Evaluating endoscopic and endoscopic-assisted access to the infratemporal fossa: a novel method for assessment and comparison of approaches.

OBJECTIVES/HYPOTHESIS: Endoscopic infratemporal fossa (ITF) surgery is a growing clinical interest. This study presents a method of analyzing approach access and visualization, identifies relevant anatomy in an endoscopic approach to the ITF, and compares endoscopic medial maxillectomy (MMA) and endoscopic-assisted sublabial transmaxillary (SLT) approaches to the ITF as a model for this paradigm. STUDY DESIGN: Human cadaver anatomic study. METHODS: Five human cadaver heads (10 ITF dissections) were used. An SLT and MMA were performed on each side. For endoscopic dissections of the ITF, 0° an

Phenotypic characteristics of hydrocephalus in stillborn Friesian foals.

Hydrocephalus is uncommon in horses. However, in recent years, it has become clear that the prevalence of hydrocephalus is greater in Friesian horses than in other breeds probably due to their limited gene pool. Before identification of candidate genes that predispose to the development of hydrocephalus in Friesian horses can be pursued, an in-depth, phenotypic, pathological description of the condition in Friesians would be of great benefit. Our study aimed to characterize the morphology of hydrocephalus in Friesian horses, to support further investigation of the genetic background of this co

Transoral anatomy of the tonsillar fossa and lateral pharyngeal wall: anatomic dissection with radiographic and clinical correlation.

OBJECTIVES/HYPOTHESIS: To evaluate the transoral anatomy of the tonsillar fossa and lateral pharyngeal wall and to correlate these findings … ts who underwent contrast-enhanced computed tomography (CT) of the neck for a variety of endoscopic skull base procedures were selected. Measurements were taken from the lateral pharyngeal wall at C2-C3 interspace and greater horn of hyoid (C6) to the external carotid artery (ECA). RESULTS: The glossopharyngeal (IX) nerve was consistently identified deep to …

Plasticity of the endocranial base in nonsyndromic craniosynostosis.

Limited in vivo data exist on the dysmorphology of the cranial base in nonsyndromic craniosynostosis. Few studies have documented the effect of calvarial surgery for synostosis on endocranial morphology. Previous work has suggested that the dysmorphology of the endocranial base is diagnostically specific for metopic, sagittal, and unicoronal sutures. The purpose of this study was to further evaluate the endocranial base in infants with nonsyndromic craniosynostosis by testing the hypothesis that the dysmorphology is, to some degree, a secondary deformation rather than a primary malformation. Aneurysms of the vertebro-basilar junction area: preliminary experience in endovascular and surgical management.

Aneurysms of the vertebro-basilar junction area have been considered the most difficult to be surgically treated because of their deep location, the proximity of the brain stem and the cranial nerves. However, at present, new endovascular techniques and new transbasal surgical approaches offer valuable management strategies. This paper concerns six consecutive patients whom we managed either endovascularly or surgically during a period of eighteen months. Direct surgical treatment was offered to two young patients with relatively small aneurysms in good neurological condition, whereas the othe

Inadvertent intracranial insertion of a nasogastric tube in a non-trauma patient.

Complications following nasogastric intubation in patients with basal skull fractures are well documented. This report is of a rare cause of inadvertent intracranial placement of a nasogastric (NG) tube in a non-trauma patient. The patient subsequently died. The use of NG tubes, their place in airway management, and lessons to be learned from this case are discussed.

Pattern of malformations in the axial skeleton in human trisomy 18 fetuses.

We examined and described the development and abnormalities of the axial skeleton in 10 human trisomy 18 fetuses. Whole-body radiographs and radiographs of midsagittal tissue blocks of the cranial base and the spine were studied. In 3 fetuses no spinal radiographs were available. Seven osseous regions or fields along the body axis were analyzed, four in the spine, and three in the cranial base and nasal bones. Malformations occurred in the occipital field in all fetuses. This was a characteristic notching, either unilateral or bilateral, of the basilar part of the occipital bone. Nasal bones w

High cervical pain and impairment of skull mobility as the only symptoms of an occipital condyle fracture. Case report.

