Two stage treatment of a proximal humeral fracture-dislocation with vascular injury: Case report of a multidisciplinary approach.

Proximal humeral fracture-dislocation associated with neurovascular injury is rare events, associated with poorer outcomes and higher risk of complications. A multidisciplinary approach including the orthopaedic and vascular department is essential in treating such kind of injury. The goal of the treatment is to restore the vascular supply and stabilize the fracture. Usually the orthopaedic surgical stabilization provides a stable substrate for the vascular repair. We report a case of 70 years old woman who sustained a 4 part proximal humerus fracture-dislocation with vascular injury at the le

Isolated C5-C6 avulsion in obstetric brachial plexus palsy treated by ipsilateral C7 neurotization to the upper trunk: outcomes at a mean follow-up of 9 years.

Cervical root avulsions are the worst pattern of injury in obstetrical brachial plexus injury (OBPI). The prognosis is poor and the treatment is mainly surgical with extraplexual neurotizations or muscle transfers. We present the outcomes of a technique performed in our institution to treat C5-C6 avulsion in obstetrical brachial plexus injury. This technique consists of a total ipsilateral C7 neurotization to the upper trunk. Ten babies with isolated C5-C6 root avulsion were operated on; we were able to review nine of them at over 12 months follow-up. The shoulder and the elbow function were a

Is systematic long-term sonographic follow-up after arthroscopic rotator cuff repair useful in asymptomatic patients?

OBJECTIVE: To establish whether ultrasonographic (US) appearance is correlated to functional outcome in asymptomatic patients 12 months after rotator cuff repair. MATERIAL AND METHODS: 26 asymptomatic patients at twelve months after arthroscopic cuff repair and 26 controls were retrospectively included. A clinical score (Constant's Shoulder Score, CSS) was compared with a US score (the modified Sugaya Score, mSS). The minimal thickness of the anterior and posterior parts of the cuff was also measured. RESULTS: CSS improved significantly from preoperatively to postoperatively (mean increased

Reconstruction of rotator cuff tears in wheelchair-bound paraplegic patients.

BACKGROUND: Rotator cuff surgery in wheelchair-bound patients is challenging, and clinical data on this condition are limited. We hypothesized that rotator cuff repair in these patients might improve functional outcomes. METHODS: In a retrospective study, data on 13 paraplegic patients (8 men and 5 women; 16 shoulders) who underwent rotator cuff repair were analyzed. The average age at the time of surgery was 48.7 years. The causes of paraplegia were poliomyelitis in 9 patients, spinal fracture in 4, and cerebral infarction in 2. Open rotator cuff repair was performed in 11 patients and arthr

The passive distraction test: a new diagnostic aid for clinically significant superior labral pathology.

PURPOSE: The purpose of this report is to present a new provocative maneuver, the passive distraction test (PDT), as an examination tool to be used in the evaluation of patients thought to have a SLAP lesion and to compare its accuracy, precision, and reproducibility alone and in conjunction with previously published maneuvers. METHODS: A retrospective analysis of 319 consecutive arthroscopies performed between May 2001 and November 2003 was performed. A total of 65 cases were excluded, 53 because of limitation of elevation to less than 150 degrees or pain in the starting test position and 12

Suture anchor and percutaneous intra-articular transtendon biceps tenodesis.

Several arthroscopic biceps tenodesis techniques have been described for surgical management of tendonitis and/or partial thickness tears of the long head of the biceps brachii tendon resulting in recalcitrant anterior shoulder pain. This chapter describes an arthroscopic tenodesis using percutaneous intra-articular transtendon technique with suture anchor fixation. The percutaneous technique allows excellent access to the biceps tendon, and the addition of a suture anchor provides superior fixation to isolated soft tissue fixation.

Upper-limb kinematics during push and pull configurations of a dual-propulsion wheelchair in individuals with spinal cord injury.

OBJECTIVES: This study compared upper-limb kinematics and propulsion patterns between the push and pull propulsions of a detachable dual-propulsion wheelchair in individuals with spinal cord injury. METHODS: Sixteen men with spinal cord injury performed 10-m straight-line trials at a self-selected speed under laboratory conditions. Gear ratios of 1:1 and 1:2 were applied to the push and pull propulsion, respectively. Upper-limb and trunk movements were recorded using a 10-camera Vicon motion capture system. Propulsion time and speed, shoulder and elbow ranges of motion, propulsion angle, trun

Neuromechanics of finger hangs with arm lock-offs: analyzing joint moments and muscle activations to improve practice guidelines for climbing.

