Reopening a Student-Run Free Clinic During the COVID-19 Pandemic to Provide Care for People Experiencing Homelessness.

PROBLEM: Student-run free clinics (SRFCs) provide health and outreach services to underserved populations while offering medical students opportunities for service and education. Many SRFCs were forced to suspend in-person operations in early 2020 due to the COVID-19 pandemic. Before March 2020, JeffHOPE, the SRFC at Sidney Kimmel Medical College at Thomas Jefferson University, operated an evening clinic at 5 locations throughout Philadelphia each week. APPROACH: JeffHOPE's response to challenges posed by COVID-19 focused on a redesign for a pilot clinic at a shelter site that expressed inter

Fibromyalgia and the concept of resilience.

The concept of resilience varies according to the context in which it is used. Resilience is broadly defined as a protective factor that makes people less vulnerable to future adverse life events, in this implying the previous occurrence of an adverse event that has to be confronted before individual equilibrium can be restored. This definition can be applied to fibromyalgia and other chronic pain situations. Resilience is profoundly related to reaction to acute or chronic stress, and is therefore involved in the stress response system. Corticotropin-releasing factor can be considered a fundam

Atopy and development of cancer: a population-based prospective study.

BACKGROUND: Atopy is the familial or personal propensity to develop IgE antibodies against environmental allergens. Atopy, theoretically, could both prevent and promote the development of cancer. However, evidence from epidemiologic studies has been inconclusive. OBJECTIVE: We investigated the longitudinal association between atopy and the incidence of total and specific types of cancers of 5 Danish population-based studies. METHODS: Atopy was defined as serum specific IgE positivity against inhalant allergens. A total of 14,849 persons were followed up prospectively by linkage to the Danish

[Anorexia, the passage from adolescence to adulthood].

Anorexia engages all those close to the patient in accepting and following the treatment process. Indeed, the continuity of the treatment depends on the quality of the connections which the person with anorexia develops with his or her parents or partner. The caregivers must be involved and understand what is at play notably with the passage from adolescence to adulthood.

Deep Learning Blood Dosimetry Predicts Severe Lymphopenia and Survival After Craniospinal Irradiation.

PURPOSE: Craniospinal irradiation (CSI) is a standard treatment for pediatric brain tumors and is increasingly used for leptomeningeal metastases. Although effective, CSI often causes radiation-induced severe lymphopenia (RISL), which is associated with poor outcomes across various cancer types. We developed and validated a novel platform for personalized dosimetry of circulating blood cells (CBCs) and assessed its predictive value for RISL. METHODS AND MATERIALS: We combined a prospective pediatric CSI registry (initiated April 2018; n = 48) and retrospectively collected patients (January 20

Developing an implementation guide for robotic pets in continuing care settings for older adults living with dementia in Canada: findings from a modified Delphi.

BACKGROUND: While live animal therapy has been demonstrated to improve the well-being of persons living with dementia, no-pet policies frequently exclude live pets in continuing care settings due to the potential for biting, scratching, and allergies to occur. Robotic pets as an alternative tool for supporting the well-being of persons living with dementia in continuing care is increasing in prevalence due to its therapeutic value. While there are benefits to using robotic pets, implementation challenges and unintended consequences exist. An evidence-based implementation guide is needed to sup

Barriers at various levels of human ecosystem for maintaining adherence to gluten free diet in adult patients with celiac disease.

INTRODUCTION: While lifelong and strict adherence to gluten-free diet (GFD) is essential for the successful treatment of celiac disease (CeD), only 30-50% of patients are able to maintain a good adherence to GFD. We determined factors influencing the adherence to GFD at various ecological levels including intra-personal, inter-personal, organizational, community and system-based levels in adult patients with CeD. METHODS: A questionnaire to assess the adherence was developed and it was administered in the CeD clinic to patients with CeD on GFD for >1 year. Adherence to GFD was assessed in a s

Developing and Testing a Web-Based Provider Training for Cognitive Behavioral Therapy of Insomnia.

INTRODUCTION: Chronic insomnia is a common and debilitating disease that increases risk for significant morbidity and workplace difficulties. Cognitive behavioral therapy for insomnia (CBT-I) is the first-line treatment, but there is a critical lack of behavioral health providers trained in CBT-I because, in part, of a bottleneck in training availability and costs. The current project developed and evaluated a web-based provider training course for CBT-I: CBTIweb.org. MATERIALS AND METHODS: Subject matter experts developed the content for CBTIweb.org. Then, trainees completed alpha testing (n

Demand for Space: Elderly Transgender and Gender Nonconforming People, Healthcare, and Theological Ethics.

Visibility for transgender and gender nonconforming people and the elderly is growing; however, thus far the overlap of the two groups has rarely been considered. Trans persons therefore remain largely invisible in the context of older people's care and medicine. The discrimination faced by this group is at least twofold: they are the targets of aggression incited by transphobia, and also by ageism. Although older trans and gender nonconforming people exist as a greatly marginalized group within another already marginalized group, even the field of theological ethics has neglected to grant the

Implementing recovery-oriented interventions with staff in a psychiatric hospital: A mixed-methods study.

WHAT IS KNOWN ON THE SUBJECT?: Recovery-oriented care is a means of providing mental health treatment, focused on the patient's individual needs and active involvement in one's own care. However, this approach presents with challenges, particularly in psychiatric hospitals, which tend to be focused on symptom reduction. WHAT DOES THE PAPER ADDS TO EXISTING KNOWLEDGE?: This study examines the influence of three different recovery-oriented training programmes/interventions (namely, illness management and recovery, peer support, and psychiatric advance directives) on the attitudes and practice of

Development of Therapeutic Alliance and Social Presence in a Digital Intervention for Pediatric Concussion: Qualitative Exploratory Study.

BACKGROUND: Despite the promising benefits of self-guided digital interventions for adolescents recovering from concussion, attrition rates for such interventions are high. Evidence suggests that adults can develop therapeutic alliance with self-guided digital interventions, which is in turn associated with intervention engagement. However, no research has examined whether adolescents develop therapeutic alliance with self-guided digital interventions and what factors are important to its development. Additionally, social presence-the extent to which digital encounters feel like they are occur
