ESR Essentials: uterine cancers-practice recommendations by the European Society of Urogenital Radiology.

Uterine cancers include endometrial cancer (EC), cervical cancer (CC), and uterine sarcomas, with leiomyosarcoma being the most common subtype. Each tumour type has distinct biological behaviour, imaging features, and treatment approaches. Radiologists must be familiar with normal uterine anatomy and its variations, as well as key imaging characteristics and common patterns of tumour spread to ensure accurate staging and optimal treatment selection. MRI is the imaging modality of choice for evaluating uterine malignancies due to its excellent soft tissue contrast and multiplanar capabilities.

What's next for VI-RADS? Updates and future perspectives from the ACR VI-RADS steering committee.

Since the introduction of the Vesical Imaging-Reporting and Data System (VI-RADS), MRI has become an important imaging modality in the management of patients with bladder cancer. Its excellent diagnostic performance for determining muscle invasion in bladder cancer has been supported by numerous prospective and retrospective studies. Nevertheless, there needs to be continued improvement of the diagnostic performance of VI-RADS and sustained efforts to address remaining unmet clinical needs within the field of bladder cancer. In this paper, we highlight several such areas, some of which are act

Gastric Cancer Peritoneal Staging: Progress and Persistent Challenges.

Staging laparoscopy with peritoneal lavage has become a cornerstone of gastric cancer staging, particularly for identifying occult peritoneal disease. Recent Dutch and European Delphi consensus studies have now established broad agreement on indications for staging laparoscopy, systematic peritoneal inspection, and technical aspects of peritoneal lavage. However, while consensus on how to perform the procedure has advanced substantially, there remains striking heterogeneity in how peritoneal lavage cytology specimens are handled and interpreted by pathology laboratories. A recent multicenter J

Micrometastasis and Isolated Tumor Cells in Oral Squamous Cell Carcinoma: Refining Nodal Staging with Emerging Technologies.

PURPOSE: Cervical lymph node metastasis significantly influence prognosis in oral squamous cell carcinoma (OSCC), guiding staging, treatment decisions, and overall survival. Sentinel lymph node biopsy (SLNB) offers a minimally invasive approach for early detection of subclinical nodal metastasis, including micrometastases (0.2-2 mm) and isolated tumor cells (ITCs, < 0.2 mm). Despite its success in melanoma and breast cancer, the clinical relevance of micrometastases and ITCs in OSCC remains incompletely defined. This narrative review explores the biological significance, diagnostic challenges,

The Usefulness of the Modified Advanced Lung Cancer Inflammation Index (mALI) as a Prognostic Factor in Patients With Esophageal Cancer who Have Undergone Curative Resection.

BACKGROUND/AIM: In several previous studies, the modified advanced lung cancer inflammation index (mALI) has been reported to be a useful prognostic factor in non-small cell lung cancer. This study aimed to demonstrate the relationship between mALI and the oncological prognosis of patients with esophageal cancer (EC) who underwent curative treatment and to clarify the underlying mechanism. PATIENTS AND METHODS: We retrospectively followed up patients who underwent curative resection of esophageal cancer at Yokohama City University between 2000 and 2020 and extracted clinical data from their m

Genomics in Assessing Melanoma Prognosis.

Genomic advancements have transformed melanoma prognosis by identifying key genetic alterations that influence disease progression and treatment outcomes. Gene expression profiling (GEP) tests, including the 31-GEP, 11-GEP, and 8-GEP + CP, refine traditional staging by stratifying patients based on recurrence and metastasis risk. These tests enhance clinical decision-making by guiding sentinel lymph node biopsy selection, surveillance intensity, and adjuvant therapy use. Studies confirm their prognostic accuracy, linking GEP results to survival outcomes. Despite their potential, challenges lik

[Cardiac involvement in stage IV lung cancer. The value of an integrated diagnosis through a multidisciplinary approach].

Stage IV lung cancers are often characterized by widespread systemic metastases, including pleural and cardiac sites. We report the case of a 67-year-old man admitted for suspected advanced lung cancer with bilateral pleural and pericardial effusion. Echocardiography revealed an intracardiac mass with heterogeneous echogenicity (hypo/isoechoic) adherent to the anterior wall of the right ventricle, associated with mild impairment of right ventricular systolic function and a mild-to-moderate pericardial effusion without clinical signs of tamponade. Whole-body positron emission tomography (PET) c

De Novo bone metastasis in breast cancer: tumor biology and survival outcomes in a retrospective study from Pakistan.

PURPOSE: Breast cancer is the leading cause of cancer-related deaths among women, with 20-30% of early-stage patients eventually developing metastases, often in bones. Despite the high incidence, risk factors for de novo bone metastasis are understudied in local populations. METHODS: This single-center, retrospective study was conducted at Aga Khan University Hospital, Karachi. Patient demographics, tumor characteristics, and risk factors were assessed. Patients with de novo bone metastasis were compared to non-stage IV cases using binary univariate and multivariate logistic regression, with

Second opinions and pathological review impact the clinical management of patients with muscle-invasive bladder cancer.

PURPOSE: Studies evaluating inter-institutional discrepancies in muscle-invasive bladder cancer diagnosis, particularly when radical cystectomy is recommended, are scarce. In this study, we aimed to examine the inter-institutional concordance rate of pathological stages in patients diagnosed with muscle-invasive bladder cancer. METHODS: We reviewed tissue samples from patients pathologically diagnosed with muscle-invasive bladder cancer at other hospitals who subsequently sought a second opinion at our hospital between January 1, 2013 and December 31, 2023. Data were acquired retrospectively

Treatment of node-positive endometrial cancer: chemotherapy, radiation, immunotherapy, and targeted therapy.

The standard of treatment for node-positive endometrial cancer (FIGO Stage IIIC) in North America has been systemic therapy with or without additional external beam radiation therapy (RT) given as pelvic or extended field RT. However, this treatment paradigm is rapidly evolving with improvements in systemic chemotherapy, the emergence of targeted therapies, and improved molecular characterization of these tumors. The biggest question facing providers regarding management of stage IIIC endometrial cancer at this time is: what is the best management strategy to use with regard to combinations of
