Clinical feasibility and surgical benefits of video-assisted mediastinoscopic lymphadenectomy in the treatment of resectable lung cancer.

OBJECTIVE: This study was performed to assess the clinical feasibility and surgical outcomes of video-assisted mediastinoscopic lymphadenectomy in the treatment of resectable lung cancer. METHODS: Between July 2004 and December 2009, we retrospectively analyzed 108 consecutive video-assisted mediastinoscopic lymphadenectomies in lung cancer patients from a prospectively collected database. Ninety-seven (89.8%) patients underwent combined operation during the same anesthesia and six (5.3%) patients underwent a staged operation for the resection of lung cancer and systematic lymphadenectomy. We

Unplanned conversion to thoracotomy during video-assisted thoracic surgery lobectomy does not compromise the surgical outcome.

BACKGROUND: Concerns remain unresolved regarding the safety of unplanned conversion to open thoracotomy during video-assisted thoracic surgery (VATS) lobectomy. We analyzed both early and late outcomes after thoracotomy conversion from VATS. METHODS: From December 2003 to December 2008, a total of 738 VATS lobectomies were attempted. Among them were 34 unplanned conversions to open thoracotomy. Patient characteristics, operative data, and early and late postoperative outcomes were analyzed retrospectively. RESULTS: Among the 34 conversion cases, 26 patients had lung cancer and 8 had benign l

Bilobectomy for lung cancer: analysis of indications, postoperative results, and long-term outcomes.

BACKGROUND: Bilobectomy for lung cancer is considered a high-risk procedure for the increased postoperative complication rate and the negative impact on survival. We analyzed the safety and the oncologic results of this procedure. METHODS: We retrospectively reviewed patients who underwent bilobectomy for lung cancer between October 1998 and August 2009. Age, gender, bilobectomy type and indication, complications, pathology, stage, and survival were analyzed. RESULTS: Bilobectomy was performed on 146 patients (101 men; mean age, 62 years). There were 77 upper-middle and 69 middle-lower bilob

Safety and utility of mediastinoscopy in non-small cell lung cancer in a complex mediastinum.

BACKGROUND: Adequate mediastinal staging is crucial in patients with locally advanced non-small cell lung cancer. Mediastinoscopy is often omitted after induction therapy or prior mediastinoscopy because of concerns for potential morbidity, safety and unknown utility. We sought to determine the safety and utility of restaging mediastinoscopy before surgical resection. METHODS: A retrospective review was made of non-small cell lung cancer patients who underwent mediastinoscopy with a complex mediastinum, defined as any or all of the following: previous mediastinoscopy or induction chemotherapy

Sentinel node navigation surgery by thoracoscopic fluorescence imaging system and molecular examination in non-small cell lung cancer.

PURPOSE: The application of sentinel node navigation surgery in non-small cell lung cancer (NSCLC) is not popular because of the difficulty of sentinel node identification and the low incidence of complications after systemic lymph node dissection. We report the intraoperative assessment of sentinel node metastasis by thoracoscopic ICG fluorescence imaging system and real-time reverse transcription-polymerase chain reaction (RT-PCR). METHODS: Sixty-one patients who underwent surgery between January 2009 and December 2010 were investigated for sentinel node biopsy. ICG fluorescence imaging was

Transmanubrial osteomuscular sparing approach for lung cancer invading the anterior part of the thoracic inlet.

Standard treatment for lung cancer presenting as a superior sulcus tumor is induction chemoradiotherapy followed by surgery, which yields rates of about 70% complete resection and 50% 5-year survival rate. However, the surgical technique to achieve complete resection for superior sulcus tumor invading major anatomical sites including the subclavian artery is challenging. The anterior transcervical thoracic approach applied by Dartevelle and colleagues provides excellent exposure of the subclavian vessels. Grunenwald and associates have improved on this approach to preserve the clavicle and ste

Squamous cell carcinoma presenting as a solitary growing cyst in lung: a diagnostic pitfall in daily clinical practice.

Gradually growing cystic lesion of the lung is commonly encountered in daily clinical practice. Thin-walled cavitary lung cancer is a rare entity; however, it could be a pitfall in the diagnosis of such radiographically benign-looking lesions, especially without an obvious solid, nodular, or tumorous appearance in the lesion. We herein report a rare case of lung cancer successfully treated by surgical resection that appeared as a gradually growing cystic lesion mimicking benign emphysematous disease, with a review of the literature. A 68-year-old man with a 24-year history of hypothyroidism pr

[Persistent cough and bronchiectasis].

A 68 yr. old lady suffered from chronic cough. Bronchiectasis was diagnosed and treated with physiotherapy. Coughing persisted over years, although an asthma disease was controlled by inhalation, a carcinoid tumor was surgically removed, and a coronary artery disease was treated by revasculating surgery. Antibiotics and systemic corticosteroids did not relieve symptoms either. After all, diagnosis and treatment of obstructive sleep apnea led to symptom free life. We assume micro-aspiration during periods of apnea being the cause of the coughing.

Radical operation for invasive thymoma with intracaval, intracardiac, and lung invasion.

