Effect of glenosphere lateralization with and without coracoacromial ligament transection on acromial and scapular spine strain in reverse shoulder arthroplasty.

BACKGROUND: Small changes in deltoid tension and moment arm due to glenosphere lateralization may be associated with an increase in acromion or scapular spine strain in reverse shoulder arthroplasty (RSA), which can lead to stress fracture. The coracoacromial ligament (CAL) may be protective and lower the strain seen on the acromion or scapular spine. This biomechanical study investigated the impact of glenosphere lateralization and CAL integrity on acromion and scapular spine strain after RSA. METHODS: Ten cadaveric specimens were tested on a custom dynamic shoulder frame. Acromial and scapu

Subcoracoid dislocation of the distal end of the clavicle. A case report.

Subcoracoid dislocation of the outer end of the clavicle in association with an acromial fracture is a rare injury usually produced by high energy trauma that moves the clavicle posterior to the conjoined tendon and inferior to the coracoid process. To restore the acromioclavicular joint, open reduction is needed. After the reduction is accomplished, the clavicle has to be stabilized with a coracoclavicular screw. The acromial fracture can be reduced and held with nonabsorbable sutures placed through the bone. To avoid osteolysis of the distal end of the clavicle and to be able to repair the t

[Injury to the Scapholunate Ligament in Distal Radius Fractures: Peri-Operative Diagnosis and Treatment Results].

PURPOSE OF THE STUDY Injury to the scapholunate ligament is frequently associated with a fracture of the distal radius. At present neither a unified concept of treatment nor a standard method of diagnosis in these concomitant injuries is available. The aim of the study was to evaluate a group of surgically treated patients with distal radius fractures in order to assess a contribution of combined conventional X-ray and intra-operative fluoroscopic examinations to the diagnosis of associated lesions and to compare short-term functional outcomes of sugically treated patients with those of patien

Minimally invasive polyaxial locking plate fixation of proximal humeral fractures: a prospective study.

BACKGROUND: The surgical treatment for displaced humeral head fractures overlooks a broad variety of surgical techniques and implant systems. A standard operative procedure has not yet been established. In this article, we report our experience with a second-generation locking plate for the humeral head fracture that is applied in a standardized nine-step minimally invasive surgical technique (MIS). METHODS: In a prospective study from May 2008 until November 2009, a cohort of 79 patients with 80 proximal humerus fractures were operated in a MIS procedure using a polyaxial locking plate. Foll

[Does the disability of shoulder, arm and hand questionnaire (DASH) replace grip strength and range of motion in outcome-evaluation?].

Scoring systems currently used to evaluate functional disabilities in upper extremity conditions frequently combine so-called "objective" parameters such as grip strength and range of motion with "subjective" parameters such as pain. Since its introduction, the Disabilities of Arm, Shoulder and Hand Questionnaire has proven useful in the pre- and postoperative evaluation of functional limitations of the upper extremity. This study examines the relationship between DASH scores, the so-called "objective" parameters of grip strength and range of motion and those of "mixed scores" such as Krimmer

Inhaled Methoxyflurane in Australian Emergency Departments-A Cost-Minimisation Analysis.

BACKGROUND: Around 60% of emergency department (ED) patients present with pain. With growing ED-demand, there is need for analgesic options that reduce resource use and cost. We estimate the potential cost-savings from broader use of inhaled methoxyflurane in Australian EDs compared with standard-of-care (SOC) analgesics or procedural sedation. The analysis focused on non-admitted patient presentations requiring analgesia or procedural sedation to treat dislocations, burns, fractures and other injuries. METHODS: We conducted a cost-minimisation analysis to assess differences in resource use a

An analysis of different reconstruction techniques for the radius: a finite element analysis.

Pathological fractures due to metastatic bone disease frequently require palliative stabilization. While polymethylmethacrylate (PMMA) provides immediate mechanical stabilization, its limited bending and multiaxial properties raise concerns. Reinforcement strategies such as plate fixation, intramedullary nails, and metallic augmentation have been proposed, yet their mechanical behavior under physiological loading remains insufficiently characterized. This study aims to evaluate the mechanical performance of three reconstruction strategies for a proximal radial defect: PMMA alone, PMMA combined

Comparative Outcomes of Surgical and Nonsurgical Treatments for Scapholunate Ligament Injuries With Concomitant Distal Radius Fractures: A Systematic Review.

