The medial longitudinal fasciculus (MLF) is a paired, highly specialized, and heavily myelinated nerve bundle responsible for extraocular muscle movements, including the oculomotor reflex, saccadic eye movements an smooth pursuit, and the vestibular ocular reflex. Clinically, lesions of the MLF are classically associated with internuclear ophthalmoplegia. However, clinical manifestations of a lesion in the MLF may be more complex and variable. We provide an overview of the neuroanatomy, neurologic manifestations, and correlative examples of the imaging findings on brain MRI of MLF lesions to p…

Parkinsonism or dystonia are associated with myoclonus in several extrapyramidal diseases. Although the latter symptom is not always prominent, stimulus-sensitive, distal, or focal reflex myoclonus is frequently observed. This review will consider the clinical and electrophysiological features of myoclonus in Parkinson's disease, multiple system atrophy, Lewy body dementia, corticobasal degeneration, progressive supranuclear palsy, Huntington's disease, dentatorubral-pallidoluysian atrophy and myoclonus with dystonia. The evidence of a long-latency reflex response, the presence of giant somato…

The ectoparasiticide amitraz stimulates alpha 2-adrenoceptors to produce side-effects such as bradycardia and hypotension. The actions of amitraz on baroreceptor reflex responses were evaluated in mongrel dogs by occlusion of both carotid arteries for 30-s periods. Incremental doses of amitraz given intravenously showed that doses of 60 micrograms/kg and above significantly depressed pressor responses to carotid occlusion. By comparison, 2 micrograms/kg amitraz given by intracisterna magna (i.c.m.) injection significantly depressed both blood pressure and heart rate responses. Pretreatment of …

Needle phobia is a recently defined medical condition that affects at least 10% of the population. Because persons with needle phobia typically avoid medical care, this condition is a significant impediment in the health care system. The etiology of needle phobia lies in an inherited vasovagal reflex of shock, triggered by needle puncture. Those who inherit this reflex often learn to fear needles through successive needle exposure. Needle phobia is therefore both inherited and learned. In a family practice, needle phobia can be managed by reassurance and education, avoidance of needles, postur…

Evoked sacral potentials were used to study the bulbo-cavernous reflex in 85 patients with true premature ejaculation (TPE) and in 52 subjects as a control group. In the perineal and perianal recordings the amplitudes of the evoked responses (means +/- SD) were 70 +/- 91.1 microV and 35.6 +/- 36 microV in the TPE patients, and 39.2 +/- 36.8 microV and 26 +/- 21.6 microV in the controls, respectively. Both these differences resulted significant (P less than 0.01 and P congruent to 0.05, respectively). Our results suggest a reflex hyperexcitability, or an impaired "modulation" of the motor neuro…

Separate age groups of C57BL/6 and autoimmune New Zealand Black (NZB) mice were compared for diazepam-induced ataxia and barbiturate-induced loss of righting reflex. Between 1 and 3 months of age, both strains showed a similar age-related decrease in ED50 for diazepam-induced ataxia. However, between 3 and 12 months the decrease in ED50 was markedly greater in NZB mice. In contrast, age-related increases in the durations of loss of righting reflex following hexobarbital or barbital were similar in both strains. The results suggest that NZB mice show relatively accelerated age-related increases…

It the sciatic nerve of a rat is crushed in the thigh, axons from the proximal side of the crush will regenerate so that the toe-spreading reflex becomes observable again after 10.4 +/- 1.7 (mean +/- S.D.) days. If the nerve is electrically stimulated for 0.25-1.0 h at the crush site, just after the crush occurs, the toe-spreading reflex first becomes observable 4.14 +/- 1.6 (mean +/- S.D.) days after the crush. Stimulation is most effective if delivered immediately after the crush but can be delayed up to an hour and still cause significantly faster regeneration. This phenomenon could be usef…

The bulbocavernosus reflex may be evoked electrically with a latency of approximately 35 msec. in normal male subjects. We have studied the sacral-evoked response in 116 individuals, 10 of whom were normal controls. For each study ipsilateral and contralateral responses were obtained from each side, resulting in 4 latency values. We herein describe the abnormalities in 54 patients with a variety of documented neurologic lesions. Our investigation reveals that the bulbocavernosus reflex has the characteristics of a cutaneous polysynaptic bilateral flexor response. The use of bilateral stimulati…

Balloon-induced distention of the main pulmonary artery causes acute pulmonary hypertension and reflex pulmonary vasoconstriction in animals. Pulmonary artery pressure responses caused by MPA balloon inflation were measured in ten human newborn infants with cardiac failure (n = 5) or persistent fetal circulation (n = 5). During balloon inflation distal mean PAP increased significantly while cardiac rate remained unchanged. MPA distention caused greater increases of PAP in those infants with lower resting PAP. The greatest balloon-induced increases of PAP were observed in infants recovering fro…