Occipital condyle fractures are rarely reported in survivors of trauma. Most cases involve patients with a major head trauma, lower cranial nerve palsy, and/or suspected lesions demonstrated on plain x-ray films of the skull or cervical spine. The authors describe a traffic-accident victim in whom an atlanto-occipital joint lesion was suspected based only on mobility investigation of the skull. Axial high-resolution computerized tomography of the atlanto-occipital joint showed a fracture of the right occipital condyle.

Skull, patella and thigh cryptococcosis after a crashing injury of the temporal bone.

An unusual case of triple location (skull, patella and thigh) Cryptococcus neoformans is described. The peculiarity of the case is based on the probability of direct post-traumatic bone inoculation, subsequent seeding from skull to patella, thigh and cerebral spinal fluid (CSF), patella involvement, lack of evidence of lung involvement and lack of predisposing underlying disease. The response to surgery and a combination of amphotericin B and 5-flucytosine administration was favorable without relapse at follow-up after 7-year discontinuation of therapy.

Complex single step skull reconstruction in Gorham's disease - a technical report and review of the literature.

BACKGROUND: Gorham's disease is a rare osteolytic disorder characterized by progressive resorption of bone and replacement of osseous matrix by a proliferative non-neoplastic vascular or lymphatic tissue. A standardized treatment protocol has not yet been defined due to the unpredictable natural history of the disease and variable clinical presentations. No single treatment has proven to be superior in arresting the course of the disease. Trials have included surgery, radiation and medical therapies using drugs such as calcium salts, vitamin D supplements and hormones. We report on our advanta

Extradural anterior transpetrosal approach: how I do it.

BACKGROUND: Petroclival and brainstem lesions remain challenging to access due to their deep-seated location and proximity to critical neurovascular structures. METHOD: We describe the step-by-step technique of the standard extradural anterior transpetrosal approach, and its extensions, illustrated through cadaveric and surgical illustrations. A surgical video demonstrates its application in a patient with a petroclival meningioma with posterolateral extension. CONCLUSION: The extradural anterior transpetrosal approach provides a direct « looking-down» trajectory to petroclival lesions with

Supraorbital ethmoid cells (SOECs), anterior ethmoid artery notch and ethmoid roof relation in PNSCT.

OBJECTIVES: We investigated supraorbital ethmoid cell (SOEC) presence and types in paranasal sinus computed tomography (PNSCT). METHODS: Th … otch-ethmoid roof distance increases, AEA runs inferiorly in the ethmoid cells and freely below the skull base; and is more susceptible to injury. The surgeons should be more careful not to damage AEA in the FESS when detecting well-pneumatized SOECs (SOEC Type 3).

Multimodal Use of Contact Endoscopy in Neurosurgery: Case Series with Technical Note and Literature Review.

BACKGROUND: Originally adopted for the cytological screening of cervical and uterine cancer, contact endoscopy (CE) is now widely used in several fields of oncological surgery. The CE method, with magnification power up to 150x, was designed to enhance visualization and identify microscopic changes indicative of precancerous and cancerous lesions at early stages. In this pilot study, we evaluated the multimodal applications of CE during different endoscopic intracranial neurosurgical procedures. METHODS: Twenty patients with skull base lesions underwent surgery using different minimally invas…

Retrosigmoid Transtentorial Resection of a Petroclival Meningioma: 2-Dimensional Operative Video.

In this video, we aimed to demonstrate retrosigmoid-transtentorial resection of a 4.5 × 4 × 4.5 cm3 left-sided petroclival meningioma compressing the brainstem in a 62-yr-old male who presented with decreased hearing on the left and imbalance.  The patient was placed in park-bench position and a left-sided suboccipital craniotomy was performed. The mass was severely compressing the brainstem as well as the cranial nerves V, VI, VII, and VIII while IV was encased by the tumor. The circumferential dissection of the tumor was facilitated while the surgical corridor and exposure were enlarged by i

Evolution of Pituitary Surgery.

Pituitary tumors may well be the most common brain tumors with a mean incidence of 16.7%. Even small tumors become symptomatic when they arise from functioning cells and produce devastating effects on the body. The nonfunctioning tumors may become quite large before producing symptoms due to raised intracranial pressure or mass effect on the surrounding structures, most commonly, the optic apparatus. Many of them remain asymptomatic through life. Evolution of pituitary surgery is testimony to the advances in diagnostic and surgical techniques in neurosurgery and improved understanding of 360°
