BACKGROUND: Patients with invasive cancer identified at the time of prophylactic mastectomy (PM) will require axillary lymph node dissection for staging; therefore, many surgeons advocate sentinel lymph node (SLN) surgery at the time of PM. The current study investigates the invasive cancer rate in PM and evaluates factors associated with invasive cancer to guide SLN surgery use.
METHODS: Patients undergoing PM at the M. D. Anderson Cancer Center between January 2000 and July 2005 were identified from a prospective database. Clinical, radiographic, and pathologic data were collected.

From 1987 to 2000 108 patients were operated on for thymic tumours. Two of these underwent replacement of the superior vena cava with polytetrafluoroethylene prostheses because the tumours invaded the superior vena cava. One of these was affected by myasthenia gravis, and was treated preoperatively with concurrent radio-chemotherapy and lymphocytopheresis. The authors present the case of a 60-year-old woman with a neck lipoma that developed dumb-bell extradural extension, causing radiculopathy. To the best of the authors' knowledge, this is the first report of a lipoma originating in the neck with dumb-bell extradural extension through the intervertebral foramen and into the spinal canal. The lipoma was first excised from the foramen via a posterior approach to allow decompression of the nerve roots. The remaining lipomatous tissue was then resected via an anterior approach to avoid the region around the vertebral artery.

We report a rare case of pilocytic astrocytoma and diencephalic syndrome occurring together in a 30-year-old woman with neurofibromatosis type 1 (NF-1). Diencephalic syndrome included emaciation and somnolence. Gadolinium-enhanced magnetic resonance imaging revealed a brain tumor in the optic chiasmal-hypothalamic region, which invaded the thalamus, brain stem and cerebellum. A biopsy specimen from the chiasmal tumor was compatible with pilocytic astrocytoma. Although she received radiation therapy, she died 22 months after the onset of the disease.

A previously well 16-month-old boy presented to Sydney Children's Hospital with generalised seizures. There was no history of neurofibromatosis and no family history. Metastatic spread to the ipsilateral adrenal gland occurs in 1.2-10% of patients with renal cell carcinoma (RCC). We report two cases in which the MRI appearances mimicked the dural tail sign; a glioma extending into the subarachnoid space, and a meningioma extending to the subdural space. They indicate that tumour invasion into the subarachnoid or subdural space, should be considered when prominent linear enhancement is observed along the dura mater adjacent to tumours.

This article is based on findings in 180 patients with cancer of the larynx and hypopharynx. Positive margins were found in 12.2% (22/180) of all tumors, 12.5% (8/64) of supraglottic tumors, 16% (4/25) of transglottic tumors, 5.3% (2/38) of glottic tumors, 11.1% (2/18) of pyriform fossa tumors, 50% (3/6) of posterior pharyngeal wall tumors, and 10.6% (3/29) of postcricoid tumors. The relationship between the inadequate surgical margins of the specimens, tumor spread, preoperative CT findings, and surgical procedures performed have been correlated.

Pulmonary blastoma is a rare primary lung tumour associated with poor prognosis. Despite modern diagnostic imaging and biopsy techniques, the diagnosis is usually not considered initially. A case of a 22-year-old man with a large biphasic left-sided pulmonary blastoma, extending to the mediastinum, is reported. It was treated by pneumonectomy and pericardiectomy with radical tumour resection from the anterior mediastinum, under cardiopulmonary bypass. Adjuvant radiotherapy was used. 3 years postoperatively there is no sign of recurring disease.

Eccrine porocarcinoma is a rare malignant tumor of the true sweat gland. It commonly presents in the lower extremities with lymphatic metastasis.