The perfused working heart brainstem preparation of rodents has become a widely used tool to study brainstem function. Here, we adapt this experimental technique for newborn guinea pigs (postnatal day 7-14) to develop a tool that enables investigation of airway defense mechanisms not observed in other rodents. The perfused guinea pig brainstem preparation generates a stable eupnea-like motor pattern recorded from the phrenic, recurrent laryngeal and intercostal nerves and basic cardio-respiratory reflexes, including the arterial chemoreceptor, the baroreceptor reflex. In addition a fictive lar…

OBJECTIVE: Paradoxical vocal cord motion (PVCM) is a well recognized respiratory condition in which active adduction of the vocal cords during inspiration causes functional airway obstruction. It is considered that laryngeal reflex acceleration underlies the generation of nonorganic PVCM. In various situations producing PVCM, multiple system atrophy (MSA) is a representative neurological disease causing nocturnal laryngeal stridor attributed to PVCM. The purpose of this review is to identify the underlying mechanisms associated with nonorganic and MSA-related PVCM. The following issues are add…

The neurophysiological findings in eight patients with the stiff man syndrome (SMS), including four of six tested with autoantibodies against glutamic acid decarboxylase, are presented. Neurophysiological findings did not make it possible to discriminate between patients with and those without autoimmunity against GABAergic neurons. Investigation of mono- and polysynaptic reflexes revealed abnormal results in a variable number of SMS patients, the abnormalities largely corresponding to those seen in spastic paresis. A stereotyped motor response to electrical stimulation of peripheral nerves wa…

OBJECTIVE: Late motor responses, such as F-waves and H-reflexes, may reflect excitability of specific spinal cord segments and motor pathway integrity, offering potential value during intraoperative neurophysiological monitoring (IONM). However, these responses are not routinely utilized. A better understanding of the effect of general anaesthesia on their characteristics, particularly across varying depths, is essential for integration into IONM.
METHODS: Participants received propofol/remifentanil-based anaesthesia, targeted at BIS values of 30, 40, and 50. F-waves, H-reflexes and Tc-mMEPs …

Autonomic reflexes, controlling unconscious processes such as heart rate and digestion, are essential for homeostasis. Vago-vagal reflexes describe one category of autonomic reflexes in which the sensory input and motor output are both mediated through the vagus nerve. These reflexes are widely considered to rely on the nucleus of the solitary tract (NTS) for the integration of sensory information before regulating the motor output from the dorsal motor nucleus of the vagus (DMV). Here, we electrically activated the solitary tract to evoke sensory input simultaneous to whole cell recordings of…

BACKGROUND: Pulmonary oxygen toxicity is very well known and proved. The influence of hyperoxia on the respiratory reflexes is not known till now.
MAIN PURPOSE: To ascertain if long-lasting breathing of pure normobaric oxygen (PNO) alter respiratory reflexes.
METHODS: 34 adult cats of both sexes, weighing 2.5-4.0 kg, were used in two experiments. In the first experiment 16 animals with inserted chronic tracheal cannula (CTC) were used. In the second experiment 18 animals without CTC were employed. Part of animals in both experiments was exposed to PNO (day by day for 2 weeks, 10 h daily), re…

A case is presented of a patient with lateral pontine stroke which resulted in a moderate unilateral sensorineural hearing loss. ABR, word recognition scores, acoustic reflexes, neurological evaluation, and CT scan were consistent with brain stem lesion. Recovery of hearing, including acoustic reflexes and word recognition ability, occurred 2 months after the stroke.

BACKGROUND: According to Italian law, brain death is diagnosed when the patient is in a coma, showing the absence of respiratory drive under specific clinical conditions, and without any brain stem reflexes. On the other hand, presence of spinal reflexes, when correctly identified, does not hamper the diagnosis.
CASE REPORT: We present a case of eyelid elevation two seconds after thoracic pain stimulation in a patient who otherwise fulfilled all clinical and instrumental brain-death criteria due to a residual preserved function of the superior cervical ganglion.
CONCLUSION: Although the obse…

Rasmussen syndrome is a rare, inflammatory and probably autoimmune disease presenting with epilepsia partialis continua which is generally in the form of myoclonic jerks and involves the upper extremities with or without head involvement. We sought to demonstrate the electrophysiological features in patients with Rasmussen syndrome. We performed continuous electrophysiological recordings of involuntary movement, as well as recordings of startle responses and long latency reflex in three patients with a diagnosis of Rasmussen syndrome. Positive and negative myoclonus were recorded. Startle resp…

The otolith organs consist of the utricle and saccule. The utricle mediates the utriculo-ocular reflex by detecting horizontal head translation and static head tilt. Skew deviation is a vertical strabismus caused by imbalance of the utriculo-ocular reflex pathway and is commonly caused by lesions in the brainstem or cerebellum. It is associated with abnormal utriculo-ocular reflexes including asymmetric reduction of the translational vestibulo-ocular and ocular counterroll responses. Skew deviation is also associated with head position-dependent changes in ocular torsion and vertical strabismu…
