid otorrhea and recurrent meningitis due to Mondini dysplasia have been treated at Baylor College of Medicine, Houston, Tex; 39 other patients with this association have been described. This review highlights the characteristics and embryologic basis of the malformation. The clinical evaluation of an infant with recurrent meningitis is outlined, and the importance of brain-stem response audiometry and thin-cut computed tomography of the temporal bones is discussed. Surgical management depends on the functional status of the dysplastic ear; usually, a stapede

Stillbirths and early neonatal mortality in rural Northern Ghana.

OBJECTIVE: To calculate perinatal mortality (stillbirth and early neonatal death: END) rates in the Upper East region of Ghana and characterize community-based stillbirths and END in terms of timing, cause of death, and maternal and infant risk factors. METHODS: Birth outcomes were obtained from the Navrongo Health and Demographic Surveillance System over a 7-year period. RESULTS: Twenty thousand four hundred and ninty seven pregnant women were registered in the study. The perinatal mortality rate was 39 deaths/1000 deliveries, stillbirth rate 23/1000 deliveries and END rates 16/1000 live bi

Predictors of maternal and perinatal mortality among pregnant women with preeclampsia and eclampsia at Garissa County Referral Hospital: a retrospective cohort study.

BACKGROUND: Preeclampsia and eclampsia are severe complications of pregnancy that significantly contribute to maternal and fetal mortality. A comprehensive understanding of the predictors of these conditions is essential for developing effective interventions. Despite preeclampsia and eclampsia being among the leading contributors to maternal and fetal mortality in resource-limited areas in Kenya, especially the arid and semi-arid regions such as Garissa, limited studies have specifically examined the predictors of maternal and perinatal mortality among women with preeclampsia and eclampsia. Inpatient and postdischarge mortality among children with anaemia and malaria parasitaemia in Kenya: a cohort study.

BACKGROUND: Anaemia and malaria are leading causes of paediatric hospitalisation and inpatient mortality in sub-Saharan Africa. However, there is limited empirical data on survival following hospital discharge. We aimed to estimate independent effects of anaemia and malaria parasitaemia on inpatient and 1 year postdischarge mortality among Kenyan children. METHODS: A retrospective cohort study among children admitted to Kilifi County Hospital (KCH) from 2010 to 2019 and followed-up for 1 year postdischarge in Kilifi Health and Demographic Surveillance System (KHDSS). The main exposures were a

Levels and causes of maternal mortality in southern India.

Most studies of maternal mortality are hospital based. However, in developing countries, where many such deaths take place in the home, hospital statistics do not reflect the true extent of maternal mortality. Furthermore, the socioeconomic and demographic factors and health behavior affecting maternal mortality are rarely known. A study conducted in 1986 in South India demonstrates a new approach to investigating maternal mortality that combines the collection of information from hospital and health-facility records, field surveys, and case-control studies. The findings from this study indica

Less invasive surfactant administration reduces incidence of severe intraventricular haemorrage in preterms with respiratory distress syndrome: a cohort study.

OBJECTIVE: Less invasive surfactant administration (LISA) has proved to safely improve morbidity in extreme preterms with respiratory distress syndrome (RDS). Its effect regarding intraventricular hemorrhage (IVH) remains controversial between most recent systematic reviews. We aimed to evaluate its effect over incidence of severe IVH in this population. STUDY DESIGN: We compared the incidence of IVH in a prospective cohort of consecutively born preterm infants <34 weeks' gestation receiving LISA (n = 108) with a historical cohort receiving surfactant delivery via tracheal tube and managed wi

Evolution of surfactant therapy for respiratory distress syndrome: past, present, and future.

Respiratory distress syndrome (RDS) due to surfactant deficiency is the most common cause of respiratory failure in preterm infants. Tremendous progress has been made since the original description that surfactant deficiency is the major cause of RDS. Surfactant therapy has been extensively studied in preterm infants and has been shown to significantly decrease air leaks and neonatal and infant mortality. Synthetic and animal-derived surfactants from bovine as well as porcine origin have been evaluated in randomized controlled trials. Animal-derived surfactants generally result in faster weani…
