Decoding hemodialysis patients' symptoms: navigating the complexity.

BACKGROUND: Hemodialysis (HD) patients frequently experience symptoms that negatively impact their quality of life. Besides individual symptoms, cluster symptoms and core symptoms are similarly important. Identifying these symptoms helps optimize patient care. This study was conducted to assess the various symptoms experienced by HD patients, their associated dimensions, factors, connections, and relationships. METHODS: A cross-sectional study of 1156 patients undergoing HD in 27 centers across different regions in Egypt. Patients were approached through face-to-face interviews and patients'

Effectiveness of blood flow-restricted slow walking on mobility in severe multiple sclerosis: A pilot randomized trial.

OBJECTIVE: We tested the safety, feasibility, and effectiveness of blood flow restriction-empowered low-intensity interval walking exercise (BFR-W) compared with conventional intensive overground walking (CON-W) at improving gait speed and functional capacity in patients with multiple sclerosis (MS) and severe gait disabilities. METHODS: 24 patients (58 ± 5 years; 7 males) with progressive MS (Expanded Disability Status Scale 5.5 - 6.5) were randomized to receive 12 rehabilitation sessions over 6 weeks. The BFR-W group (n = 12) performed interval walking (speed paced by a metronome that incre

Functional analyses of STIM1 mutations reveal a common pathomechanism for tubular aggregate myopathy and Stormorken syndrome.

Tubular aggregate myopathy (TAM) is a progressive disorder characterized by muscle weakness, cramps, and myalgia. TAM clinically overlaps with Stormorken syndrome (STRMK), combining TAM with miosis, thrombocytopenia, hyposplenism, ichthyosis, short stature, and dyslexia. TAM and STRMK arise from gain-of-function mutations in STIM1 (stromal interaction molecule 1) or ORAI1, both encoding key regulators of Ca2+ homeostasis, and mutations in either gene result in excessive extracellular Ca2+ entry. The pathomechanistic similarities and differences between TAM and STRMK are only partially understo

The effects of limb dominance and a short term, high intensity exercise protocol on both landings of the vertical drop jump: implications for the vertical drop jump as a screening tool.

The effectiveness of vertical drop jumps (VDJs) to screen for non-contact ACL injuries is unclear. This may be contributed to by discrete point analysis, which does not evaluate patterns of movement. Also, limited research exists on the second landing of VDJs, potential lower limb performance asymmetries and the effect of fatigue. Statistical parametric mapping investigated the main effects of landing, limb dominance and a high intensity, intermittent exercise protocol (HIIP) on VDJ biomechanics. Twenty-two male athletes (21.9 ± 1.1 years, 180.5 ± 5.5 cm, 79.4 ± 7.8 kg) performed VDJs pre- and

Discrete subaortic membranes in adults--a clinicopathological analysis.

INTRODUCTION: A discrete subaortic membrane (DSM) is one of the causes of subaortic stenosis in children. The incidence, characteristics, and the therapeutic options for such membranes in adults have not been well documented. This report documents the clinical and pathological features of DSM in adults. METHODS: DSMs, surgically excised over a 10-year period in a large adult tertiary care center, were reviewed with regard to the age and gender of the patients, clinical findings, and the morphological features. RESULTS: Among the 19 adults, there were six males and 13 females, with age rangin

Polyimide cuff electrodes for peripheral nerve stimulation.

This paper describes a new tripolar spiral cuff electrode, composed of a thin (10 microm) and flexible polyimide insulating carrier and three circumneural platinum electrodes, suitable for stimulation of peripheral nerves. The cuffs were implanted around the sciatic nerve of two groups of ten rats each, one in which the polyimide ribbon was attached to a plastic connector to characterize the in vivo stimulating properties of the electrode, and one without a connector for testing possible mechanical nerve damage by means of functional and histological methods. The polyimide cuff electrodes indu

Effects of Pre- and Post-Supplementation of Taurine in the Hippocampus of a Gerbil Model of Transient Global Cerebral Ischemia.