We report a rare case of invasive type AB thymoma (WHO classification system) with intracaval and intracardiac extension into the right atrium. A cardiopulmonary bypass facilitated an en bloc excision of the tumor with a lobectomy, along with a combined resection of the right atrium and reconstruction of the bilateral brachiocephalic veins and right atrium. Aggressive surgical intervention should be considered in such cases, as long-term prognosis is primarily dependent on a complete resection of the extended thymoma.

[A case of a second cancer of metachronous multiple primary non-small cell lung cancer successfully treated with TS-1 and CDDP chemotherapy].

The patient was a 66-year-old man who had undergone right upper lobectomy and ND 2a systematic lymph node dissection for lung cancer (M/D adenocarcinoma, p-stage IB) in March of 1999 . On November 2003, postoperative routine chest computed tomography(CT) demonstrated a mass in left S6, and pathological diagnosis revealed P/D squamous cell carcinoma (cT1N2M0, stage IIIA) by CT-guided needle biopsy and mediastinoscopy. At first, we tried two courses of a combination chemotherapy consisting of carboplatin (CBDCA) and paclitaxel every 3 weeks. After 2 courses, the regimen was stopped because of gr

Clinicopathologic study of resected, peripheral, small-sized, non-small cell lung cancer tumors of 2 cm or less in diameter: pleural invasion and increase of serum carcinoembryonic antigen level as predictors of nodal involvement.

OBJECTIVE: The number of surgical interventions for small-sized lung cancer has increased with the development of computed tomography. We attempted to identify clinicopathologic characteristics of peripheral, small-sized, non-small cell lung cancer to show the limitation of partial resection or segmentectomy. METHODS: A retrospective analysis of 143 patients who underwent a complete resection for a peripheral non-small cell lung cancer of 2 cm or less in diameter was performed. The relationships between nodal involvement and other clinical factors were also assessed in patients who underwent

Nodal downstaging predicts survival following induction chemotherapy for stage IIIA (N2) non-small cell lung cancer in CALGB protocol #8935.

BACKGROUND AND OBJECTIVES: CALGB 8935 was a phase II protocol for mediastinoscopically staged IIIA (N2) non-small cell lung cancer. Induction cisplatin/vinblastine chemotherapy was followed by surgical resection, adjuvant cisplatin/vinblastine, and radiotherapy. We now evaluate the prognosis of pathologic nodes. METHODS: Failure-free survival was calculated from a landmark 3 months after resection to account for heterogeneity in adjuvant therapy. RESULTS: Nine of 42 (21%) resected patients had no residual N2 disease. This subset of 9 had a median failure-free interval of 47.8 months from lan

The coincidence of mediastinal tuberculosis lymphadenitis in lung cancer patients.

Mediastinal lymph node enlargement in operable non-small cell lung cancer is of clinical importance since it indicates the high possibility of nodal metastasis. The coincidence of tuberculosis and lung cancer is detected by the mediastinal lymph node staging of lung cancer patients. In our study, we retrospectively re-evaluated the records of patients who had been hospitalized with the diagnosis of lung cancer for the past 10 years. The mean age was 58 +/- 10 years (ranging from 39 to 72). A tuberculous lymphadenitis was detected in 16 of the 315 (5.1%) patients in one and/or multiple stations

Congenital cystic lung lesions: is surgical resection essential?

Can all congenital cystic lung lesions be treated conservatively, without the need for surgery? Our purpose here is to present the morbidity associated with symptomatic cystic lung lesions which have failed to respond to medical treatment. In the past 8 years, 22 consecutive cystic thoracic lesions were retrospectively assessed for clinical presentation, diagnostic modalities, operative findings, technical tribulations, and outcome. The endpoint was complete cessation of recurrent pneumonia and dysphagia. Age at presentation was 7.7 +/- 2.2 years, with 4 +/- 2 episodes per year of lower respir

Dramatic improvement of respiratory condition after lobectomy for localized bullous emphysema.

A case of dramatic improvement of respiratory function after lobectomy for bullous emphysema with severe hyperinflation of the left lower lobe was reported. A 72-year-old gentleman was admitted to our hospital due to increasing dyspnea with Hugh-Johns class V. His chest X-ray and computed tomography revealed a hyperinflation of the left lower lobe. Despite medication and respiratory rehabilitation, blood gas analysis showed hypercapnia and his symptoms had been progressing. Bronchofiberscopy revealed that the left lower lobe bronchi opened on inspiration and closed on expiration. Since complet

Clinical predictors of pathological response after immunotherapy in salvage surgery for non-small cell lung cancer: a supplementary analysis of the CReGYT-05 immune checkpoint inhibitor-salvage study.

OBJECTIVES: Radiological responses based on tumor size reduction are often unreliable after immune checkpoint inhibitor (ICI) therapy for non-small cell lung cancer (NSCLC), particularly in salvage surgery. The aim of this study was to evaluate the association between MPR and prognosis, and to determine whether changes in metabolic response on positron emission tomography/computed tomography (PET/CT) and tumor marker levels can identify patients who achieve a major pathological response (MPR) despite residual disease. METHODS: This multicenter retrospective study (CReGYT-05 ICI-Salvage study)
