Surgical versus medical interventions in CRS and nasal polyps: comparative evidence between medical and surgical efficacy.

CRS with nasal polyps (CRSwNP) is a subgroup of CRS, where polyps can be visualised in the middle meatus. In the general population, the prevalence is estimated to lie between 0.2 and 4 %. Up to 15 % of patients with asthma have nasal polyps and up to 45 % of patients with nasal polyps have asthma. The management of CRSwNP involves primary medical management, with surgery normally reserved for recalcitrant cases. Surgical techniques for CRSwNP range from simple polypectomy and endoscopic sinus surgery (ESS) to radical nasalisation with removal of the middle turbinates. We have reviewed the com

[Definition of inflammatory subtypes of chronic rhinosinusitis with nasal polyp and asthma].

Chronic rhinosinusitis with nasal polyps (CRSwNP) and asthma is a common clinical refractory airway disease. Comprehensive treatment of nasal endoscopic surgery including nasal endoscopic surgery and medication, which can significantly improve nose-pulmonary symptoms and make sinusitis and asthma easier to be controlled by medication, has certain superiority. But the existence of disease heterogeneity of CRSwNP with asthma causes different reactions to the current treatment, which manifests as parts of polyps and asthma easy to recur and difficult to control. According to the research recently

A review of non-pharmacological and pharmacological management of seasonal and perennial allergic conjunctivitis.

Allergic eye disease encompasses a group of hypersensitivity disorders which primarily affect the conjunctiva and its prevalence is increasing. It is estimated to affect 8% of patients attending optometric practice but is poorly managed and rarely involves ophthalmic assessment. Seasonal allergic conjunctivitis (SAC) is the most common form of allergic eye disease (90%), followed by perennial allergic conjunctivitis (PAC; 5%). Both are type 1 IgE mediated hypersensitivity reactions where mast cells play an important role in pathophysiology. The signs and symptoms are similar but SAC occurs per

Extended endoscopic techniques for sinonasal resections.

The evolution of endoscopic sinus surgery has led to a paradigm shift in the management of sinonasal and anterior skull base tumors in the past decade. Endoscopic resection is considered by many institutions to be the gold standard approach even for extensive pathology. Endoscopic tumor surgery should not imply less surgery but rather an alternative to external operations providing the same access and enabling equivalent or superior visualization for resection of tumors. It also avoids much of the potentially significant morbidity associated with external operations. Successful endoscopic tumo

T cells in chronic rhinosinusitis with nasal polyposis.

PURPOSE OF REVIEW: The cause of nasal polyp disease remains controversial. Examination of the T lymphocytes involved in nasal polyp inflammation may lead to an improved understanding of the cause, prognosis, and treatment of chronic rhinosinusitis with nasal polyposis. RECENT FINDINGS: T lymphocytes are important directors of the inflammatory process in allergic rhinitis and asthma, but the role of T lymphocytes in chronic rhinosinusitis with nasal polyposis has not been thoroughly investigated. The T lymphocyte infiltrate in nasal polyps may vary based upon genetic factors, polyp histology, …

[A-mode ultrasonography and roentgenography in diagnosing chronic nonpolypoid maxillary rhinosinusitis].

OBJECTIVES: The aim is to demonstrate diagnostic alternatives for A-mode ultrasonography in examining chronic nonpolypoid rhinosinusitis, its implementation, as well as its advantages and disadvantages in regards to roentgenography. METHOD: The prospective research conducted included patients with chronic nonpolypoid rhinosinusitis, at 79 maxillary sinuses. Chronic diseases are classified into two separate categories depending on the clinical entity. Comparative analysis of the ultrasound and roentgenography findings was conducted for both, while the status of the maxillary sinuses obtained b

Fungal rhinosinusitis: a categorization and definitional schema addressing current controversies.

BACKGROUND: Fungal (rhino-) sinusitis encompasses a wide spectrum of immune and pathological responses, including invasive, chronic, granulomatous, and allergic disease. However, consensus on terminology, pathogenesis, and optimal management is lacking. The International Society for Human and Animal Mycology convened a working group to attempt consensus on terminology and disease classification. DISCUSSION: Key conclusions reached were: rhinosinusitis is preferred to sinusitis; acute invasive fungal rhinosinusitis is preferred to fulminant, or necrotizing and should refer to disease of <4 wee

Increased expression of acidic mammalian chitinase in chronic rhinosinusitis with nasal polyps.

BACKGROUND: Chitin is an abundant polysaccharide found in fungi, insects, and parasitic nematodes. Innate immune host defense against chitin-containing pathogens include production of chitinases. In human lower airways, acidic mammalian chitinase (AMCase) is produced in epithelial cells via a Th2-specific, IL-13-dependent pathway, and may act as an inflammatory mediator in asthma. The role of AMCase in chronic rhinosinusitis (CRS) has not been studied previously. METHODS: Eleven controls and 22 subjects with medically recalcitrant CRS were prospectively enrolled before undergoing endoscopic s

Methodologic assessment of studies on endoscopic sinus surgery.

BACKGROUND: Functional endoscopic sinus surgery (ESS) has become the procedure of choice for surgical treatment of chronic rhinosinusitis. Does the published literature support the claims of greater efficacy than medical treatment alone or older sinus procedures? OBJECTIVE: To analyze the methodology of the published literature regarding the efficacy of ESS. DATA SOURCES: MEDLINE search for primary studies published in 1987 through 2001, written in the English language, reporting results on more than 100 patients, using the MeSH (medical subject headings) terms sinusitis [subheadings surgery

The role of cytokines in rhinosinusitis.

Since the last decade, new insights into inflammatory processes have become possible by investigating the pattern of cytokines in acute and chronic sinus diseases. This review aims to update and discuss the findings of in vitro and in vivo studies concerning the role of cytokines in sinusitis and nasal polyposis. The proinflammatory cytokines interleukin-1beta, interleukin-6 and the neutrophil-chemoattractant interleukin-8 may play a major role in acute sinusitis, as shown in viral and allergic rhinitis. In chronic sinusitis interleukin-3 dominates the cytokine profiles, giving support to a va…

[Environment and nasal hyperreactivity].

The nasal mucosa is densely innervated by sensory nerve fibres which play an important part in the protection of the lower airways. Protective reflexes such as sneezing, secretion and nasal congestion can be elicited by exogenous noxious particles and numerous airborne irritants. The functions of the nasal mucosa is influenced at least in part, by biologically active neuropeptides that are released from sensory nerves endings. These sensory neuropeptides are involved in local vasodilatation, vascular permeability and mucus secretion. In addition, these neuropeptides are chemotactic for inflamm