INTRODUCTION: Climbing imposes substantial demands on the upper limbs and understanding the mechanical loads experienced by the joints during climbing movements is crucial for injury prevention and optimizing training protocols. This study aimed to quantify and compare upper limb joint loads and muscle activations during isometric finger hanging exercises with different arm lock-off positions. METHODS: Seventeen recreational climbers performed six finger dead hangs with arm lock-offs at 90° and 135° of elbow flexion, as well as arms fully extended. Upper limb joint moments were calculated usi

Biofidelity Assessment of the WIAMan Thorax by a Comparative Study With Hybrid III, THOR, and PMHS in Frontal Sled Testing.

The Warrior Injury Assessment Manikin (WIAMan) anthropomorphic test device (ATD) has been originally developed to predict and prevent injuries for occupants in military vehicles, in an underbody blast environment. However, its crash performance and biofidelity of the thoracic region have not been explored. The aim of this study was to determine and evaluate the WIAMan thoracic responses in a typical frontal sled test. The 40 kph frontal sled tests were conducted to quantify the WIAMan thoracic kinematics, chest deflection, and belt loads. Comparative biofidelities of the WIAMan thorax and othe

Usefulness of ultrasonography for diagnosing iatrogenic spinal accessory nerve palsy after lymph node needle biopsy: a case report.

BACKGROUND: Spinal accessory nerve (SAN) palsy is rare in clinical settings. Iatrogenicity is the most common cause, with cervical lymph node biopsy accounting for > 50% of cases. However, SAN palsy after lymph node needle biopsy is extremely rare, and the injury site is difficult to identify because of the tiny needle mark. CASE PRESENTATION: A 26-year-old woman was referred to our hospital with left neck pain and difficulty abducting and shrugging her left shoulder after left cervical lymph node needle biopsy. Five weeks earlier, a needle biopsy had been performed at the surgery clinic beca

Marker-Based Method for Analyzing the Three-Dimensional Upper Body Kinematics of Violinists and Violists: Development and Clinical Feasibility.

AIMS: High string players (violin and viola) often suffer from musculoskeletal disorders. Although 3D motion analysis has proved helpful in diagnosing different musculoskeletal syndromes and identifying injurious movement patterns in violin and viola performance, more detailed analyses of upper body movement strategies and especially of the shoulder complex have not yet been recorded. The use of spherical surface markers on some anatomical landmarks is, however, inappropriate when an instrument is being played. The aim of this study was to develop and evaluate a novel marker-based method for a

Outcomes of Early Use of an End of Range Axilla Orthotic in Children Following Burn Injury.

Scar contracture is a significant complication of burn injury. This study aimed to describe outcomes of early axilla orthotic use at end of range abduction in children, following a burn to the axilla region. A retrospective review of 76 children (mean age 3.9 years [SD 3.6]) treated at a tertiary children's hospital from 2006 to 2016 was conducted. No child developed axilla contracture for the duration of the 2-year study follow-up with no adverse events recorded. If orthotic use was ceased <60 days post-burn, it was considered not an essential intervention to maintain range of movement, leavi

Ergonomic Microscope: Need of the Hour.

BACKGROUND: Prolonged use of conventional microscope develops musculo-skeletal injuries like chronic pain syndrome, including shoulder, neck, back aches & fatigue. Since the problems go unnoticed, the injuries can lead to some serious permanent damages. This further leads to a compromise in the health and welfare of the person and the institute. Hence, an understanding about the ergonomics is the need of the hour in this postmodern era. Inspite of few studies and surveys about ergonomics, there is still a steep rise in the musculoskeletal disorders. AIM OF THE STUDY: The aim of our study was

Phenotypic divergence in rotator cuff tear and volumetric muscle loss mouse models following fibroadipogenic progenitor depletion.

AIMS: Fibroadipogenic progenitors (FAPs) are a group of resident muscle stem cells capable of differentiating into fibroblasts and adipocytes, contributing to intramuscular fibrotic and fatty degeneration after injury. However, FAPs are also thought to play a crucial role in muscle regeneration by facilitating satellite cell myogenesis. Despite this dual role, the precise functions of FAPs in muscle degeneration and regeneration remain unclear. This study aimed to utilize a FAP depletion mouse model to define the role of FAPs in two distinct, clinically relevant injury models of rotator cuff t

Considerations of Throwing Athletes With Upper Extremity Injury When Completing the Single Assessment Numeric Evaluation (SANE): A Report From the Athletic Training Practice-Based Research Network.

CONTEXT: The Single Assessment Numeric Evaluation (SANE) is a widely used patient-reported outcome (PRO) measure that provides an efficient but limited view of patient perceptions of health. Knowledge of what throwing athletes with upper extremity injury consider when answering the SANE would inform score interpretation and increase its value for clinical decision-making in this patient population. OBJECTIVE: To investigate the global rating of the SANE and its ability to capture constructs of health reflected in PRO measures that are commonly used in throwing athletes with upper extremity sp