Distal radius fractures (DRFs) are common, especially in elderly populations, and often occur alongside scapholunate (SL) ligament injuries (SLIs), which can progress to scapholunate advanced collapse if untreated. There is no standardized treatment for SLIs with DRFs, with options ranging from conservative management to surgical interventions. This review aims to evaluate functional, radiographic, and clinical outcomes of surgical and nonsurgical treatments for SLIs associated with DRFs. A systematic search was conducted across PubMed, ScienceDirect, and Embase according to International Pros

A survey of practice on the use of condition-specific patient reported outcome measures with patients who have distal radius fractures.

INTRODUCTION: Distal radius fractures (DRF) are common injuries seen by therapists. Patient Reported Outcomes Measures (PROMs) are a way to measure recovery, yet previous research suggests PROM utilisation is low amongst therapists. This study looks at current use of condition specific PROMs amongst UK therapists treating patients with DRF and explores possible barriers and facilitators to their use. METHODS: An online anonymised cross-sectional survey hosted on Qualtrics software was distributed to UK physiotherapists, occupational therapists, and therapy technicians via special interest gro

Efficacy and safety of volar locking plate versus cast immobilization for distal radius fractures: a systematic review and meta-analysis of randomized controlled trials.

Volar locking plates (VLPs) are increasingly used for distal radius fractures (DRFs), yet their efficacy compared to cast immobilization remains debated. This meta-analysis aimed to compare VLPs versus cast immobilization for DRFs across various follow-up durations. Randomized controlled trials reporting patient-reported functional scores, wrist range of motion (ROM), radiological assessments, and complications were included. Meta-analysis was performed for 6-week, 3-month, 6-month, 12-month, and >12-month follow-ups. Subgroup analysis stratified studies by age group, ≥ 60 years and < 60 years

Tumor-induced Osteomalacia Successfully Treated by En Bloc Tumor Excision and Reconstruction Using a Tumor-bearing Frozen Autograft: A Case Report.

BACKGROUND/AIM: Tumor-induced osteomalacia (TIO) is a rare pathology caused by overproduction of fibroblast growth factor 23 (FGF23). Its common clinical features include generalized muscle weakness, bone pain, and fractures. Complete resection of the offending tumor is the mainstay treatment. In this report, we present the first case of TIO by an FGF23 producing tumor treated using a tumor-bearing autograft treated with liquid nitrogen. CASE REPORT: A 63-year old female presented with generalized body pain, particularly in the left arm. The patient was diagnosed with a FGF23 producing tumor

Solitary Osteochondroma at Unusual Sites: A Case Report and Literature Review.

Osteochondromas (OCs) are bone lesions composed of cartilaginous and medullary bone capped with hyaline cartilage. OCs result from the separation of epiphyseal growth plate cartilage, pushing through the periosteal bone cuff. They commonly appear as pedunculated or sessile masses in the metaphysis of long bones and are the most common benign bone tumors. While rare in the scapula, OCs can occur there. Symptoms may arise from fractures, osseous abnormalities, or potential malignant transformation, especially in the presence of hereditary multiple exostoses (HME). The estimated rate of malignant

A Comparison of Rib Osteochondral Graft to Medial Femoral Trochlear Osteocartilaginous Graft for the Salvage of the Fragmented Scaphoid Proximal Pole: A Single-Center Experience with Minimum 2-Year Follow-Up.

Purpose  Fragmented fractures of the proximal pole of the scaphoid that cannot be internally fixed may be salvaged by four-corner arthrodesis or proximal row carpectomy. Replacement of the proximal scaphoid with costal-osteochondral graft (COG) or the medial femoral trochlea (MFT) graft are two alternative solutions for this clinical presentation. The purpose of this study was to compare the clinical and radiographic results of the COG and the MFT graft with a minimum 2-year follow-up from a single centre. Methods  A retrospective study was performed to investigate the outcome of COG and MFT w