Taurine is a free amino acid with various effects, such as developing the nervous system, an immune function, an antioxidative effect, enhancing muscle and cardiovascular function, and reducing fatigue. In this study, we investigated the effect of taurine supplementation on ischemic neuronal damage in the hippocampus of gerbils. Taurine (150 mg/kg) was orally administered to gerbils before and after induction of transient ischemia. Histologically, we examined surviving and degenerating neurons by neuronal nuclei immunostaining and fluoro-jade C (FJC) staining. Gliosis was morphologically confi

Applying Principles of Biomechanics of the Spine to Martial Arts: A Review on Balance of Stances in Goju-Ryu Karate-Do.

Balance is referred to as a state of equilibrium where forces of agonist and antagonistic muscles are equal. This is particularly relevant in the practice of Goju-Ryu Karate-Do, a martial art style with combinations of hard and soft techniques. Particularly, karate stances not only depend on technical aspects but also on the ability to achieve a centered posture. In this narrative review, we aim to integrate the existing knowledge about alignment parameters of the spine to various stances in Goju-Ryu Karate-Do to improve biomechanical understanding, allow technical modifications, and consequen

A multivariate statistical index, the fibromyalgia analog model, identifies Dahl S rats as a model of fibromyalgia with improved face validity.

Fibromyalgia syndrome (FMS) is clinically diagnosed using a widespread pain index and symptom severity scores. Despite this, multivariate statistical models using clinical data consistently suggest the presence of single overarching factor that can capture much of the variation across a spectrum of biomarkers for FMS, potentially aiding diagnosis. We hypothesized that a comparable single index could be developed for preclinical FMS rat models to quantitatively compare and rigorously test new candidates for improved translational potential and therapeutic screening. We used male and female rese

[A Case of Statin-Associated Muscle Symptoms (SAMS) Due to Drug-Drug Interaction between Statin and Pazopanib].

A woman in her 60s underwent a left nephrectomy in 2007 after being diagnosed with left renal cell carcinoma. In 2020, computed tomography (CT) revealed pancreatic metastasis, and subsequently axitinib therapy was initiated. However, axitinib therapy was discontinued due to hand-foot syndrome and nivolumab therapy was initiated in 2021. Follow-up CT scans indicated the progression of pancreatic and lymph node metastases, prompting the initiation of pazopanib therapy. Twelve days after the initiation of pazopanib therapy, she was admitted to our department with complaints of fever, fatigue, and

Cumulative Transcutaneous Spinal Stimulation with Locomotor Training Safely Improves Trunk Control in Children with Spinal Cord Injury: Pilot Study.

BACKGROUND/OBJECTIVES: Non-invasive spinal cord transcutaneous stimulation (scTS) has expanded the therapeutic landscape of spinal cord injury (SCI) rehabilitation, offering potential benefits beyond compensatory approaches to paralysis. Children with SCI are particularly susceptible to developing neuromuscular scoliosis due to trunk muscle paralysis and ongoing skeletal growth, making targeted interventions crucial. As demonstrated in adults and pediatrics with SCI, the ability of scTS to acutely and safely enable an upright posture and trunk control could be leveraged as a therapeutic adjunc

Acute Toxicity of Aqueous Extract from Bredemeyera floribunda Root Bark in an Animal Model.

The medicinal plant Bredemeyera floribunda Willd. is used to treat cardiovascular disease, chronic fatigue, low libido, as well as increased diuresis. However, studies considering the toxicity of this plant are scarce. Develop an aqueous extract of B. floribunda considering traditional use and determine the average lethality (LD50), signs, and symptoms of toxicity. The B. floribunda extract was obtained by immersing the root bark in ultrapure water for 18 hours at 4°C, under constant stirring. The test extract was administered in a single dose of 2.000 mg/kg by gavage to rats. Signs and sympto

Monkeypox: A New Challenge for Global Health System?

The COVID-19 pandemic, starting in 2020, has presented a major challenge in terms of early diagnosis and the subsequent containment and management of severe cases. The spread of viruses such as monkeypox in non-endemic countries is now creating new difficulties for healthcare professionals. Proper case definition and clinical examination are crucial for the early identification of suspected cases. For this reason, we performed a review of the literature in order to report the first signs, which are useful for healthcare providers for early case identification. Since 2022 to date, 86,930 labora
